All About Colorectal Surgery

Colorectal surgery is the broad term for surgical procedures performed on the colon, the rectum and the anus. There are various different surgical procedures which fall under colorectal surgery and these are used to treat a vast array of disorders, such as:

  1. Anal cancer
  2. Colorectal cancer
  3. Severe complications related to constipation
  4. Anal injuries
  5. Inflammatory conditions of the gastrointestinal tract
  6. Congenital defects
  • Procedures: The surgeries under colorectal surgery are performed after diagnostic tests such as proctoscopy, sigmoidoscopy and defecating proctography. The most common diagnostic test is colonoscopy. These help to identify the origin and nature of the problem and decide which surgical procedure is to be followed. The procedures under colorectal surgery are as follows:
  • Colectomy: This procedure involves removal of a section of the large intestine. This is known as partial colectomy. In extreme cases, such as advanced cancer or severe gastrointestinal infection, the entire colon is removed and this is called total colectomy. Sometimes, the rectum is also taken out along with the colon and this is called proctocolectomy.
  • Colonic polypectomy: An abnormal growth of tissues in the inner lining of an organ is known as a polyp. Colonic polypectomy is done to eliminate polyps from the colon and rectum before they become malignant. This can be done endoscopically. Surgery is required in case of large polyps.
  • Strictureplasty: Chronic or repeated bowel inflammation causes scar tissue to accumulate in the large intestine. This results in the narrowing of the colon. Stricturoplasty removes the scar tissue so that proper flow of digestive contents is resumed.
  • Colostomy or Ileostomy: A damaged section of the colon is removed and the shortened intestine is then attached to another opening (stoma) in the anterior wall of the abdomen.
  • Hemorrhoidectomy: This surgical process is used for swollen hemorrhoids or blood vessels, which form in the anal canal. Hemorrhoidectomy is extremely effective in removing hemorrhoids, but the surgery also involves a number of complications.
  • Anoplasty: Anoplasty or imperforate anus correction is done to correct birth defects in the rectum and the anus. The structural flaw does not allow the stool to pass properly from the rectum and so it is repaired through surgery.

Mr. Lybrate-user, with fbs 276 mg & pp 426 mg/100 ml, you have very severe uncontrolled diabetes. You need to start immediate treatment with appropriate anti-diabetes drugs. There is no option. Otherwise you are liable to get any one of the severe long term complications of diabetes. It is also necessary for you get your eyes checked to see if there is any affection to your retina. Your bmi is 28 kgs /m2 (normal <23 kgs/m2), showing over weight. Your ideal body weight should be 70 kgs. Hence your diet should be such tahe provides calories based on ideal body weight, daily exercise and blood glucose levels. When diabetes is in check fbs should be nearer 100 mg, pp 150 to 160 mg/100 ml & hba1c%<6.8. Otherwise there can be serious consequences. The first few days after transplant depending on how well your new kidney is working you may need to continue to restrict certain foods for a short time. Your dietitian, or medical team, will let you know if this is necessary. It is important that you try to eat well during this period to help your wound heal. If you are finding it difficult to finish your meals inform the ward staff or the dietitian. What to do when you go home once your kidney is working well you will no longer need to be on a special diet but it is important that you still watch what you eat and have a healthy balanced diet. This is particularly important as your transplant medications can increase your blood pressure, cholesterol and blood sugar levels. These medications can also weaken your bones so you need to have a good intake of calcium and vitamin d. Continue a low salt diet almost all of us eat too much salt. About 80% of the salt we eat comes from processed foods, fast food, and can noteen and restaurant food. About 10- 15% is added at home and only 5% occurs naturally in food. To reduce your salt intake, you need to use less salt in cooking and at the table, eat more fresh foods and less processed and ready-made foods. Avoid using salt substitutes, for example: lo-salt and so low. Limit fat. Eat more fruit & vegtables fruit and vegetables are the perfect snack food. They are high in fibre, vitamins and minerals and low in fat. Aim to eat 4-5 potions of fruit and vegetables each day. Fresh, frozen, dried or tinned all count! include a wide variety of fruit and vegetables. A small glass of fruit juice (unsweetened) can count as 1 portion each day. Choose fruit tinned in natural juice instead of syrup. Dried fruit contains a lot of sugar and should be consumed in small quantities. Avoid adding salt, butter, oils or rich sauces to vegetables. Grapefruit and grapefruit juice is best avoided if you are taking: cyclosporin (neoral/sandimmum), prograf (tacrolimus) or cholesterol lowering drugs.

If renal failure is there, then normal kidney functions have to be carried out by dialysis. Without that there is no way to excrete metabolic waste generated in the body. So dialysis has to be continued regularly as advised.

Hello I have a shameful problem. I have a big painful sometimes itchy lump on the hole of my ass, I don't know what it is or what to do about it. It's just 1 lump on 1 place, not around the whole it look more dark and red. Its thick round and big. What could it be and what can I do about it?

Most lumps at anal opening are due to piles. However other diseases like abscess or even cancer can present like a lump and you should have it examined. Dont feel shame as treatment is more important.

Drink more water eat more fruits, water melons etc keep good hygiene. Avoid pressure on lower abdomen if doing sex. No harm as such just by sex but careful. Nimbu pani every day tulsi patte every day kadhipatta 4-5 leaves everyday.

Hi, Most of the renal stones are silent, they pain when they are moving inside or passing down. Yet times pain depends on the location of the stone inside the kidney. Treatment options - better to go to surgery as stone is already 15 mm
