Hello lybrate user, leg pain after running is common due to spasm in the leg muscles known as tibialis anterior which functions to pull the foot towards your body during walking, running. When a person runs or exerts an unaccustomed stress there occurs production of lactic acid in the muscle which is quite painful. Relief by: rest ice application stretching exercises will help a lot.

I am suffering from lateral knee pain in left leg from 1 month. No muscle pull, no injury, my hb is 16.3 and no diabetes and no sugar. I am perfectly ok. Please tell me cure and cause.

Hi lybrate-user it seems to me as iliotibial band syndrome as it rubs against outside of the knee joint Tight muscles in your hips or along the side of the leg can be a major contributing factor to IT band syndrome. All of these tissues are connected, so even though the location of the pain is in the knee, the hips can very well be the area that need the most attention. Weak hip muscles can be one of the biggest reasons people get IT band syndrome.  To strengthen the muscles most commonly affected, you can try some of the exercises suggested . Further evaluation like walking video analysis and shoe or orthotic issues need to see and give you the best treatment. Hope this helps you. Regards.

From past two days I am having severe neck pain with light headache and vomiting. Is cervical spondylosis causing this and what are the methods to avoid it.

COMT, MPTh/MPT
Physiotherapist, Surat
Yes if upper cervical vertebrae are involved then you may have symptoms like headache, dizziness and neck pain. In physio we have a treatment for cervical spondylitis so I would recommed you to consult any physio and start taking physio modalities and manipulations to reduce pain and increase mobility. I have specially designed one program as I-care specially in cases of neck and back pain which I have pratically applied on many of my patients and received a very good result. The treatment include: 1. Ice massage - Take ice pain perform massage over the area which is painful and have swelling. Do “to” and “fro” motion. Apply it for 8-10 minutes and then relax your body for another 3-5 minutes. 2. Deep massage - Using your finger tips start kneading your affected area. 3 finger kneading or 2 finger kneading or you can use thumb also for kneading. Once done with kneading again relax your body for 3-5 minutes. 3. Now apply any pain relieving ointment like volini or oraflame etc. Do not massage with the ointment. Just apply it gently involving all the area. Do not penetrate it. Once done with application again relax your body for 3-5 minutes and start performing exercises. Exercises: Because of pain and stiffness your neck range starts decreasing so with the entire treatment program we can help you to make your neck mobile and reduce pain to live happily. 1. Position - Sitting Bend your head forward until you feel stretch behind your neck. Hold for 5-10 secs. And relax. Repeat 5 -10 times. Perform same by taking neck backward and then side ways. Perform the exercises slowly until a stretch is felt. Do not over stretch, perform slowly and gently. 2. Position - Sitting Isometrics: Infront of mirror. Place your both hands on your forehead. Apply contradict pressure from hands towards your head and with your head towards your hands. There should not be any neck moment. Instead an isometric contractions should be there in neck. Hold for 5 sec and relax. Repeat it for 5-10 times. Perform same by placing your hands on the back of your head and giving isometric contractions to make posterior muscles stregthening. Then same as with placing one hand on right side and giving pressure from hand to head and vice versa. 3. Position - Sitting Stretching - Bend your head forward and and then gently pull your head further by applying pressure from your hands. A stretch is felt on back of your neck. Same a stretch your side neck muscles. 4. While exhaling push your upper rib down with your one hand and then tilt your head and give a slight stretch by rotating your head to opposite side. Hold for 5 sec and then relax. Repeat for 5-10 secs. 5. Neck and back straight, keep your chin in. Hold for 5 sec and feel a stretch in your neck. Relax. Repeat for 5-10 times. 6. Lie on bed with towel roll under your neck. Press the roll into the floor by using your neck muscles. Hold for 5 sec and relax. Perform 5-10 times. Advice - 1. Use very thin pillow while sleeping. 2. Do not sit for long time with your neck forward bending. 3. Lying on bed and reading book or newspaper should be avoided. 4. If there will be any pain while performing exercises or headache stop doing exercise and consult physio right away. 5. Sit in erect posture without bending your back or neck. 6. Position your desktop so that it should be just infront of your neck you do not have to bend your hands or neck. Please contact me if you face any problem while performing the I-care at home. I am sure you will get a relief in your pain as I have performed I-care in many patients and have received a good response. Please mail me at swatisagar121@gmail. Com if you face any problem in understanding any point. Ill reply you with the exact pictures showing how it can be done. There are many mobilisation techniques that can only be given by physio. You cannot do those at home. But try I-care program I am sure you will get a relief. Thanks and Good Luck. Get well soon.

