Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take BioD3 Max 1 tab dailyx10 Paracetamol 250mg OD & SOS x5days Do back(spine)/shoulder/knee exercises Make sure you are not allergic to any of the medicines you are going to take Do not ignore it. I have started morning running+walking from yesterday. And when I go today and started running then I was getting pain in my leg muscles. I was not able to run and only walk slowly and come back. But I want to continue it don? t want stop but also I want to release from this pain every day. Want to tell you that my age is 29, height is 5. 1 and weight is around 50 kg. I am not under weight and not over weight but still I have little fat on my waist. I have little belly fat and having two fat wrinkles on my waist. Also my stomach is not flat, I am having belly fat. I want to remove it totally. I am going to get married after 1 month and want to wear lehenga and sari. For that I have to remove my fat. Kindly suggest how can I remove my belly fat and how can I release my leg pain with continue running.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You appear to be a very determined to be bride. Anyway what I understand from your history is that you have 2 issues: one is leg pain and other is belly fat. Your leg pain started because your leg muscles are not used to this kind of load. Normally you should always start with less and increase it gradually over a period of time with regular muscle strengthening. Also you need to do a pre-exercise regime wherein you stretch your muscles and do a light warm up before actually starting to run. Normally it builds up over a period of time, but process has to be a slow and a steady one. Now since you have pain: please start with ice packs, limb elevation, restriction of your physical activity tom as much as possible, gentle otc anti-inflammatory as per your suitability. Your pain should settle in about 5-7 days time. Your weight in any case is not much, so do not worry about burning those extra calories please. Umaji, Thanks for the query. When one is adequate dosage of levothyroxine, TSH is maintained < 3 mU/L, at that level all the troublesome symptoms of hypothyroidism are in complete check. As you are complaining of some of the symptoms of hypothyroidism, it is better to check the TSH level. If the level is higher than above mentioned, dosage may have to be increased. If you want I can help in achieving better control of the condition, for that you need to come back with following details: weight, height, T4 & TSH levls, then I will be able to guide specifically. I have been suffering from depression and anxiety for last 3/4 years. I am on D-veniz (50 mg twice) and buspin (5 mg twice). I am also taking two tablets of clonotril 0.5 mg for sleeping. I am also taking zubira for hyper triglyceride and pregabalin for spondylosis. I am still depressed with anxiety, muscle pain and sleeping trouble. Dear lybrate user. I can understand. The more you try to get rid of anxiety and depression, the more those troubles you. You have to adjudt with those so as they don't disturb you at all. I mean, let them be there and if you are able to live your life normal, it is a complete cure. CBT can only help you. If there is a cure, past medicines must have cured. Believe me, there is no cure, just adjustment. I can help you achieve it. I suggest online CBT. Take care

Back pain often strikes when something's amiss with the delicate column of bones, muscles, ligaments and joints that holds you up. But many other conditions, like strains, arthritis, hairline spine fractures and even kidney infections, can cause your back to throb.

What you can do

Ice first, heat later. As a pain reliever, ice works great. It temporarily blocks pain signals and helps reduce swelling. Several times a day, lay an ice pack wrapped in a towel on the painful area for up to 20 minutes. Alternatively, you can use a bag of frozen peas or corn. During the first few days of home treatment, apply the ice pack as frequently as necessary. Later you may still want to use ice after exercise or physical activities. After about 48 hours, switch to moist heat to stimulate blood flow and reduce painful spasms. Dip a towel in very warm water, wring it out, then flatten and fold it. Lie on your stomach with pillows under your hips and ankles. Place the towel across the painful area, cover the towel with plastic wrap, then put a heating pad - set on medium - atop the plastic. Leave it on for up to 20 minutes. You can repeat this three or four times a day for several days.
Perfect your posture. Look for the posture that places the least stress on your back. To do it, stand straight with your weight evenly balanced on both feet. Tilt your pelvis forward, then back, exaggerating the movement. Then settle into the position that feels most comfortable. Now 'work your way up' your back, focusing on one area at a time. First concentrate on the area near your waist, then your chest area, and finally your neck and shoulders. Try to feel which position is least stressful and most comfortable. This is the position to maintain when you're standing, walking, and beginning or ending any exercise.
Rise and shine. Each morning before you get out of bed, lie on your back and slowly stretch your arms overhead. Gently pull your knees to your chest, one at a time. To rise, roll to the edge of your bed, turn on your side, put your knees over the edge, and use one arm to push yourself up as you let your feet swing to the floor. Once you're on your feet, put your hands on your buttocks and lean back very slowly to stretch out your spine.
Rub in some relief. Ask a partner to massage the aching area. If you want to use a 'back rub' cream or ointment, go ahead, but use caution, as most topical creams produce skin irritation after a few applications. For a simple back massage aid, stuff several tennis balls into a long sock, tie the end of the sock, and have your partner roll it over your back.

