Good morning, I’m Dr Nidhi Agarwal, I am a consultant gynaecologist, based in Gurgaon. Now today I’ll speak on Polycystic Ovarian Disease that is PCOD which is very common in women in recent years.

PCOD actually occurs in young women. They usually present to us with irregularity with their periods. The ovaries in PCOD patients actually become bigger and denser. As the name suggests, ‘poly’ means many that is many cysts are formed in the ovaries. This lady usually presents with a disruption in the normal menstrual cycle.

Now, what happens in a normal lady who menstruates every month?

There is one egg which is selected by nature which increases in size, and when it becomes bigger and appropriate in size, it is ruptured in the ovary which is then taken up by the Fallopian tube. And if it doesn’t meet the sperm and pregnancy doesn’t occur, then the lady menstruates. This should happen monthly up to 35 days. In PCOD patients, this natural selection of the egg every month doesn’t occur. Instead, many small cysts are formed in the ovaries. So that is why there are many cysts formed in the ovaries, and then the patient does not have normal periods. That is why we say it a Polycystic Ovarian Disease.

Now how does a patient of PCOD present to us?

A patient of PCOD, if she is a young girl in her teens or in the early twenties, she usually comes to us saying that she is not having her monthly periods. She menstruates at one and a half months, two months, two and a half months, and when she menstruates, she usually has excessive bleeding with clots. If a lady is in her late twenties and has got married, and she is not able to conceive, then also the patient presents with the problem in conceiving. Also, there is usually an excess of hormones produced by the ovaries, that is an excess of estrogen as well as androgen which is the male hormones. The lady usually presents with male like pattern of hair growth that is hair growth on her chin on her face, around the nipples of her breasts, in her lower abdomen, especially over her thighs and she says that along with excessive hair growth, she has developed excessive acne and things also appear all over her scalp. These are basically because of an excess of androgens, which are the male hormones. This change in the hormonal value occurs because of Polycystic Ovarian Disease. Also, very often, patients come to us saying that she is gaining weight excessively. A patient comes and says that in the last 5 months, I have gained 10 kgs of weight. So patients usually present to us with symptoms of insulin resistance, in which there is overweight. Now, PCOD has actually become very common and patients who are as small as 11-12 years of age come to me, young girls presenting with PCOD. And the incidence is very very very high; 1 out of 10 women are now having PCOD.
How do tackle this PCOD?

See, there are three criteria which are very important. As I have told you, first is a presentation, that the lady is not ovulating, that is the egg which is formed every month is not released. Instead, multiple eggs multiple cysts form which we call in their ovaries. So, if the ultrasound shows that they are polycystic ovaries if the patient presents to us with less ovulation, or anovulation which we say which occurs when the patient is not having her regular periods, or there are signs of excess androgen that is male hormone with features of hirsutism. Then, out of these three, if two criteria are there in the lady someone is having PCOD.

Very often patients ask me, ‘Why do we, why am I suffering from PCOD?’ So if you ask me, there is no one particular cause which I can see, that this is the reason you have got. There are so many factors, it could be just genetic- your mother or sister has been suffering from PCOD, then your chances of having PCOD are always higher. There could just be low-grade information in her ovaries, which has disturbed the ovaries to an extent that it is releasing more of male hormones, making them bigger in size and that is causing the ovaries not to ovulate. Or it could just be because of obesity and high insulin levels. Again, because of high insulin in the body, it puts stress on the ovaries and they don’t ovulate.

So there are so many causes, you have to investigate the patient, there are a lot of investigations that have to be done and we can come to a diagnosis, it is possible. So, I’ll just conclude by saying that PCOD is very very common nowadays.

Use barrier method i.e. Condoms now to avoid pregnancy. But the best contraceptive for a newly -wed is using oral contraceptive pills. So, go to a gynaecologist who will get a few tests done before prescribing you oral pills which are to be started from first or second day of your periods. Till that, use condoms.

My wife aged 25 years is suffering from PCOD problem. She is getting treatment from Gynecologist since 8 months. Nearly 10 tests have been done to diagnosis the exact problem like FSH, LH, TSH, Prolactin etc. And found results are within good limit. Still Eggs are not maturing due to multiple cysts around the ovary. She has been doing regular exercises, yoga etc. She is prescribed for Myocyst M tabs for 2 months twice daily. And after that, CCQ 100 tabs for D2 to D6 & Valest 2 tabs for D7 to D12 and USG (Ultrasound) suggested, which resulted good and provided Hucog HP 5000 injection. Again suggested for being in contact for next 5 days. What is the chance for her eggs maturation. Please suggest.

With all the treatment your wife has taken, it is sure that she must have had formed good and mature eggs and that is why your doctor must have given her HUCOG injection. So, don't stress. Trust your doctor and your wife will conceive in due course of time. Since the eggs have formed in this cycle, it doesnot mean that she will conceive definitely in this cycle (chances of conception are 15--20%). So, don't stress and give time to yourself as well as the doctor and your wife will definitely conceive.

My wife suffers with vaginal infection .now it's almost cured with one year course. But prolactin level is high approx 25 .it keep fluctuating. She used cablogomin tablet as per Doctor prescription to lower prolactin. But is der any chances of baby conceive .I m too tense. IVF is possible or not in this case.

Take cabgolin as advised by your doctor to lower your prolactin levels. Once it is in normal range and there are no other factors affecting your wife's fertility, she will conceive without any problem.

My wife TSH was very high (9) during her 5th week of pregnancy and she has been prescribed with Thyronorm 88 Mcg. Now after taking the pills for last one and half month, her TSH level is extremely low (0.021), whether she need to stop the medicine.

No, please don't stop thyronorm. Go to your doctor who started with this medication and she/he will decrease the dose of thyronorm so that TSH remains less than 3.Nothing to worry.