Partial And Total Knee Replacement Surgery

Hi!

This is Dr. Hitesh Kubadia, I’m an orthopedic and joint replacement surgeon, specializing in minimally invasion joint replacement surgeries using the muscles sparing approach. I also do a lot of partial knee replacement i.e. called a unicompartmental knee replacement which is the next thing in trying to preserve as much as natural tissue as possible.

I’m here to explain something about osteoarthritis of the knee which is a very common problem faced by lot of elderly population and it disabled them in day to day life. These are the small models of the knee here so that I can explain it to you all and you all can understand what is the actual problem here. Now if you look at this is the knee model so if you look at the knee the knee is made up of a lower portion of the femur and upper portion of the tibia, so this is covered by a cartilage here, this blue thing which is marked up here is the cartilage and this is a very smooth glistening surface which allows for frictionless movement across the knee. OK.

As we age as we creates strains in this cartilages, these cartilages develop some bumps in their surfaces. So this surface doesn’t become as smooth as it once was. As we age as the friction keeps on increasing with a period of time this gets worn out completely. So much so that the underline bone gets starting visible and this causes tremendous frictions on your movement and causes pain. As people still pull on and then what happen this all the 3 compartments of the knee, the medial compartment, the latter compartment and the front compartment and the inner compartment the outer compartment and the compartment which is in the front all get worn out.

.So this causes pain and disability and pain for the patient and effects the day to day activities of life. Osteoarthritis has various forms of treatments, initially, everything is nonsurgical so when you try all those non-surgical methods to relief so that your life continues with lot of more than ease. But when these non-surgical methods like injections in the knee or physical therapy and injections in the knee start failing to give you enough relief then we need to think about surgical options.

So when we see on the x-ray on the clinically examine the patient and we see only one compartment of the knee which is damaged and the other part of the knee is very healthy it makes sense to do only you know change only that compartment of the knee which is actually causing the problem for the patient. So what is done here, as you look at this model is the friction between only in this compartment of the knee the other compartment of the knee is nice and smooth so what is done is only this compartment is changed. So what is changed with is you just resurface the area which is actually damaged and the resurface the lower portion that is damaged so which we removed this portion of the lower portion of the tibia and what is replaced by a small alloy of cobalt chrome and other cobalt chrome alloy at the plate this plate and medical grey plastic is inserted between the two surfaces and this movement becomes more than glistening over the period of time.

So this gives relief and gives fantastic relief again this is a minimally invasive surgery and since all the ligaments and the part of the knee is preserved and they are natural and they are glistening so the feel of the knee is much more natural and we have now enough reports across the world. These things lasting for even 20 years there is actually slowly passing the results of a traditional total knee replacement. We just spoke about the partial knee replacement but when we see the arthritis is confined to all the three compartments so this is no point in changing only the one part of the knee so that’s we come to the total knee replacement. So that’s what gives the patient long-term relief.

Again this done through a minimally invasive approach, muscle sparing approach so that the recovery time is much faster and much shorter. Right! So here we look at this model see this the whole knee is damaged the inner compartment the outer compartment the whole knee is damaged. So what is done here? Again resurfacing part so only the damaged portion of the cartilage and bit of bone is removed like this and we changed the whole thing off, and this is replaced with a cobalt chrome allow base plate and a medical grey plastic. Again on the femur side again cobalt chrome alloy which is fitted across. This is fitted to the bone with bone cement and this gives you lasting movements and comfortable movements across life. If you are or your family members face knee pain or hip pain in day to day life and they need a consultation with me they are most welcome to contact us through HKS Clinic or through lybrate.com
