Hello, good afternoon my name is Dr. Hitu Madan. I am a gynecologist practicing in the old part of Gurgaon. I am attached to many hospitals and corporate hospitals also. Today the topic of discussion is Infertility. The most common problem encountered nowadays. Infertility is basically a failure to conceive within one year of regular unprotected intercourse. However, there are many causes of infertility but the prime factor which I involve are the male factors which encounter for the 40 percent of the causes and the female factor 50 percent and 10 percent may be the both the causes or rest of the 10 percent may be the idiopathic causes.

Infertility is mainly of two types. Primary infertility and secondary infertility.

Primary infertility is the one in which the female has never conceived and the secondary infertility is the one in which the female has conceived once thereafter subsequently she is unable to conceive. There are many factors involved in this. The things which we have to evaluate in this is the male factor and female factor the both. The male factor only the one thing major included is the sperm count and the volume of the seminal fluid. Basically the motility of sperm is seen in this.

The other factors may be the anatomical disturbances and may be the difficulty in sexual intercourse and drug induced fertility may be there, alcohol induced and there may be some of the unknown causes. In female we see for the anatomical causes, the wearing causes and lastly the infection. The basic importance lies in the examination. The female has to be thoroughly examined to see any anatomical abnormality or any other abnormal dysfunction mainly related to the menstrual cycle. In males we have to go for semen analysis, urine routine and blood sugar for the baseline studies. In females we go for the ultrasound, the thyroid function test, prolactin, blood sugar test.

The other causes are to be ruled out as per the examination the problems are found. Regarding the management of infertility well you have to go as per your doctor and the other things which are to be seen are you should maintain and have a healthy lifestyle and control your weight and you should undergo the proper tests as per told by your doctor and most of the cases conceive only with the initial management only and baseline management only. But the cases which are not treated with the baseline things they might have to undergo surgeries and in the last resort they might have to go for IVF also.

I have got my own centre on Old Railway Road called New Madan Medical Centre. Today, the topic of discussion is PCOS that is the polycystic ovarian syndrome.

The rate of the incidence of PCOS is highly increasing because of changing of the lifestyle, weight gain, sedentary dietary habits, smoking, and alcohol. Of course, genetic phenotype also has a factor in this. PCOS is mostly encountered in the young girls and also a major cause of infertility nowadays. The main symptom encountered in this is the irregular menstrual cycle. Basically, what happens in this is that there an imbalance in the female sexual hormones mainly the estrogen and progesterone. They go out of balance and there is more production of the androgen that is one of the types of the male hormones. Then there are problems encountered in this are infertility, changes in appearance of a lady and in extreme cases there may be cardiac symptoms also.

The deranged ratio of the LH and the FSH hormone leads to all these problems. Certain other symptoms are the irregular periods, acne, weight gain, infertility, depression. Females in extreme cases may have diabetes, hypertension and increased cholesterol level. This is not curative but it can be controlled first by weight reduction- decreasing your BMI by less than 25. In cases of androgen excess, you can control it with the help of the hormonal pills. In the ladies who want to have pregnancy, ovulation induction can be done and if other management required per se, can be done. In certain cases, the lady might have to undergo surgical therapy.

Ectopic Pregnancy - How to Treat It?

An ectopic pregnancy takes place in the case when the completely fertilized egg attaches itself to a place outside the inner side of the uterus. Ectopic pregnancy is also called tubal pregnancy because in most cases, it takes place in the Fallopian tube. Usually, the Fallopian tubes are not responsible for holding an embryo, which keeps on growing. Hence, the egg has no scope to develop in the Fallopian tube. This problem must be treated as soon as possible to avoid further complications. Here are several ways of treating ectopic pregnancy.

