Hi I have de quervian's tenosynovitis on my right hand. It's fine the whole day but it hurts in the night if I don't put on the wrist splint. Is this how it will have to be for the whole life? I have gone to a foctor who gave me some pain killers for a week and suggested complete rest of the hand for a week or so. That is not an option for me. Is there a way out of this other than a surgery?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. De quervain's tenosynovitis is inflammation of sheath of 1st extensor tendons due to which there is pain at the level of styloid process. The treatment depends upon the symptoms you are having, normally ice packs for local application, thumb spica splint for 3-4 weeks not only at night but all throughout, anti-inflammatory medication as per your suitability, local analgesic spray for application but no massage, ultrasonics and laser as physical therapy modalities are useful in many cases. But if it does not get better, then release is better, it is actually a daycare procedure which take about 8-10 minutes to do in ot under local anesthesia, so it is very safe procedure. First of all your back posture may not b correct. If u r working and have a long sitting job, it is possible. secondly, it can b due to some degeneration in your lower back, due to which nerve compression can b there. see some near by physiotherapist asap. Thank you.

Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretching. Do the cat/cow stretch.

Ageing - How You Need To Take Care Of Your Joints?

Skin is one of the most obvious parts of our body, which ages with an increase in the number of candles on our birthday cake. As you age, joints tend to become stiffer and less flexible. The body does not recover from bumps and bruises as easily as it did before, and it becomes difficult to stand for extended time intervals. This is caused mostly by a breakdown of cartilage and inflammation of the synovium.

What is cartilage?
Cartilage is a flexible connective tissue. It connects the ends of two bones in a joint. It is softer than bone tissue but less flexible than muscle tissue. Cartilage is responsible for the smooth movement of joints. 
With age, cartilage begins to degenerate. As the cartilage gets thinner, movement becomes more and more difficult. If left untreated, the cartilage may completely wear off leaving no buffer between the bones. This is one of the leading causes of knee replacement surgery. 

What is synovium?
Synovium is a membrane responsible for keeping the cartilage healthy. It produces a thick fluid that keeps the cartilage and joint lubricated. In cases of osteoarthritis, this membrane can swell up and produce excess fluid. This in turn causes the joint to swell as well. 

Along with age, factors like being overweight, genetics, gender and lifestyle also play a role in the condition of your joints. Being obese point towards a sedentary lifestyle and puts extra pressure on joints, leading to the thinning of cartilage. While men have higher chances of becoming osteoarthritis patients before 45, women have higher chances of developing this disease after 50. A healthy lifestyle is the easiest way to ensure that your joints work well even when you're old and grey. 

Exercise
Movement of the joints keeps the fluid in them moving. It also prevents the cartilage from shrinking and stiffening up. Exercise also makes the bones stronger, and slows the rate of bone tissue deterioration. Stretching exercises are also a good way to maintain the flexibility of joints. 

Healthy diet
Foods that are known as natural antihistamines, antioxidants and foods that fight inflammation promote healthy joints. Some of these are:

  1. Green tea: It prevents the breakdown of collagen and cartilage
  2. Nuts and seeds: These are rich in omega 3 fats that are essential for good bone health
  3. Ginger: It relieves pain and joint swelling
  4. Yogurt: It helps soothe tender joints and fights inflammation
  5. Pomegranate: It protects the cartilage against damage

Hot and cold therapy can also help relieve joint pain and reduce inflammation. As you get older, avoid putting too much pressure on your joints and give them adequate rest. Lastly, avoid smoking, as people who smoke have an increased risk of cartilage loss. If you wish to discuss about any specific problem, you can consult an orthopedist.

Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day.

I have no knowledge of Panchakarma. This is quite a common condition Rule out diabetes & vit. D deficiency. Sleep on a hard bed with a soft bedding on it. Use no pillow under the head. Any way Take CALDIKIND PLUS 1 tab daily for 10days Paracetamol 250mg TDS x 3 days Contact me after that. Make sure you are not allergic to any of the medicines you are going to take. U do full bad rest some days you can use heating paid problem area some exercise also do your problem last 6 years so you can one months also take physiotherapy a physiotherapist give you heating paid+US+IFT+exercise= feel batter that time, pain also remove but self use heating paid & exercise.

Hello , only scooty driving may not be the sole reason for the pain. Try to maintain your postures n positions straight while driving it. Plus if u have a sedentary life style try being a little active. Use stairs instead of lift and walk for small distances. I think this shud help.

