Pulmonary Hypertension: How to Treat It?

Pulmonary hypertension mainly occurs due to narrowing of the arteries of the lungs as a result of which the flow of blood is restricted. In this kind of condition, blood fails to carry oxygen to your heart and thus your heart gets adversely affected. The blood pressure of your body will get increased to a great extent if this situation remains untreated, thus leading to bdreadful consequenses, specifically situation like heart failure, serious pulmonary diseases, blood clots in lungs and congenial heart defects, etc.

There are certain forms of pulmonary hypertension, which are serious in nature and worsen with time, so much so that they are even fatal at times. It also includes forms which are non curable in nature, however, in order to improve the quality of life, symptoms can be reduced with proper treatment. The treatment for pulmonary hypertension is often complex and it takes some time to find the most appropriate treatment and requires extensive follow-up care. Your doctor might also need to change your treatment if it's no longer effective. However, when pulmonary hypertension is caused by another condition, your doctor will treat the underlying cause whenever possible.

Common Treatments for pulmonary hypertension:

Medications:

  1. Blood vessel dilators (vasodilators): Vasodilators open narrowed blood vessels. They are one of the most commonly prescribed vasodilators for pulmonary hypertension is epoprostenol (Flolan, Veletri). The drawback to epoprostenol is that the effect lasts only for few minutes. This drug is continuously injected through an intravenous (IV) catheter via a small pump that you wear in a pack on your belt or shoulder.
  2. Ventavis: Another form of the drug, iloprost (Ventavis), can be inhaled six to nine times a day through a nebulizer, a machine that vaporizes your medication. Because it's inhaled, it goes directly to the lungs.
  3. Treprostinil (Tyvaso, Remodulin, Orenitram): It is another form of the drug, which can be given four times a day and can be inhaled or can be taken as an oral medication and can also be administered through injection.
  4. Endothelin receptor antagonists: These medications reverse the effect of endothelin, a substance in the walls of blood vessels that causes them to narrow. These drugs may improve your energy level and symptoms. However, these drugs shouldn't be taken if you're pregnant. Also, these drugs can damage your liver and you may need monthly liver monitoring.
  5. Sildenafil and tadalafil: Sildenafil (Revatio, Viagra) and tadalafil (Cialis, Adcirca) are sometimes used to treat pulmonary hypertension. These drugs work by opening the blood vessels in the lungs to allow blood to flow through more easily.
  6. High-dose calcium channel blockers: These drugs help relax the muscles in the walls of your blood vessels. They include medications, such as amlodipine (Norvasc), diltiazem (Cardizem, Tiazac, others) and nifedipine (Procardia, others). Although calcium channel blockers can be effective, only a small number of people with pulmonary hypertension respond to them.
  7. Soluble guanylate cyclase (SGC) stimulator: Soluble guanylate cyclase (SGC) stimulators (Adempas) interact with nitric oxide and help relax the pulmonary arteries and lower the pressure within the arteries. These medications should not be taken if you're pregnant. They can sometimes cause dizziness or nausea.
  8. Anticoagulants: Your doctor is likely to prescribe the anticoagulant warfarin (Coumadin, Jantoven) to help prevent the formation of blood clots within the small pulmonary arteries. Because anticoagulants prevent normal blood coagulation, they increase your risk of bleeding complications. Take warfarin exactly as prescribed, because warfarin can cause severe side effects if taken incorrectly. If you're taking warfarin, your doctor will ask you to have periodic blood tests to check how well the drug is working. Many other drugs, herbal supplements and foods can interact with warfarin, so be sure your doctor knows all of the medications you're taking.
  9. Digoxin: Digoxin (Lanoxin) can help the heart beat stronger and pump more blood. It can help control the heart rate if you experience arrhythmias.
  10. Diuretics: Commonly known as water pills, these medications help eliminate excess fluid from your body. This reduces the amount of work your heart has to do. They may also be used to limit fluid buildup in your lungs.
  11. Oxygen: Your doctor might suggest that you sometimes breathe pure oxygen, a treatment known as oxygen therapy, to help treat pulmonary hypertension, especially if you live at a high altitude or have sleep apnea. Some people who have pulmonary hypertension eventually require continuous oxygen therapy.