  1. Expectant management: In case you are not experiencing any symptoms of pregnancy or the pregnancy cannot be found, close observation is required. This is known as expectant management. You should conduct regular blood tests and follow them up in order to detect, whether the hCG level in your blood is ascending or descending. You are likely to have vaginal bleeding. Use pads and towels instead of tampons. Pain in your tummy is also indicated.
  2. Proper medication: In case ectopic pregnancy is detected at an early stage and constant monitoring is not suitable for you, a medicine by the name of methotrexate can be used for treatment. The medicine stops this pregnancy from developing. It is usually injected into your buttocks. Regular blood tests must be carried out to know how the treatment is taking shape on your condition. Trusted contraception measures should be taken for three months following the treatment as methotrexate is very harmful for an infant in case you become pregnant. Keep away from alcohol, as the combination of methotrexate with alcohol is harmful to the liver.
  3. Surgical means: The most common mode of surgery for removing ectopic pregnancy during an early stage is laparoscopy or keyhole surgery. While a laparoscopy is carried out, you will be in a state of anesthesia. A narrow tube or the laparoscope is inserted through surgical cuts in the tummy. The Fallopian tube, which had the ectopic pregnancy is totally removed in case the other Fallopian tube is functioning well. Else the pregnancy has to be somehow removed without cutting off the Fallopian tube. Removal of the Fallopian tube is the best remedy for treating ectopic pregnancy and getting pregnant again is also possible. It takes a normal woman almost six weeks to recover after the surgery. Laparotomy is another kind of surgery carried out to repair a ruptured Fallopian tube.

How to Deal With Pregnancy Bleeding

Bleeding from the vagina during the pregnancy period in women can be quite a traumatic experience. Bleeding during pregnancy occurs due to several reasons. While some of these reasons can be ignored and are considered natural, the others might be of concern. Vaginal bleeding accompanied by spotting is common in women within twelve weeks of pregnancy. The cervix getting infected might also be a cause of the bleeding, which stops on its own after a certain period. However, bleeding from the vaginal area during pregnancy phase may also lead to something as severe as miscarriage or an early born infant. 

Once u get bleeding after getting pregnant consult ur doctor as early as possible, it's the doctor who will guide u whether to wait or some action has to be taken. For proper assessment and control of the bleeding, you should undertake the following actions:

  1. Keep a track on the bleeding: You must have an idea about the amount of blood loss you have had. It will make your doctor's work much easier to understand the intensity and the root of the problem. He will also be able to present you with a control plan. You have to keep a pad in your underwear and let it get soaked. Keep a track on how many pads get used by in a span of 24 hours. Notice the other features of the bleeding as well; whether there is pain or not and the intensity of the bleeding. Keep a constant check on the color and shade of the blood, and mention the presence of blood clots if any to your doctor.
  2. Take sufficient amount of rest: Untroubled, peaceful bed rest for long duration is the primary treatment to prevent pregnancy bleeding. During the early days of bleeding, total bed rest is recommended by all doctors. In case the bleeding cannot be checked regardless of sufficient rest, you should call the doctor.
  3. Abstain from any form of heavy physical work: Any kind of physical labor, which produces strain in the body such as running, jogging, jumping, cycling or going up and down a staircase quite often should be strictly prohibited during pregnancy. Undertaking these activities causes the uterus to jerk, and may lead to the breakage of delicate blood vessels within the placenta. On the off chance that the bleeding is less, activities of this kind should be still strictly avoided. Even after the bleeding stops, keep away from any strain for two more weeks.
  4. Do not indulge in sexual intercourse: Sexual intercourse during pregnancy can enhance the vaginal bleeding to a great extent. For women facing bleeding during this phase, it is advised to keep away from having sex until your doctor advises.

Pregnancy bleeding is a serious issue and it must not be neglected in the slightest of extent.

5 Signs That You Might Be Miscarrying

Miscarriage basically refers to the phenomenon, when a developing embryo or the foetus  in a pregnant woman dies a natural death, before it can manage to survive on its own. The common signs of miscarriage are vaginal bleeding accompanied with pain. It leads to depression, anger and guilt. The reasons for miscarriage are manyincluding use of tobacco, smoking, diabetes, extreme obesity, drug or alcoholic addiction. Here are some important signs that indicate you might be miscarrying:

