What Do You Do About a Severe Toothache?

Those who have experienced it would vouch for the fact that toothache is one of the worst pains. There could be times when the attack happens out of nowhere and you are crying for relief.

The tooth has 2 parts - the visible part called the crown and the invisible part called the root which is embedded in the jaw bone and covered by the gums.  

Both the crown and the root have 3 layers from inside out. The crown has enamel, dentin, and pulp. The enamel is the mineralized part of the tooth, dentin has fine sensory dentinal tubules, and the pulp receives nerve and blood supply to the tooth through a small orifice at the end of the tooth called the apex.  On the root surface, instead of enamel, there is a softer substance called cementum.  The dentin and the pulp continue through the tooth, but are thinner in the root portion of the tooth.

The mouth has the largest amount of bacteria in the body. These act on the food deposits on the tooth and produce acid which leads to breakdown of the enamel. The only symptom when enamel breakdown happens is food lodgment, and it continues till treatment ensues. Once the breakdown reaches the dentin, sensitivity sets in, and most people go for treatment then. If not, the next layer is the pulp, when there is severe pain.  This acute pulpitis causes pain in spurts and can be unbearable.

On the root surface, if there is periodontal disease and the gum line goes down, then cementum gets worn off (far more easily than enamel) and decay reaches the dentin and pulp (again faster than in the crown).

Whatever the case, the treatment would be the same:

  • Dental examination, clinical testing, and x-rays would be diagnostic. Tapping the tooth would reproduce the same pain and that is indicative of acute pulpitis
  • Antibiotics and pain killers would be given to control the pain
  • Once the infection subsides, root canal therapy is initiated.  Using the decayed portion to gain access to the root, thin instruments called reamers and files are used to clean out the pulp space completely. They are then shaped to accommodate an inert substance called gutta percha which ensures infection does not seep into the tooth again.
  • With RCT, the tooth is weakened, and therefore a crown needs to be placed. This could either be a ceramic crown or a full metal crown based on economic and esthetic reasons.

The best way to avoid this is regular visits to a dentist so that decay is identified in the early stages and treated with the minimal cost and maximum natural tooth preservation.

C if you have cervical erosions than you might req fillings otherwise after applying toothpaste do not rinse for five min you may cont to use same.

Home remedies and medications won't help much, pain will be reduced but to get rid of pain necessary treatment has to be done, I think caries has progressed deep and you may require Root canal treatment, kindly visit your nearby Endodontist soon.

I am 47 years old male. I have mouth ulcer from today morning. Threes days back one of my teeth is exctracted. Now I am using antibiotics and pain killers as medication. Please suggest the best medicine for mouth ulcer, urgent please.

Consumables which are acidic in nature like soft drinks, cordials and fruit juices should be consumed in a limited quantity. They can soften the tooth material which would further lead to the dissolution of minerals into the tooth enamel, resulting in cavities.

I have habit of chewing tobbaco. So my mouth can not open as usually. And my teeth bleed some time. And gum became red so any tube or oral tube to help.

As per your complaints and symptoms you may have osmf (oral submucous fibrosis). You need thorough examination and some clinical and pathological examination along with screening and biopsy to confirm the diagnosis. Stop taking tobacco for always. Avoid spicy hot and souf food. You may need sum steoid injections and surgery. So kindly consult a maxilllofacial surgeon.

Fluoride for Teeth - 4 Myths and Facts About it

Fluoride is a chemical naturally present in water and in some foods and is essential for healthy bone growth.  A few decades ago, fluoride was discovered to have anti-cariogenic (against tooth decay) properties and slowly gained popularity. While most believe that fluoride can do wonders for your teeth and help you get rid of all problems, it is not true.  There is only so much that fluoride can do and there are also undesirable effects from it.

Read on to know some myths about fluoride versus the actual facts.

Myth: Drinking fluoridated water can lead to fluorosis, which produces brown, pitted teeth, which are weak and aesthetically unappealing.

Fact:  People develop fluorosis only when the water contains excessive amounts of fluorine.  The currently defined permissible level is about 0.7 parts per million. At this level, fluoride provides the maximum protection against caries, at the same time not leading to fluorosis.  In some cases, mild fluorosis can be seen, which only produces white speckles on the tooth and not brown pits.  These teeth have a higher resistance to decay than people drinking non-fluoridated water.

Myth: Fluoride is a forced medication

Fact: Fluoride is a chemical with special benefits to the bones and teeth.  In cases where the water does not contain sufficient quantities of fluoride or where the person is prone for decay, adding fluoride will provide the extra level of protection against caries.

Myth: Adding fluoride to the drinking water leads to various other medical conditions (heart disease, cancer, etc.)

Fact: Various studies have shown that this is not true. In fact, fluoride has been endorsed as safe to use and does not contribute to any diseases like heart disease, cancer, autism, or kidney problems.

Myth: Fluoride is not safe for children.

Fact: Children need a good amount of fluoride in their tooth forming years. This produces teeth that are more resistant to decay and are stronger generally. If the regular supply of water does not contain sufficient fluoride, then the child can be given supplements in the form of fluoridated toothpastes and gel applications at the clinic to provide the anti-cariogenic benefit.

Myth: If there is enough water in the fluoride, then there is no worry of tooth decay.

Fact: Fluoride only makes the teeth resistant to easy decay, it does not protect from decay completely.  Proper oral hygiene habits (brushing, flossing, rinsing, dental visits) are essential to ensure that there is no plaque accumulation and food deposits to prevent caries.  Fluoride is not a replacement for these practices.

Like with anything else, even excessive fluoride is not good for health, therefore the recommended level is 0.7 ppm.  Ensure fluoride supplements are used only after a discussion with your dentist.

The type of treatment you have for toothache /sensitivity/swelling will depend on the cause of the pain, so your dentist will examine your mouth and may carry out an X-ray to try to identify the problem. •If your toothache / sensitivity/swelling is caused by tooth decay, your dentist will remove the decayed area and replace it with a filling. •If your toothache / sensitivity/swelling is caused by a loose or broken filling, the filling will be taken out, any decay will be removed and a new filling put in place. •If the pulp inside your tooth is infected, you may need root canal treatment. This procedure involves removing the infected pulp and then inserting a special type of filling to seal the tooth and prevent re-infection. •If your toothache / sensitivity/swelling can't be treated using these methods, or if your tooth is impacted (wedged between another tooth and your jaw), it may need to be removed.

There are 2 types of braces: metal ceramics (tooth colored) both can be placed in front region or towards the tongue. Most economical is metal braces placed at front. Costliest is ceramic towards tongue. Treatment vary from 25k 10 1.5L.

Advise deep cleaning, polishing, whitening (bleaching) procedures, artificial enamel filling or capping. Advance procedures can be done with laser. Use Snowdent tooth paste for whitening until then. Apply SNOWDENT paste on the teeth. Wait for 10 to 15 min .Then brush your teeth with the same paste. Applying it overnight for better results. Cause of discoloration – Smoking, coffee, tea, drugs, aging, staining food (wine, blueberry, etc) genetics.

This could either be due to sensitivity of tooth due to loss of enamel or due to decay in teeth. Visit your dentist to check the cause of your prob n get treated accordingly. Meanwhile use a desensitizing toothpaste such as senquelf. Rub some of it onto to the tooth surface n wait for 5 mins before rinsing.