Hello Doctor My age is 32. I have minor cervical spondylitis. I am having cervical pain in my left side back neck and close to left side ear which radiates to left upper side shoulder also. I feel dizzy and light headed also My orthopedic Dr. had given me some muscle spasms medicines which did help for some time later he said he will give nerve compression medicine. I feel some nerve near my left ear is causing pain. I feel some nerve is pinched will nerve compression tablets help me to get rid of this pain. I feel better when I take hot water fermentation I am also doing some neck exercises. Its been more than 15 days now still the pain is there. Will nerve compression tablets give me relief Pls reply.

Had you consulted Physiotherapist? As you are already taking treatment with Orthopedician, you need to consult good Physiotherapist for Physiotherapy some tips for neck pain 1.Avoid using computers continuously, take rest for 5 min after working for half an hour 2. In pain free interval do neck exercise (kindly learn exercise by registered Trainer) 3. You can use thin pillow while sleeping so that normal posture of neck can be maintained 4.Maintain proper posture of body while watching TV/ studying.

This back pain is due to no back exercise. Due to this muscle get stiff. When suddenly you get move fast that time this stiff muscle get break and pain start. In ayurved there is mentioned one procedure KATI BASTI .DO THIS FOR FIVE DAYS WITH MEDICATED OIL if you want to do it you should do it under doctor observation. So consult ayurvedic doctor near you. Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take BioD3 Max 1 tab dailyx10 Paracetamol 250mg OD & SOS x5days Do back(spine)/shoulder/knee exercises Make sure you are not allergic to any of the medicines you are going to take Do not ignore it. It may have to be further investigated. It could be beginning of a serious problem. If no relief in 4_5 days,then contact me again. Contact your family doctor or nearest hospital for emergency help.

Hello, You can take Tablet Pantocid 40 Mg before food once a day for 2-3 days.. I am giving you some health tips to avoid acidity 1.Take small frequent meal kindly take 5-6 small meal in whole day 2.Take 6-8 glass of water in a day 4. Moderate physical activity regularly 5.avoid excessive tea, coffee 6. Do Not postpone Breakfast 7. Avoid excess citrous fruit 8. Avoid smoking and Alcohol 9. Regular Physical activities 10. you may have arthritis in knee. You need mri scan of your knee for better diagnosis. In between please do ice application and quadriceps -hamstring strengthening exercise.

Cortisone Shots - Are There Any Risks Associated with them?

Cortisone shots are injections that help relieve pain and inflammation. They are injected directly into the joints such as the ankle, wrist, knee, hip, shoulder, elbow or spine. These injections are a combination of corticosteroids and local anaesthetics. These corticosteroids are similar to the hormones produced by the adrenal gland. Cortisone injections are often used to treat arthritis, carpal tunnel syndrome, trigger finger, shoulder bursitis and plantar fasciitis. Though they do not cure the disease but they provide temporary relief.

These injections may be slightly painful but provide relief in a day or two that lasts for a few weeks or months at a stretch. Hence, cortisone shots provide relief faster and have longer lasting results than other forms of anti-inflammatory medication. Another benefit of cortisone injections as compared to other drugs that need to be taken in through the mouth is that it avoids side effects of anti-inflammatory medication such as an upset stomach etc. Cortisone injections can also be used to treat backaches and pain that radiates from the spine to an arm or leg.

However, cortisone injections are associated with a number of risks. These include:

  1. Infection in the joints
  2. Nerve damage
  3. Thinning of tissue and skin around the injection site
  4. Pain and inflammation in the joint
  5. Weakening or rupturing of tendons
  6. Osteoporosis or thinning of bones
  7. Lightening of skin around the injection site
  8. Osteonecrosis or death of bones in the injected area
  9. Blood sugar spikes – this is seen mostly in diabetic patients
  10. Weakening of the immune system if the person has an underlying infection

In most cases, these side effects are short-lived. Higher dosages and frequent administration can increase the body’s exposure to the corticosteroid. This increases the risk of long-term side effects such as:

  1. Acne
  2. Easy bruising
  3. Weight gain
  4. Cataract formation
  5. Puffiness of the face
  6. High blood pressure

Cortisone may also be held responsible for the deterioration of cartilage inside a joint. For this reason, doctors do not advise cortisone injections to be had more than 3-4 times a year. The minimum interval between two cortisone injections must be 6 weeks. Doctors also typically limit the number of injections given in one joint.
When used judiciously, cortisone injections can greatly improve the quality of a patient’s life. Hence it is very important to get treated only by a qualified doctor and to completely understand the pros and cons of using this form of treatment. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily.
