Hi sir, i am suffering from cervical pain from last 2 years, now there is a lots pain in my whole body, arm, back, leg, please tell me, what should I do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to have a persistent neck pain as a health issue. If you have localized neck pain with no other symptoms like radiating upper limb pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your neck while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like dynamic x rays of your neck region. Also since you have complained of pain at other regions of the body as well, I will advise you to get some blood tests done empty stomach from a good lab and send reports across to me: cbc, esr, crp, serum electrolytes, serum vitamin b12 and d levels and thyroid profile. Dear lybrate user as per your description you are having lumbar spondylitis due to disc bulge or disc protrusion. Surgery is the very last option, you have not mention your correct age, I hope it is not 10 yrs. First of all if it is not very severe start doing bhujang - asana, tad-asana, surya- namaskar, if you cannoy do all the steps of surya namskar, you can skip few, as per your body says. Apply medicated oil on your back and do hot fomentation. Your you are having constipation, take 1 spoon of castor oil with milk at night. Consult for proper medication.

It looks like you are anaemic. If you have back pain after you sit for long hours then it is due to your haemoglobin levels as it is important to check that. Anaemia always leads to the symptoms of being tired and also having back / leg pain though there may not be any pathological reasons for back pain. The lower end of the vertebral column is inflamed and that bone is called coccyx bone and the condition is called Coccydynia. The conservative management would be to keep ice in that inflamed area during one time in a day and also to sit in a hot water bowl where the penetration of the heat would help to reduce the inflammation. And to reduce the weight falling on the inflammed surface we suggest patients to use air cushion pillow (also called as doughnut pillow) where in the buttock's weight will not fall exactly on the painful tail bone and that would automatically reduce the pain.

Vertebroplasty - How It Can Help Manage Pain?

Vertebroplasty means shaping the backbone i.e. vertebra for backbone and plasty is related to shape. This is an outpatient procedure done to treat compression fractures of the spinal cord. The backbone is made up of multiple small bones which are stacked one on top of the other with a filling material in between that is known as vertebrae. The stack of bones are held in place by the soft, elastic vertebral material.

Vertebroplasty & similar procedure called kyphoplasty is a non-surgical procedure where collapsed vertebrae is brought back to shape by injecting medical bone cement under fluroscopy / image intensifier in OT, thereby relieving pressure on the spinal cord or spinal nerves with special canula directed to the core of vertebral body. Indicated in osteoporotic vertebrae, spinal fracture, metastatic tumour or primary tumour of spine. This technique has replaced major surgery done earlier to reshape & fix vertabrae.

Technique: Bone cement, in a semisolid state, is injected into the weakened or fractured bone space. Once this solidifies, it provides additional support to the spine, enabling normal movement. It also helps in reducing the pain significantly and improves the overall mobility.

Procedure:

In what takes about an hour, the following steps are done. Earlier, a few decades ago, this would be an open surgical procedure. However, with advance technology, now it is done using a small incision

  • Local anesthesia is given in the area that needs to be operated
  • A biopsy needle is inserted into the fractured space with the help of an X-ray
  • The acrylic bone cement is injected into the bone space with a needle to fill up the space, once it solidifies, it stabilizes the vertebral space
  • The needle is removed and the incision is covered with a bandage
  • Usually the patient is sent home in an hour’s time unless the overall condition is really bad
  • There could be some soreness at the site where injection is given, but one can notice a significant reduction in pain from the day of the procedure
  • There is also improvement in mobility while doing daily chores
  • The pain relief can last up to 3 years after the procedure
  • Returning back to normal activities takes about 3 to 4 days after the procedure

When to use this procedure?

Vertebroplasty is usually done in case of non-radicular pain that is intractable. In most cases, they do not respond to traditional therapies including physiotherapy, pain medications and even surgery. The pain could be due to:

I am 81 years old. MY CARDIAC evaluation showed all normal. But MY Uric acid is 8.29 mg/dl against normal of 3.5 -7.2. My RBC was 5.65 nor being 4.5 -5.5 and the tron content is 37.6 nor 70 - 80. I have severe joint once in all the 4 joints of my legs and my left toe showed some softness. My back is aching and I am not able to move about freely and walking even standing for a few minutes painful. What should I do?

I suggest you to do hot water fermentations that would help you to get relieved from joint pains. I also suggest you to go for good ayurvedic body massage which would help you to get relieved from joint pain and also swimming activity is advisable. But food precautions, it is better to avoid tamarind in the food instead you can add lime or mango. And also avoid eating fried foods.

Mr. Lybrate-user, it is necessary to do muscle stretching before & after every sport to prevent injuries as well as to avoid delayed onset muscle soreness leading to muscle tightness & pain. If the pain is only from the muscles, then apply moist heat pack for less than five minutes followed by gentle low back & upper back stretches.
