Ever wondered how your body releases an egg each month or how the uterus wall thins and thickens at different times of the month? It is the job of your body’s chemical messengers – hormones, which manage the various processes involved in preparing your body for fertilization.

Hormones and Fertility
A subtle balance of different hormones, involved in managing the reproductive organs, enables and maintains fertility.   1. FSH or Follicle-stimulating hormone - The pituitary gland in the brain releases the Follicle-stimulating hormone. The major role of this hormone is to allow a woman to have better control over her menstrual cycle and production of an egg in her ovary. The levels of this hormone designate the proper functioning of the ovary as well as the quality of the produced egg. When there is an imbalance in this hormone, the ovary of the woman will not function properly to produce eggs, thus creating infertility in women.
  2. LH or Luteinizing hormone - The pituitary gland releases this hormone. It plays a vital role in stimulating the ovary of a woman to discharge eggs. It instigates the manufacturing of progesterone to prepare the uterus of a woman to accept a fertilized egg. When the level of this hormone is disturbed, it will not stimulate the ovary to release eggs.
  3. Estradiol Hormone - This is a vital form of the oestrogen hormone. When the levels of this hormone become irregular, it will reduce the chances of making a woman conceive.
  4. Thyroid hormones - This hormone assists greatly in making the ovulation as well as the ovary of a woman to function normally by interacting with the progesterone and oestrogen hormones.
  5. Progesterone hormone - This hormone plays an important role in making a fertilized egg to continue as well as to develop in the uterus of a woman. Any imbalance in the level of this hormone will not allow the egg in the uterus to grow.

For the normal functioning of your body, many hormones are essential, and when there is an imbalance, the system goes for a toss. The thyroid gland plays an important role in regulating metabolic processes all through the body. The thyroid disorders tend to affect the structure or function of the thyroid gland.

The pituitary gland is a small, pea-sized attached to the base of the brain. It controls growth, functioning and development of other hormonal glands. The deficit of one or more pituitary hormones is a state of Hypopituitarism. This clinical term is used by endocrinologists to mean that one or more pituitary glands are deficient. In hypopituitarism, the pituitary gland does not produce the normal amounts of hormones.

Causes-

Hypopituitarism has the following causes

  1. Brain tumours

  2. Sarcoidosis (A lung disorder)

  3. Tumours in the pituitary gland

  4. Hemochromatosis (Too much iron in the body)

  5. Tumours in hypothalamus (Controls the production of hormones)

  6. Histiocytosis X (A rare autoimmune disease where the immune cells damage the organs)

  7. Tuberculosis

  8. Stroke

  9. Sheehan syndrome (Excessive blood loss during childbirth)

  10. Lymphocytic hypophysitis (inflammation in the pituitary gland)

  11. Radiation treatments like chemotherapy

Symptoms-

Symptoms depend on which hormones is not being produced.

  • Adrenocorticotropic Hormone (ACH): Lack of ACH affects the adrenal glands (endocrine glands producing hormones) and cortisol (a steroid hormone). Symptoms include

    • Fatigue

    • Low sodium levels

    • Loss of weight

    • Pale skin

  • Thyroid-stimulating Hormone (TSH): Deficient TSH mostly affects the thyroid (produces growth-regulating hormones) gland. The symptoms are

    • Fatigue

    • Weight gain

    • Skin dryness

    • Sensitivity to cold

    • Constipation

  • Luteinizing hormone (LH): Low luteinizing hormone in women affects their ovaries and ovulation. Symptoms include-

  • In men, lack of LH affects their testes and sperm production. Symptoms include
  • Follicle-stimulating hormone (FSH): This hormone affects men and women the same way the luteinizing hormone does. Thus the symptoms are the same.
  • Growth Hormone: In children, the growth hormone affects their bone, fat and muscle. They have the following symptoms

    • Increase in body fat
    • Abnormal peak bone mass (bone tissue present after the skeleton matures)
    • In adults, the whole body is affected. Body fat increases while muscle and bone mass decrease.​
  • Prolactin: Prolactin deficiency only affects women. The prolactin hormone gets the production of milk started after childbirth. Lack of prolactin thus affects breastfeeding.​
  • Antidiuretic Hormone (ADH): The kidneys are affected by lack of ADH. Symptoms include—
  • Oxytocin: Lactation and childbirth are affected by lack of oxytocin.

Adrenal gland is an endocrinal gland located above the kidney which produces hormones that regulate blood sugar, stress and sexual functions. Any problems in the pituitary gland can affect the adrenal gland as the pituitary gland regulates all the endocrine glands.

Here are the different types of adrenal gland disorders

  1. Cushing's disease - This disorder occurs when the adrenal glands produce too much cortisol in the body. Cortisol is a hormone that adrenal glands produce when a body is exposed to stress. The causes of this disease are intake of steroids and tumors present in the pituitary gland. The symptoms are rise in blood pressure, weakness in the muscles and weak bones.

  4. Pheochromocytomas - These are tumors that develop in the adrenal gland which causes excessive production of a hormone called catecholamines. These hormones help in regulating blood pressure. The primary causes of this disorder are genetic factors. Common symptoms of this disorder are headaches, shortness of breath and sweating.
  5. Conn's syndrome - This disorder occurs when the hormone aldosterone is produced in excess by the adrenal glands. The symptoms of this disorder are feeling excessively thirsty, muscle weakness and high blood pressure.

