avoid strain full activity like computer work and lifting weight. do hot fermentation and stretching exercise . rest must if it work then OK otherwise physiotherapy treatment for few days

If you have muscle pull then do cold therapy for initial 48 hours twice a day followed by hot fomentation twice a day. Do not lift heavy weights for few days and when your pain relieved do some strengthening exercises of shoulder, biceps, triceps & forearm muscles.

I have back pain from 1 week. Yesterday I went to physiotherapist and took treatment. Kind of massage. She told me to take hot pack in home and to come for 2 sittings. It was fine for 1hour after the treatment but in the night it became more worse than before. I couldn't sleep night because of that. Is that treatment is right? Do I need to change the doctor?

Hi lybrate-user, I would suggest you to see good physiotherapy doctor whoz speciality is mpt orthopedic, let him assess you completely and with due respect to Dr. Prashanth parate I completely disagree with him, like he said first see Ms. Ortho, but y. Do you need to undergo any surgery or what? that's immature talk on his part. So many things include in assessment, subjective, objectives, special test (musculo skeltal), dermatome & myotomes, tone, mmt, rom etc etc, after all this will come to conclusion. Differential diagnosis etc etc many things required to come to final diagnosis. Then need proper treatment protocol for particular days with that you need to follow do's n do not's.

The nerves coming out of vertebral column from in between the inter vertebral spaces are getting pressed at lumbar and sacral level. Your inter vertebral disc may be rubbing with the out coming nerve root. Your weight 81 kg. Is quite high for your height of 169.5 cms. More the weight more the problem. Reduce your weight to bring it in the house of about 65 to 68 kg. Avoid forward bending of your back to avoid pinching of nerve roots in the discs. Do back bending exercises like raising your trunk while sleeping on your back so that a clear space is created between the mattress and your back. Sleep on hard flat bed and avoid coir sponge mattresses. Don't lift heavy weight as it may aggravate your problem in future.

Lybrate-user, fasting blood glucose level of 247 mg/100 ml is too high. That means after food it will go close to 350 to 400 mg/100 ml. This high level of glucose leads to excess of glucose being excreted from the urine, leading to a great degree of weight loss, tiredness, lethargy, no energy to do work and a host of other symptoms. It will also affect her eyes (retina), small blood vessels, nerves, kidney and heart. So rapid blood glucose control is the only option. She has to be put on appropriate medication and blood glucose levels should be brought down to fasting closer to 100 mg, pp 150 to 160 mg & hba1c%<7. Otherwise situation can be dangerous. Please initiate proper treatment, plus diet control and some physical exercise. Thanks.

I feel suicidal for no reason, I often try to slit my wrist. What do I do to make these thoughts go away?
They are not just thoughts alone; there is a lot of unexpressed anger lurking within you that needs to desperately express itself. This anger is turned inwards and the result is this tendency to hurt yourself or terminate yourself. Whenever it seems that there is no specific reason, then it is bound to be something from early childhood that needs to be dealt with. Meet with a counselor and identify the feeling and thoroughly expend it out of your system. Your parents will know a lot of factors that may have contributed to this situation. Life is extremely precious and no one has a right to willfully end it unless there are very justifiable reasons. If you visit a geriatric ward in any hospital you will know the premium that is placed on survival, and how every possible way you can stretch life is attempted on frail and weak bodies that are in the twilight of their lives. You must also show that regard for your life, especially when you are that young and life is just beginning. You are obviously a bankrupt person spiritually: no spiritually sound person will contemplate suicide. Life is a gift of God and to be held in high esteem and preserved at all costs. You are probably depressed and when people are in that state, life looks miserable, hopeless, and futile. Depression can be resolved and if you have not used all the options to resolve depression, you would be inadvertently throwing away your precious life, never to get it back again. This life is an opportunity and since you have probably not utilized it well for various unfounded reasons please look at it with other people for other options and it will seem better. People who contemplate suicide are lonely people: just addressing that problem alone you will find good reasons to live. Failed or the threat of failed relationships are at the bottom of most suicides. A newer theory about suicide, devised by an American psychologist called Thomas Joiner, states that three main factors can cause someone to turn to suicide. They are: •A perception (usually mistaken) that they are alone and no one cares about them. •A feeling (again, usually mistaken) that they are a burden on others and that people would be better off if they were dead. •Fearlessness towards pain and death. Fearlessness poses the biggest risk that someone will commit a successful suicide attempt – they may fear the pain more than the fear of death. Go out there and conquer the world and don't ever let it get the better of you, ever.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. If you have pain alone with no associated swelling, skin redness or warmth, fever etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum vitamin d and serum potassium levels, esr and crp, serum uric acid levels. Get them done empty stomach and send reports across to me. Once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance.

Sleep on a hard bed with soft bedding on it. SPRING BEDS, FOLDING BEDS OR THICK MATRESS ARE HARMFUL Use no pillow under the head. DO HOT FOMANTATION. Paracetamol 250mg OD & SOS x 5days. Caldikind plus 1tab OD x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again.. Do not ignore .It could be beginning of a serious problem.

Hi there are various reasons for joint pain physical examination is needed. You can start physiotherapy treatment to strengthen the muscles around the knee joint. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Hello Lybrate user according to Ayurveda Vata vikriti is the main factor for pain. Keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Take castor oil 15 ml with lukewarm milk at bed time and take ajwaiyan, haldi, dalchini in equal amount. Mix it in luke warm water add 2tablespoon of cowghee and apply this paste on the most tender are of back. Cover it with paper to prevent it from drying and remove it after 20 mins. Take AShwagandha ghan vati 500 mg 2 three times a day. Do it for 15 days and take advise accordingly.