Surgeries:

  1. Atrial Septostomy: If medications don't control your pulmonary hypertension, this open heart surgery might be an option. In an atrial septostomy, a surgeon will create an opening between the upper left and right chambers of your heart (atria) to relieve the pressure on the right side of your heart.
  2. Lung Transplantation: In some cases, a lung or heart lung transplant might be an option, especially for younger people who have idiopathic pulmonary arterial hypertension.

Reducing Complications:

  1. The complications need to be reduced by taking proper health care. Healthy diet needs to be taken regularly along with proper medications. Smoking should be stopped and exercising sessions should be attended daily without any fail
  2. Overweight or obesity needs to be controlled properly for reducing the complications. Specialized caring strategies need to be maintained for avoiding severe kinds of health complications that are quite annoying.

Now let’s look at the downsides of garlic consumption.

A few people may be allergic to garlic. They should never eat garlic raw. And, if they get a skin outbreak, high temperature or headache, they should stop consuming garlic. Eating up to 10 cloves a day is ok, but more than this can have unsavory results like indigestion and garlic-breath. Indians are great garlic users, but we make amistake while cooking with garlic. We cook garlic immediately after crushing or chopping it. But, to maximize the health benefits, you should crush the garlic and use it to cook your curries only after letting it to sit for about 15 minutes. This triggers an enzyme reaction in the crushed garlic cloves that boosts healthy compounds in it. Also consume raw garlic first thing in the morning without chewing to reap its awesome health benefits. If you are scared of garlic breath, you canconsume garlic pills, but the resultant data on these supplements has been mixed. There is no clear indication that the active ingredients in garlic responsible for giving it its health punch, remain active in a pill form too.

Hello, Ipills are high dose progesterone pills and can cause derangement of menses and ovulation if frequently taken and can also cause infertility in such cases.

Hello, Get a serum beta hcg test done to rule out pregnancy anyhow, which is unlikely and if negative then just wait for your menses to start and avoid unprotected sex till menses arrive.

Hello, What is the normal menstrual cycle in your case? And is this happening for the first time? As there are many causes of irregular menses apart from pregnancy like nutritional deficiency, PCOS, stress/ tension, excessive exertion, any other or recent medical condition, side effect of any medication, hormonal imbalance, etc. There is an old saying - Prevention is always better than cure! So, recommend you to follow the same, use any contraceptive method (condom, oral contraceptive pills) till the time you don't want to conceive. Once you have decided to plan for a baby, then you may stop intake of any of these methods. So, if you are married or sexually active, get pregnancy test done or else will havee get ultrasound (pelvis) done. Take homoeopathic medication for your irregular menses – Schwabe's Five Phos. And Alfaalfa Tonic – both thrice daily for 1 month.

My right hand between palm and elbow has started paining since last 4 days. Pain is mainly in the inner muscles/veins. Apart from paining on itself, it pains on pressing with a humble pressure though there's no hurt on it neither a sprain. At nights, pain becomes worse and now it has covered daytime too. It soothes when the middle finger is stretched and hand is given a hot compress. Today the pain seems spreading along the right leg. I have not taken any medication for it but I am taking pills for thyroid and polycystic ovaries. Please suggest.

Pain in the arm can result from a number of factors. Abnormalities or injury of the skin, nerves, bones, joints, blood vessels, and soft tissues of the arm can all result in pain. Arm pain may be the result of referred pain from another part of the body. Please go to a physiotherapist to assess and treat the underlying condition. If pain is caused by an activity, modify or stop the activity. Rest the arm and resume activity gradually. Monitor the response to the activity. Pain experienced in the right arm is usually not serious, but when it is coupled with other symptoms like pain in the leg or tightness of the chest, it may be more serious. Try some home remedies: ice will reduce pain and swelling. Apply ice or cold packs immediately to prevent or minimize swelling. Apply the ice or cold pack for 10 to 20 minutes, 3 or more times a day. Elevate the painful area on pillows while applying ice and anytime you are sitting or lying down. Try to keep the area at or above the level of your heart to help minimize swelling. Apply heat and begin gentle exercise with the aid of moist heat to help restore and maintain flexibility. Some experts recommend alternating between heat and cold treatments. Gently massage or rub the area to relieve pain and encourage blood flow. Do not massage the area if it causes pain. Do not smoke or use other tobacco products. Smoking slows healing because it decreases blood supply and delays tissue repair. Best wishes.