Pregnancy brings with it the need to be extra careful about the overall well-being of the mother-to-be. Extreme caution has to be taken in managing health, as antibiotics and a lot of other treatment modalities are best avoided during pregnancy. With diabetes becoming common in the younger ages and women choosing to delay pregnancy, there are multiple women who are diabetic and choose to get pregnant later in life.

With diabetes being a chronic and a lifestyle condition, it definitely has its effects on pregnancy. However, the key is to manage diabetes cautiously in such a way that its effects on pregnancy, both the mother and the developing baby, are minimal. The increased blood sugar levels from the mother pass on to the baby, so keeping a close watch on the blood sugar levels is very important.

  1. The maximum effect of diabetes on the baby is during the first few weeks of pregnancy. It is when the vital organs (brain, heart, lungs and kidneys) are forming, and high blood sugar levels can be harmful. This is slightly tricky as most women realise they could be pregnant only after about 6 weeks. If pregnancy is planned, it is best advisable to keep a check on sugar levels much ahead of the pregnancy.

  5. Pregnancy can induce the development of diabetic retinopathy, which may prevent vaginal delivery, as the woman may not be able to exert the required pressure.
  6. Abnormal calcium and magnesium levels, which need to be monitored for a while after birth
  7. Most diabetic women reach full term if the sugar levels have been under good control.

Diabetic pregnant women are high at risk. However, with advanced medical care, these risks and complications are managed very well producing healthy babies from normal deliveries with effectively no harm to the mother’s health. Once a diabetic woman tests positive, it is advisable to seek medical help to plan diet/meal plan, physical activities, exercise regimen, lifestyle changes, and medication dose adjustments. The thyroid hormone plays an important role during pregnancy in the development of the baby and also regarding the health of the mother. How pregnancy affects normal thyroid function?
The pregnancy hormones known as human chorionic gonadotropin or hCG and estrogen lead to increased thyroid hormone levels in your blood. The hCG made by the placenta stimulates the thyroid to produce excessive hormones. Increased estrogen leads to higher levels of the thyroxine binding globulin, which transports the thyroid hormone in the blood.

Because of these normal hormonal changes, thyroid function tests are difficult to perceive during pregnancy. The thyroid hormone is very important for the normal development of a baby’s nervous system and brain. During the first trimester of pregnancy, the foetus depends on the supply of thyroid from the mother via the placenta.

In healthy women, the thyroid enlarges during pregnancy. An enlarged thyroid can indicate a thyroid disease, which should be diagnosed immediately. Thyroid problems are difficult to diagnose during pregnancy because of the increased hormone levels, increased thyroid size, fatigue and other factors.

Hyperthyroidism in pregnancy
Hyperthyroidism may occur in pregnancy because of Graves’ disease where enough thyroid hormones are not produced. Graves’ disease is an autoimmune disorder in which the immune system attacks the cells and organs of the body instead of fighting bacteria or viruses. With Graves’ disease, an antibody is released by the immune system called the thyroid stimulating immunoglobulin or TSI. This causes the thyroid gland to produce excess thyroid hormone. In many cases, the antibody is also associated with eye problems like bulging, irritation and puffiness. Graves’ disease commonly appears during pregnancy, but women with this disease from before may actually observe improvements in the symptoms during the second and third pregnancy trimesters. The remission or the disappearance of the symptoms of Graves’ disease may occur due to the general suppression of the immune system, which occurs during early pregnancy. The disease is likely to recur and worsen in a few months after the delivery. It is important for pregnant women with Graves’ disease to be monitored carefully.

Hypothyroidism during pregnancy may lead to a developmental delay in the child. Sometimes, hormone therapy is given to women who are at the borderline stage in thyroid function during pregnancy or just before pregnancy.   1. Genetic or racial: While the European women have a more clean face that is free of hair, the Mediterranean and Indian women are highly likely to have more facial hair growth. There is also a strong family history relation, and it is hereditary.
  2. Androgens (testosterone): This is a male hormone, essential for the development of male sexual characters. Though it is present in very minute amounts in all women, in some, there could be higher than these minimal levels, leading to minor masculine characters, facial hair being one of them.
  3. Hormonal imbalances: There are multiple hormonal issues which can manifest as increased facial hair growth. The two most common ones include polycystic ovarian syndrome ((PCOS)) and adrenal conditions including tumours, Cushing's syndrome, and hyperplasia. In all these cases, there is increased amount of androgens, leading to excessive facial hair.
  4. Medication side effect: Some medications like testosterone, cyclosporine, anabolic steroids, and Minoxidil also lead to increased amounts of facial hair in women. Some other medications used to treat migraines, seizures, schizophrenia, and hypertension can also cause hirsutism. This is indirectly due to increased levels of male hormones in the system, producing these effects.
  5. Birth control pills: These also affect the hormone levels and lead to increased facial hair.
  6. Obesity: Most women with facial hair are also on the heavier side, and most people have a reduction in hair once weight is managed.

Treatment: The underlying cause should be treated for controlling hair growth. If there is an inducing medication, an alternative should be looked at. While hormone levels can be altered, in most cases, not much can be done. These need to be dealt with using one of the following methods - depilatory creams, waxing, plucking, shaving, laser reduction, electrolysis, and weight loss.