A natural boost

Pick a pepper product. Your drugstore carries liniments that contain capsaicin, the heat-producing substance in hot peppers. Applied to your skin, capsaicin depletes nerve endings of a neurochemical called substance P. Researchers have found that substance P is essential for transmitting pain sensations to the brain, so when there's less substance P in circulation, the pain meter is turned down a bit. Look for a cream or ointment containing 0.075% or 0.025% capsaicin. And be patient: You may have to use it for several weeks to feel the full effect. Stop using it if you begin to feel any skin irritation.
Three or four times a day, take 500 milligrams of bromelain. Derived from pineapples, this enzyme promotes circulation, reduces swelling, and helps your body reabsorb the by-products of inflammation. Look for a strength between 1,200 and 2,400 MCU (milk clotting units) or 720 and 1,440 GDU (gelatin dissolving units). Wait for at least an hour after each meal before taking the bromelain, or it will work mainly in your gut instead of your muscles.

Do hot fomentation keep a pillow under the legs at night &/when resting. Any way take ibuprofen 200mg od & sos x 5days do knee exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. You will need other supportive medicines also. Do ask for detailed treatment plan. Don? t ignore. It could be beginning of a serious problem.

LOW BACK PAIN IN THE ELDERLY

  • It is now known that acute low back pain tends to occur less once we pass the age of 55; so if you are over 55 or thereabouts, you may notice that you experience a more persistent ache in the low back but no longer have the acute and severe episodes that affected you in your more active and vigorous days. Nevertheless, this aching can cause significant problems, especially if you are forced to reduce activity. The human body thrives on activity and decays with prolonged inactivity. It is undesirable for any of us, irrespective of age, to reduce our levels of activity. Only if reduced activity is forced upon us by Significant health-related problems should we exercise less.You may also be told that you have degenerative changes in your back or that you have arthritis and will just have to live with it.While it may be true that your back has worn somewhat with ageing, it is certainly not true that you will just have to live with it.
  • It has been found that many people who have joints in their spines that are worn with age have never had back pain, and we now know that the wearing in itself is not a cause of pain.It is my experience that there are few persons who will not derive some benefit from the postural advice or the exercises, or both.
  • Every older person should carry out the advice regarding the correction of the  sitting, standing an lying postures. Not all of you in the older group will be able to carry out all the exercises as advised, but you should all try. I have found that age is not necessarily a barrier to the successful application of the exercises, and although there are some who may not succeed because of weakness or disability, most will be able to advance at least part way through the recommended programme. My advice to you is to start by perhaps reducing the number of exercises to be performed at each session and to perform fewer sessions during the day. Do not hurry the process, and always rest adequately after completing.

Core Strengthening Exercise- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Hams Stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. repeat 10 times, twice a day. Quadriceps Exercises- Lie straight, make a towel role and put it under the knee, press the knee against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.

Hi = relax and stay positive. = increase fluid intake. = eat seasonal fruits for natural vitamins = do streching of hamstring, quadriceps, and calf muscles. = if you are overweight, reduce weight by cycling or swimming as it gives low impact on joints. = do check your calcium and vit. D = apply moist heat pad to affected joint and muscles. If still you dont get relief consult physiotherapist as there are various causes for leg/ joint pain and some test may be required for further investigation. Best wishes.

Mr. Lybrate-user as per your description you must be having urinary tract infection due to stone. First you have to check by doing ultra-sound of your stomach. If it is positive you have to take medicine. Consult after ultra- sound for proper medication. Start taking cucumber, daily.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes of pain in lower back region. It can be due to faulty posture, related to weak muscles, issues with intervertebral disc, narrowing of spinal canal, any bony instability besides many other, so accurate diagnosis is the key to proper management of the condition. You have mentioned early osteoarthritic changes which is surprising at this stage but I would like to see the investigations which you have done which mentions these changes. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Hot fomentation x twice daily. Neck exercises. Neck stretching. Postural correction. Shoulder shrugs. Core strengthening exercises. Take frequent breaks at work use cervical pillow. Use back support. Self massage the back of neck.
