I am a 42 years lady. Since last 3 days I am having severe back pain. In this even I am having pain while doing something even while sleeping also. What to do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. This is acute low backache and if your pain is only localized to your back with no other symptoms: then initially I will advise you: - rest for 3-5 days - ice packe at home 3-4 times in a day local analgesic spray (dynapar qps ) for local application - anti-inflammatory and muscle relaxant for 5-7 days - to maintain proper posture of your back initially at all times - physical therapy with modalities only, once pain subsides, then gentle back exercises can be initiated. - avoid lifting heavyweight/ acute forward bending in mornings your condition should actually become better if you follow what I have advised in next week to 10 days. In case pain does not improve or increase or there are any other neurological/systemic symptoms which appear, then you may contact me. Do not hesitate to contact me if you need any further assistance.

Avoid Squatting- Avoid sitting Cross legged.Contrast Fomentation (Hot and Cold). Quadriceps strengthening exercises- Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight Leg Raise: Lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana -- Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

My friend was standing near a wall by it's support and suddenly the edge of wall strike his lower spine near the buttocks. He his having pain in the upper buttock region and in sitting and sleeping. How to know if he has developed a photo or he has problem in the bone near the upper buttock? And how to cure it?

Mr. Lybrate-user, the problem could be damge to blood vessels supplying to lower limb. To confirm how serious is the problem it is essential to get a colour doppler test as well as angeiography of lower limb vessels. With that it is possible to asses the extent of blood supply. Plus it is vital to have excellent blood sugar control with fasting near 100 mg and pp 150 to 160 mg and hba1c% <7. You are very young, so get these investigations and consult a vascular surgeon along with your diabetologist.

Since last one year I am suffering from Knee Problem, while check up with Dr. I learnt that Near by knee some part is rubbed. I am taking Tablets, but seems to be only pain keeler. Is there any right way to get clear from it. Whether this controllable Or is that curable too. Even with Exercise or reduce of weight I get some relief. I am wearing knee cap too. - To protect more damage I was advise to wear knee cap. If any other advise pl give me.

Hi, I am suffering Frozen shoulder, cervical spondylosis, thoracic shoulder blade and lower back l5-s1 and sacroiliac problem. Numbness, pinching, radiating pain weakness in neck shoulders hands elbow to fingers. Lower back to hip to toes radiating pain numbness, some times vertigo feeling. Stiff neck. What are the tests to be done and x-ray to be taken. Medicines and treatment to taken. Exercise r physiotherapy to be taken. I eat lots of spicy food, no proper time maintaining. Sometime skip food, eat heavy food at night. No exercise, late night sleeping getting late in the morning. Daily going outside on bike for long distances to n rough roads. Some times pinching pain left r right side abdomen. And left testicles. Standing/sitting some times for long hours, looking up/down continuously for some time due to job requirements. Please guide what to change myself, what tests and treatment to be taken and which doctor to consult orthopedic r neurologist r ayurvedic massage to be taken. In the meantime kindly suggest some medicine and exercise to be taken. Up to now no medicines r treatment taken. When taking rest and having proper food pain and other problems are vanishing. Now only neck and thoracic pain is there due to poor posture sleeping r sitting and due to poor mattress. Some times I forget completely pain as it goes off and comes back all of the sudden.

Hi if your ligament is torn then you will need to get it repaired if it is very severe otherwise strengthening your knee muscle will help and application of ice three times a day will increase the healing of your knee ligaments.