  1. Bleeding: Recurrence of bleeding, which keeps on starting and stopping indicates the downfall of your hormonal balance. You must get your pregnancy hormone levels checked. Very intense bleeding, which accounts for soaking your pad within an hour is an indication of the beginning of a miscarriage. However, bleeding is a common factor in normal pregnancy as well.
  2. Cramps: Severe cramps in the vaginal region, accompanied by very heavy breathing and suffocation are other signs that you might be headed for a miscarriage. Sometimes, bleeding occurs along with the cramping.
  3. Disappearance of Pregnancy Symptoms: If you are pregnant, but the signs and symptoms of pregnancy start disappearing, a miscarriage may be indicated. However, it might not be the first major signal. For normal pregnant women, the signs of pregnancy do not appear for many days. You may not feel nauseated or your breasts do not get sore. For ten to fourteen weeks, this phenomenon is a normal one. However, in case it continues for a more prolonged duration, it might be an early sign of miscarriage.
  4. Pregnancy test results vary between positive and negative: Results of a pregnancy test done very early may vary between positive and negative. This is completely normal. In case of obtaining the same results when you are already over two weeks pregnant, it may indicate a miscarriage. You might be having an ectopic pregnancy. Spotting is also noticed in the vaginal region. If you had a pregnancy test showing positive results, but the results come negative after re-taking the test a few weeks later, it is high time for you to consult a gynaecologist. You have to deal with ectopic pregnancy before choosing a surgical path. Varying results signify that something is wrong. The urine produced by the body is less concentrated.

5 Things You Need To Know About Endometriosis

Endometriosis is a common gynecological disorder where the tissue lining of the uterus grows outside the uterine cavity. Endometriosis is caused most commonly by menstrual blood flowing back in the Fallopian tubes, instead of leaving the body. This endometrial tissue may then leak into the abdomen through a surgical scar or cut in the uterine lining, leading to endometriosis.

Here are five things you should know about this condition:

It is hard to diagnose
The first roadblock to diagnosing endometriosis is that though it is a very common condition, there is a lack of knowledge about it. The classic symptoms include heavy periods, painful bowel movements and urination, depression, fatigue and pain all over the body. The only way to correctly diagnose endometriosis is through a laparoscopy or keyhole surgery.

Endometriosis is not equal to infertility
One of the myths surrounding endometriosis is that it causes infertility, however, this is not true. When treated properly, endometriosis does not affect your fertility, and it is still safe for you to have children. At the same time,you should also know that being pregnant does not cure the condition.

It can reduce your sex drive
One of the symptoms of endometriosis is chronic pain all over the body. This, coupled with fatigue and depression can reduce your sex drive drastically. Additionally, the act of intercourse itself also becomes painful for women suffering from endometriosis. This pain can be managed with the use of pain relievers, or by experimenting with positions, which is most comfortable for you.

Some women have a higher risk of endometriosis

The exact cause that triggers the endometriosis is still unknown. However, certain factors are said to raise the risk of endometriosis, such as:

1, Genetics: Endometriosis may be passed genetically from generation to generation.

2. Pelvic infections: Scarring of the uterine walls via infections or surgeries can give the menstrual blood that falls back to the Fallopian tubes a way out of the uterine cavity, and hence, lead to endometriosis.

3. Short Menstrual cycles: Women with menstrual cycles that are shorter than 27 days, or those who have periods that last longer than 7 days are more susceptible to endometriosis than others. However, myths that suggest delaying pregnancy as one of the causes of endometriosis are false.

There is no known cure
The treatment for endometriosis addresses its various symptoms as there is no known cure for the disease itself. Even a hysterectomy will not cure this disease unless it includes the removal of your ovaries as well. Over the counter pain medication may help deal with the muscle aches, while hormonal treatments that slow down the production of estrogen can help with the painful menses. Light exercises such as walking and swimming can also help deal with the discomfort of endometriosis.

Medicines r insufficient don't give her citrus things that will increase it give her bland diet small and frequent meals medicines needs to be revised.

If you r breast feeding your baby, suppression of cycles may be there if otherwise you r medically fit but take care you can become pregnant if you don't take precautions.

Check gor upt as such in lactation most of ladies don't have periods but inspite of that you can become pregnant do use contraceptive also.
