You should keep your blood sugar controlled by means of dietary restrictions, exercise and medications. You may take vitamin b 12, b6, folic acid, pregabalin and nortriptyline with consultation of your doctor. This may prevent the recurrence but medicines and other things have to be continued. Unfortunately no permanent cure is available.

Dear at this age shaking of hand may be due to anxiety or excessive alcohol use or thyroid disorder. Get tsh, t3, t4 done, stop alcohol if abusing. If you feel yourself worried always, can not relax, have dry mouth, palpitation, excessive sweating, churning sensation in stomach, dizziness etc then you may be suffering from anxiety. Treatment of anxiety with medicines may also cure this shaking of hands.

Hey. Well to say that you are having a migraine headache, then the following things should be present. 1. One sided headache which is very severe lasting for few hours to days. 2. Head ache associated with nausea and vomiting. 3. Sensitivity to loud noise or bright light. 4. Along with this, many people have some triggers for migraine head ache like cheeze, yoghurt, colas, caffeine, stress. Etc. So now, you can very well decide whether you have migraine or not. I hope this helps.) take care.

I'm 50 years old. And i'm having a severe pain in my shoulders. The pain starts from the neck, and comes to the shoulders. Whenever the pain comes, the area gets swelled up. What should I do? Is it related to physiotherapy or should be consulted to neurologist? what should be done to this pain?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You will have to tell me since when have you been experiencing neck pain, the radiation of pain is it to shoulders or goes in your hands also? is there any weakness, numbness, paresthesias etc also associated with it? swelling can be due to multiple reasons, but if your swelling is in both shoulders then it can be related to your physiotherapy, my advice will be to stop your physical therapy till swelling subsides, take anti inflammatory medications initially, do ice packs at home, take adequate calcium and vitamin d as per your requirement and let me know in 2-3 weeks time how you are feeling? do not hesitate to contact me if you need any further assistance.

I am suffering with insomnia since last ten days, actually my job is a night shift job, so can you please suggest me for good sleep in a day time. Thank you sir/Madam.

MD, MBBS, Fellowship (Diplomate American Board)
Neurologist, Bangalore
Hs. I'll try and outline some general advice to help you improve your sleep habits. The concept is called sleep hygiene. Question 1) what is your height and weight? 2) do you snore? has anyone told you that you do? 3) under normal circumstances, do you sleep continuously through the night? or do you wake up once or more? 4) do you have any difficulties breathing? 5) what would you say is the quality of your sleep? do you wake up every morning feeling refreshed? 6) under normal circumstances. What are your sleep timing like? I see that you work at night? when do you try to sleep? how many hours do you get? 7) can you describe your sleep environment? is it dark? is it quiet? what is the temperature in the room? 8) do you smoke? drink alcohol? or drink energy drinks like redbull to stay awake? 9) do you have any medical conditions for which you are undergoing treatment or have undergone treatment in the last 5 years? 10) do you take any medications or supplements (vitamins etc) on a regular basis? I will leave the questioning there for now lets talk about a concept called" sleep hygiene. Your brain needs sleep to repair and strengthen the various neural pathways that get used during the day. Sleep is actually an active process for the brain. Unlike your muscles and your other organs. Your brain can actually be more active at certain times when you are asleep. My reason for telling you this is that the circumstances under which you try to go to bed are very important. Sleep is controlled by something called the'circadian rhythm, you can think out it as the'sleep cycle. That sleep cycle is controlled by various internal and external influences. One of the most important is light! light controls when we go to sleep and when we wake up to a great extent. So. If you try to fall asleep in a room that is very bright! like a room with the curtains open in the afternoon. Then your brain will be receiving the wrong signals and you will not sleep. The concepts and advice given by the us. National sleep foundation are a good set of guidelines you can use to help improve your sleep hygiene.

My wife, 66 years, suffered a intracerebral brain hemorrhage stroke more than a year ago. She is not under any medication for this other than her regular drugs for diabetics and hypertension. She still feels numbness around her right cheeks and continues to feel bitter taste in her saliva. Is this a common phenomena as some doctors say? Is there any medicine that can help to improve without much side effects? (She is too sensitive to most of the common medications.)

Neurologist,
Namaste, It is not clear from what you wrote/ the doctor said that how old is that brain hemorrhage and how severe- but yes, there are people who get strokes because of bleeding in the brain but revived well due to good physical therapy, amongst other reasons.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum vitamin d and serum potassium levels, esr and crp once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.
