Is their treatment to remove hepatitis b positive from body.

Welcome to lybrate we are here to help you on this medical health advice platform. Hepatitis b is transmitted by parenteral route. Following are the ways of it's transmission: 1. Exposure to blood and blood products. 2. Sexual contact with an infected person. 3. Use of contaminated needles. 4. From an infected mother to baby. From wherever he got the infection, that is not important now, important is the treatment. He needs to get following investigations: 1. Hepatitis b profile (hbsag, anti hbc antibody, anti- hbe antibody) 2. Hbv dna - quantitative 3. Upper gi endoscopy 4. Liver function test. Fatty liver is also due to hepatitis b. I am having throat pain from last two days. What should I do?

Hello welcome to lybrate. We are here to help you. You seem to be suffering from upper respiratory tract infection (urti). At home, you can do warm saline gargles, steam inhalation. You should drink hot fluids like ginger tea, that will soothe the pain. But in case you have fever then you should see your doctor who will examine you and prescribe an antibiotic like azithromycin or amoxycillin. You can continue cetrizine that you are taking for net 2-3 days. In case of severe pain, you can add a tablet of aspirin in water and do gargles with that. I am diabetic for the past 18+ years. What is the fasting sugar level allowed for me?

Hello. Welcome to lybrate. Ideally your fasting blood sugar should be less then 126 mg/dl. The treatment is based on the blood sugar levels, your age, weight and associated complications of diabetes, if any. There are two ways of diabetes management: 1. Life style changes - includes diabetic diet, daily exercise, cholesterol and weight management. 2. Drug therapy - if blood sugars are so high that they can't be well controlled on dietary measures, the medicines are required. Medicines are again decided based on the blood sugar levels you should not play with these drugs as this can be harmful. Get them prescribed by a doctor only. They would start you with an assumed dosage and will modify your medicines based on your blood sugar values. I would advise you to get following investigations done: 1. Hba1c 2. Fasting lipid profile 3. Serum creatinine 4. Urine for micro albumin. 5. Ecg. Based on the reports you would be started on the treatment. I hope it helps. Stay healthy.

Welcome to lybrate I understand your concern. You are suffering from premature ejaculation. Don't worry it's not so serious problem. It is mostly psychogenic. Almost every man experience it sometime in his sexual life span. I advise following in such problem: 1. To stop thinking about this problem. 2. Increase the duration of foreplay. 3. To divert your mind by doing some difficult mathematical calculations when you start feeling that you are going to ejaculate. 4. There are many causes of headache, and if correctly diagnosed and treated , you may become ?Headache? free.

Common Causes Of Headaches:

Migraine
Tension Type Headache
Idiopathic Stabbing

Exertional Headache

Vascular Disorders
Subarachnoid Haemorrhage
Brain Tumour
The patient who presents with a new, severe headache have causes that may be different from patient with recurrent headaches over many years.

People frequently ask me to get imaging of brain done to rule out the cause of headache. They have special attraction for MRI and CT Scans. But not every headache requires investigations. But there are headaches which require investigations. Let us understand , when a headache requires immediate attention.

Headache Symptoms that Suggest a Serious Underlying Disorder :

Worst headache ever
First severe headache
Headache worsening over days or weeks
Associated with Neurological Deficit
Fever or unexplained systemic signs
Vomiting preceding headache
Pain induced by bending, lifting, cough
Pain that disturbs sleep or presents immediately upon awakening
Known systemic illness
Onset after age 55
Pain associated with local tenderness, e.G, region of temporal artery
If you have any of these symptoms, you need to have a doctor consultation as soon as possible.

Now, let us read about two most common types of headaches, i.E Migraine and Tension type headache that we see in clinical practice.

Migraine :

Migraine, a very common cause of headache, more commonly affects women than men. It is usually an episodic headache which is usually unilateral (on one side of head) and is associated with certain features like:

Nausea (Feeling of vomiting)
Photophobia (Sensitivity to light)
Phonophobia (Sensitivity to sound)
Light-headedness
Scalp tenderness
Vomiting, Nausea (Feeling of vomiting)
Visual disturbances
Paresthesias (tingling sensations)
Vertigo
Alteration of consciousness
Diarrhea
Seizure
Confusion
People having migraine, usually during acute attacks, prefer to be alone and don?t want any sound, light, want to sleep in dark room.

Acute attacks are usually triggered by some stimulants called as ?triggers? which are commonly following :

Bright lights
Sounds
Hunger
Stress
Exertion
Hormonal fluctuations during menses
Lack of or excess sleep
Alcohol, Wine, Cheese or other chemical stimulation
Patients should try to notice their triggers and should do life-style adjustments to avoid them.

How to Deal with Migraines?

Patients having migraine should seek a medical consultation. There are drugs which are given to terminate acute attacks of migraine, as well as to prevent them.
They should have a fix routine of sleeping like from 10 pm to 6 am. They should not sleep for more than 30 min during daytime, as excessive sleeping during day time would cause problem in falling asleep during nighttime, leading to disturbance of sleep cycle and precipitation of migraine.
They should avoid wine, cheese, lemon.
Tension - Type Headache :

It is a chronic head-pain syndrome characterized by tight, band like discomfort. It is bilateral (means on both sides of head). The pain typically builds slowly, fluctuates in severity and may persist more or less continuously for many days. The headache may be episodic or chronic (present >15 days per month). It may be associated with sleep disturbances.

How to Deal with Tension Type Headache?

The pain can generally be managed with simple analgesics but should be taken under medical supervision as they have a long list of adverse effects. There are other drugs too, but can be given only under medical supervision.
Behavioral therapies including relaxation are also effective.
Patients with this type of headaches are advised to throw away their ?Headaches? to prevent the ?Headache? , means they should try to sort out the tension or problem that is causing the headache.

