All About Gestational Trophoblastic Disease

DGO, MBBS
Gynaecologist
Ask Free Question
Gestational trophoblastic disease is a rare condition. It only happens when trophoblast cells abnormally grow inside the uterus. Also, this only happens after conception. These cells surround the egg which has been fertilized in the uterus. It is also worth note that the trophoblast cells usually connect the fertilized eggs to the uterus' walls and also form a part of the placenta. It is only when there is a tumor that this disease is diagnosed. There are many forms of gestational trophoblastic disease. Here they are:

Types
1. Complete hydatidiform moles
2. Partial hydatidiform moles
3. Invasive moles
4. Choriocarcinomas
5. Placental-site trophoblastic tumors (pstt)
6. Epithelioid trophoblastic tumors (ett)

It is worth note that most of these diseases have different stages. The stage of how far the cancer has developed can be found out using the following techniques.
Testing for cancer
1. Chest x-ray
This is simply an x-ray of the chest.
2. Mri
An mri is an abbreviation of magnetic resonance imaging. It makes use of a form of magnet which transfers radio waves to a computer to find out what is going on within your body.
3. Ct scan
This is similar to an x-ray except that more detailed and larger pictures inside the body can be taken and the scan has a slightly different procedure. There are several ways to treat it depending on how far the cancer has spread into the body. Here they are:

Treatment
1. Surgery
Surgery is usually done while the mole is still non-cancerous. The chances of the mole becoming cancerous are increased by pregnancy. Therefore, it is crucial that you do not become pregnant until the surgery is complete.
2. Chemotherapy
This is a less-ideal option but has to be taken if the mole has become cancerous.

If the two girls are commercial sex workers, then you must be on guard. Unprotected sex with any unknown partner can be a high risk activity and may result in transmission of hiv infection. As per your saying, you had unprotected sex with 2 girls and also had sex with your wife, this act of yours is very irresponsible one. When a person comes in sexual contact with a hiv infected person, the hiv virus can enter the body. Hiv virus is intracellular virus, it needs a cell to multiply body immunity helps in fighting the hiv virus and can keep the virus destroyed, unless the virus has multiplied in vast quantities and has damaged the" protective" white blood cells. This takes time. If the infection is well controlled then the elisa will be negative. That a person is free from hiv, can be said with confidence only after 2 elisa tests 6 months apart, and both being negative. The earliest marker is hiv viral load. This is in the first weeks after infection (usually from 1 to 6 weeks after exposure). A high viral load is related to seroconversion symptoms. The first hiv protein (antigen) that can be measured is p24 (from 2 to 8 weeks after exposure). Viral load and p24 tests are not accurate for diagnosing early hiv if the results are negative. An hiv antibody response can be detected as early as two weeks in a few people and in more than 99.9% of people by 12 weeks. An antibody test at 4 weeks will detect 95% of infections. Antibody testing at 4 weeks can give you a good indication of your hiv status, but you need a test at 12-24 weeks after the exposure to be considered hiv negative. All this will matter if those 2 girls were hiv positive! if they are negative, there will not be any chances of you or your wife getting hiv also, it would be better to abstain from sex with your wife or anybody for the next 6 months or if that is not possible, then a condom with a spermicidal coating is the best option.

Walking is the best exercise. It should be brisk walk, as far as comfortable, not just a stroll. 20-30 mins a day, 3-4 times a week. That apart there are ample breathing, back and sitting exercises during pregnancy. In yoga, don't go for difficult pose which might lead to accidents. Avoid excessive back bending. Whatever you do should be gentle and comfortable without excessive straining or sweating. When in doubt consult your doctor.

Chewing Your Food Properly - Why it is Important?

Ask Free Question

You probably do not think much about chewing while eating your food. Chewing is indeed important for your health. Well, this simple behaviour of chewing your food properly helps in proper digestion, absorption of essential nutrients and keeps several diseases at bay. When you chew your food properly, your body gives out digestive enzymes which break the large particles into smaller ones. Smaller particles are easy for you to digest.