Hello lybrate user- prolonged menses or over flow during menses is known as atyartava disease in ayurveda. It occurs due to improper diet and daily schedule. Try these tips to overcome this diseases- a) avoid salty and spicy foods, avoid fast foods, green leafy vegetable and curd. B) go to bed before 10 pm. C) increase dairy product other than curd in your diet specially cow's ghee. D) use ashokaristha 2 tsp twice daily with water and tab pistil forte 1 tab twice daily.

If your pregnancy is less than 8wk (counting from 1st day of last period), you can undergo medical abortion. Medicines are available but you have to take them under guidance of a gynaecologist. This is effective in 95% cases.

Can Hypertension Affect Pregnancy?

Blood pressure is the thrust exerted by the blood against the artery walls or blood vessels. A certain count of blood pressure is necessary for blood circulation, but anything excessive may prove to be trouble. A reading above the count considered normal; 140/90 (mmHg) may induce symptoms, such as short breath, severe headaches, nosebleeds and anxiety.

Any sort of hypertension during pregnancy can take a toll on the baby.

1. Preeclampsia is a condition wherein, the blood pressure peeks high after 20 weeks of conception accompanied by traces of protein in urine and functional disorders in a few organs.

2. Hypertension might often result in the delivery of an underdeveloped (abnormally small size) baby.

3. High blood pressure might also give rise to a complication wherein, one might have to go for Caesarian section rather than a normal vaginal delivery.

4. The placenta detaches itself from the uterine wall much prior to the delivery.

5. Preeclampsia escalates one’s risks of suffering from heart disorders or other cardiovascular diseases.

6. It restricts the blood flow to the placenta (an organ nourishing the baby), thus cutting off adequate supply of oxygen and nutrients to the fetus.

Signs and symptoms:

1. Excessive protein content in urine

2. Impaired liver functioning

3. Low urine levels

4. Nausea or vomiting

5. Intense pain and tenderness in the upper abdomen

6. Problems in eyesight such as double vision or temporary loss of vision, light sensitivity and blurriness.

7. Abnormal swelling

8. Persistent and a severe headache

Treatment:

Medications do meddle with pregnancy. However, certain medications are considered safe to be used for keeping blood pressure levels under control. These include Angiotensin Converting Enzyme (ACE) inhibitors, Angiotensin Receptor Blockers (ARBs) and Renin Inhibitors. However, do consult a doctor to get your dosage administered accurately. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Try conceiving during fertile days of your cycle i. E. Ovulation days. Get basic tests of you done i. E. Amh tsh fsh prolactin and ultrasound pelvis and husband semen analysis done. Also start doing regular exercise, it will enhance fertility. Take nutritious diet with fruits juices and salad.

Heartbeat The fetal heartbeat is often detectable at a six-week ultrasound. The normal fetal heartbeat at six weeks is about 90 to 110 beats per minute. Detecting a heartbeat at this stage indicates that the pregnancy will probably continue and not end in miscarriage, although this is not absolutely guaranteed. If the sonographer cannot detect a heartbeat, the pregnant woman will generally be advised to come back in another week or two to check again, since sometimes a heartbeat that is undetectable at six weeks may be stronger and more noticeable at seven weeks or more.

Stop taking pressure of pregnancy, you are still very very young, first enjoy marital/sexual life. Yes, but guidance is the key, checkup with a sexologist nearby. In majority of the cases irregular menses are accredited to hormonal imbalance, but in some cases it may be pathological and secondary to some pathology of ovaries mostly. Now if you want to regularized the menses then you must establish permanent balanced first and this can be achieved only with homoeopathy, because in modern medicine we get only a hormonal support with a very temporary relief.