It is not necessary that you will have periods after taking these pills but your periods can be irregular after taking this tablet.Use condom in every act of intercourse in this cycle. Don't stress as nobody can tell that when you will have your next periods as they may happen early or they may be delayed.

Do a urine pregnancy test and if it negative, wait for your periods for one week. If still after one week your bleeding does not start ,then see a gynaec. However, if your test is positive, then see a gynaec immediately.

You will probably start with your periods soon. Try to lose 5-6 kg weight and if your periods are regular, try to do intercourse between 12th to 18th day of your periods. Take one tablet of 5 mg folic acid daily. If you have been trying to conceive since 6--9 months, then there is no harm in seeing a doctor who can guide you better with face to face conversation.

Baby movements in first pregnancy usually start between 18-20 weeks. So, you too will be feeling these movements when your time comes i.e. In 2 -3 weeks time.

Hello .my wife have used the MTP kit and got abortion. And we have undergone ultra scan in that the there is a blood endometrial canal. And then doctor suggested the cytolog at 29/04/17. And one on 30. Night 10: 30 she used it but no bleeding is started.

You don't have to worry. The blood in the endometrial cavity will get absorbed if it has not come out after taking cytolog. Do a urine pregnancy test after 1 week and if it is negative, then all is well.

Good morning doctor. Before a year I had a pcod problem. But from past 6 to 8 month I am getting my period regularly. But this month I don't get periods. I am worried. I also did pregnancy test, it's negative. Kindly advise.

May be that you still have PCOD which is responsible for delay in periods. Repeat your urine pregnancy test after 1 week and if it is still negative, you go and see a local gynaecologist who will start you with the treatment. Reduce your weight.

If IgG is positive that means that you have suffered from infection in the past and you are immune to it. So, you need not worry about it at all. If IgM is negative it means that you don't have any recent infection and so you and your baby are doing well. Relax!

Ask her to eat a diet which is rich in proteins like eggs, non-vegetarian food twice weekly and if she is a vegetarian, lots of pulses like black gram and rajma, fruits, paneer, tofu, dry fruits etc. She should take 6-8 dates everyday and ghur i.e jaggery after every food. One glass of lime water is also good for her. A good iron tablet or syrup alongwith multi-vitamin tab is advised for at least a month. She should improve with good diet and iron and multivitamin tablets. If she doesnot feel better after 10 days of this treatment, she should consult a physician for the same.

Yes, you can become pregnant even if you have irregular periods.Use barrier contraceptive i.e condom to avoid pregnancy as well as sexually transmitted diseases.Your chances of getting pregnant are minimal if the condom doesn't slip or get torned during the act.

Now I am 18 weeks 1 day pregnant. My belly is not as much big now but my GP told to take NT scan. I took that scan when I am 15 weeks. The result of the scan is everything normal the nuchal translucency measure is also normal. But why belly is not big enough that's my doubt. What I thought is my baby's growth is normal or not.

I think that you need to change your doctor. He is advising you to go in for NT scan which is done between 11 weeks to 13 weeks 6 days of pregnancy. At 18 weeks, there is not much protusion of belly. It will anyways be visible at 6 months of pregnancy. At 18 weeks, we get Level -2 scan done to look for any gross congenital malformations in the baby. Go for this scan.

These tablets are natural micronised progestrone tablets which are meant for maintainence of pregnancy. You have twin pregnancy and therefore you have been advised to take them so that your uterus remains relaxed and the babies grow well inside. The vaginal route is the preferred mode for micronised progestrone. So, keep these tablets vaginally. All the best and take care.

Seek the help of a local gynaecologist who will give you hormonal tablets to support your pregnancy. Although let me tell you frankly that chances of continuation of pregnancy are only 50% if bleeding occurs during pregnancy. Get your repeat ultrasound after 10 days. If the fetal pole and yolk sac and fetal heart beat, then it is a good sign and your pregnancy will do well. All the best.

I am having uterine fibroids as per diagnostic reports. So what to do right now? Their size is 35 x 29 mm approx. And other one is 18 mm diameter.

If you do not have any symptoms like excessive bleeding or irregular bleeding or pain during periods , then the fibroids can be managed conservatively. The size of the fibroids is not important rather it is the symptoms of the patient that will define the treatment.If the fibroids have been picked up as a routine ultrasound finding, then you can follow up with regular ulrasounds every 6 months to make sure that there is no abnormal increase in the size of the fibroids. If the fibroids cause excessive bleeding , excess pain (which cannot be managed with medicines) or increase abnormally in size then only you need to undergo surgery. Also the fibroids tend to decrease in size after menopause.

If the parameters of your height and weight are mentioned correctly then I must say that you are obese and you need to lose weight. You have PCOD and hormonal imbalance due to excessive weight and vice- versa. Your aim should be reduce weight at the rate of 2--3 kg per month and lose 12--15 kg weight over a period of 6--8 months. Go to a gynaecologist who can counsel you about the importance of weight loss and start you with medicines for treatment of PCOD. At the age of 23, you can wait for 6-9 months for normalization of your hormones and then plan a pregnancy.

I have one kid (4 years old) While delivery in cesarian Fibroid taken out and tested which is normal Fibroid. But after 4 years, again Fibroid appears in uterus. Due to that heavy bleeding in periods and Head ache. Doctor said operation to be done. Is there any medicine to decrease the Fibroid size. We want one more child, conceive is possible without removing the fibroid. Is there any possibility to remove that without surgery. Pl clarify.

If your fibroid is very big and your bleeding can not be controlled with medicines, then you might need a myomectomy i.e removal of fibroid. First try a conservative treatment with medicines but I think that your gynaecologist is the best to guide you for this problem.