If your food is not properly digested you can suffer from many digestive issues like bloating, stomach pain, indigestion, headache, constipation and also exhaustion. The number of times you are actually supposed to chew your food depends on the type of food you are eating. Softer food items break down easily and are also easy to digest. You should at least chew these food products 5-10 times before swallowing. Dense food takes time to break down and hence is difficult to digest. You should chew them about 30 times before swallowing. You can also take the package for Eating Right.

The benefits of chewing your food properly

  1. Easy digestion and exposure to saliva: Chewing your food for a longer time helps your food to come into direct contact with your saliva. The saliva helps to lubricate your food, as a result the food goes down easily through the oesophagus. Digestive enzymes present in your saliva also make the process of digestion easier.
  2. Chewing is good for your teeth: Chewing can be a good workout for your gums. It keeps your teeth bones stronger. The saliva released clears any food particles stuck inside your mouth, there is less plaque build-up and it also prevents tooth decay.
  3. Chewing helps to keep weight under check: To chew food properly you must eat consciously and when you slow down, you are instantly eating less. This simple process prevents you from overeating and helps you to avoid any excess weight gain.
  4. Chewing helps to prevent any kind of bacterial growth: Your food most of the time contains bacteria and saliva helps in killing some of them. So if you are eating longer and slower, it gets time to kill the bacteria which might affect your digestive process.
  5. Chewing gives you time to taste and enjoy your food: Properly chewing every morsel of your food lets you taste and enjoy the food you are eating which helps you in staying fit and healthy.

If you wish to discuss about any specific problem, you can consult a general physician and ask a free question.

Okra (Ladyfinger) is fast gaining a reputation as a so-called 'superfood' for people with or at risk of Diabetes or Cancer.

These tender pods are VERY LOW IN CALORIES, providing just 30 calories per 100 gms, and contain NO SATURATED FATS OR CHOLESTEROL. They are also rich in nutrients, completely Non-Toxic, and have No Adverse side effects.

HOW CAN IT HELP TREAT DIABETES?

Okra having Anti-Diabetic properties has increased in recent years, with multiple studies confirming Okra as a potent blood glucose-lowering (or Anti-Diabetic) food.

When  dried and ground okra peels and seeds are consumed they reduced blood glucose levels.

Pap Smear: All the Details You Wanted

DGO, MBBS
Gynaecologist
Ask Free Question
The pap smear test or the pap test is a screening test for cervical cancer. This test helps in detecting the possible growth and presence of precancerous as well as cancerous cells within the cervix - the opening of the uterus. This test was named after the doctor who discovered its use when it comes to detecting the signs of the early onset of this kind of cancer. Let us find out more about the pap smear test.

Reasons: the main reason behind getting a pap smear test done on a regular basis is to rule out any risk or sign of cervical cancer. Also, it can help in detecting the 40 sexually transmitted viruses that may be lurking around in your system. It is a fact that genital warts are born from the presence of the human papillomavirus or hpv which can be of 100 different types - out of which 40 lead to stds. These stds can be the start point of cervical cancer.

Hpv and the pap smear test: the pap smear test may not be able to detect the exact kind of virus, but it can easily detect any cellular growth and changes that may be caused due to the virus. This helps in homing on to the risk or the beginning of cancer. In case this is detected, then the treatment can start before the problem becomes too difficult to manage.

Candidates: the perfect candidates for the pap smear test include women who have crossed the age of 21 years or above. Also, women who have a weak immune system due to the presence of the hiv virus also need to get this test done regularly as they are prone to risk of contracting cancer and infections, more than others. Once a woman over 30 years of age has had three normal pap smear readings in a row, she can ask her doctor to schedule her for a pap smear test once in five years.

Preparation: to begin with, you must ensure that you are not menstruating on the day of the test, in which case it will have to be rescheduled. Avoid having sex at least a few nights before the test as well.

Procedure: the procedure for the test is pretty quick, but a little uncomfortable. The doctor will ask you to lie down before putting your legs up in stirrups and then insert a speculum into the vagina so as to extract sample cells for testing. The scraping action may cause mild pain later.

Get in touch with your doctor about getting regular pap smears to rule out the possibility of cervical cancer.