I am 24 years old man. I can't continue sleeping. What was the problem ?

Welcome to lybrate we are here to help you. Following are the most common causes of disturbed sleep: 1. Anxiety 2. Alcohol consumption 3. Smoking 4. Snoring 5. Obesity 6. External noise disturbances. 7. Sleeping for long in day time. You should evaluate your self, if you have any of these causes. I would advise you to get following tests done: 1. Fasting blood sugar 2. Thyroid profile 3. Hemogram 4. Serum calcium and vitamin d levels. I am suffer from headache. Please suggests.

Welcome to ask the doctor. We are here to help you. The headcaches are mostly due to either migraine or tension headache. The migraine headaches are usually unilateral, associated with throbbing, vomiting sensations, photophobia and phonophobia while tension headaches usually present with heaviness in head, band like sensation over the head with sleeping difficulties. Drugs like flunarizine, propanolol, amitryptilline are used in treatment of migraine. Tension headaches are treated with amitryptilline, escitalopram with behavioral modification treatment. If on examination your doctor feels that the headache requires evaluation, he will get you a mri done. For back pain, you should do hot fomentation of the affected area and apply diclofenac ointment locally. I hope it helps. Stay healthy.

My husband is having breathing problem.

Hello welcome to lybrate. There are two possibilities: 1. Bronchial asthma. 2. Cardiac issues. Breathlessness in night which awakens you from sleep is usually a sign of heart failure and is called as paroxysmal nocturnal dyspnea (pnd). This pnd required evaluation. I would advise in such scenario: 1. Hemogram with esr 2. Ecg 3. Chest xray 4. Pulmonary function test 5. Echo if cardiac issues are found you would be treated accordingly, else you will be treated for asthma. Your doctor would evaluate you clinically. If you don't have severe chest congestion, he would treat you with anti-allergic drugs like levocetrizine and montelukast. He may add deriphylline or doxophylline for the chest spasms. If your chest congestion is very severe and there are wheezes and rhonchi's, then inhalers containing bronchi dilators like salbutamol would be required. Hello welcome to lybrate. We are here to help you. You seem to be suffering allergic cough due to smoking, also known as smoker's cough. I would advise in such scenario to get: 1. Hemogram with esr 2. Ecg 3. Chest xray if the chest xray and these blood investigations show no sign of infection then, you would be treated for allergic cough. Initially you would be treated with a combination of levocetrizine and montelukast. But your doctor will examine your chest for any wheezes/spasm. In such scenario, your doctor would prescribe you inhalers containing bronchi dilators like salbutamol. You might also require short dose of steroid inhaler like budenoside. Migraine headaches are usually unilateral, associated with throbbing, vomiting sensations, photophobia and phonophobia. They aggravate on stress. They also aggravate on watching tv, loud noises, sleep deprivation. A clinical examination specially neurological examination, fundus examination are must to rule out any other neurological problem which might also require neuroimaging. I would also advise you to get your eyes tested. You should see your doctor. He will classify the headache and if it is migranous, treatment would be with the drugs like flunarizine, propanolol, amitryptilline. Welcome to lybrate you seem to be suffering from erectile dysfunction. The two most common causes of erectile dysfunction in males are diabetic neuropathy and psycogenic erectile dysfunction. You have not mentioned any history of diabetes but you should get your blood sugar tested. I would also advise you to get color doppler of penile vessels. In such situation I advise following: 1. To take methylcobalamine supplemtation. 2. To use sildenafil as on when required basis that is whenever sex is desired. 3. To cut down alcohol in case you take as this reduces erection when sex is tried after consumption of alcohol. 4. To keep blood sugars well controlled. This is my best opinion for you. Welcome to lybrate. We are here to help you. Your seem to be suffering from acid reflux disease, which causes bloating and gas problems. So, the best way out in such scenario, is to take a combination of patoprazole and domperidone for a few days. This combination will help him with the acid reflux that causes bloating. Also, a good probiotic containing lactobacillus also helps. Curd is a very good natural source of probiotics and helps in settling the stomach issues very well. Papaya cleans the bowels as it acts as a good fiber laxative, so he will have less bloating. Avoid spicy food and stick to soft and bland diet. In case the problem persists you would require to get an ultrasound of abdomen and upper gi endoscopy done. I hope it helps. Stay healthy.

Hello welcome to lybrate you seem to be suffering from acute muscular pain. I would advise you to get a x-ray cervical spine to rule out any disc prolapse. For acute muscular pain I advise following: 1. A combination of pain killer and muscle relaxant containing diclofenac and thiocholchiside. 2. To do hot fomentation of the affected area. 3. To apply diclofenac ointment locally. 4. To get physiotherapy done particularly ultrasonic heat massage. Your pain would start getting better in 5 days. I hope you are not thinking that just by drinking green tea you will loose weight. If yes then trust me you won't ever. Take whichever green tea you want. I am 18 years old I am having stomach ache regularly what should I do?

Hello. Welcome to lybrate we are here to help you our best on this medical platform. I have read your query in detail and I understand your concern. It seems you are suffering from acid peptic disease/ acid reflux disease. This usually presents with pain in upper part of abdomen. But since your pain is mainly on the right side of abdomen, I would like to rule out any evidence of gall bladder stones that can cause the pain. If you have any hunger pains, any fullness of stomach or gaseous distension of abdomen after taking meals, you should go for: 1. Upper gi endoscopy- to rule out peptic ulcers. 2. Ultrasound abdomen- to rule out gall bladder stones. Infact, the pain due to acid reflux aggravates on taking pain killers and this is the reason, you are not getting any relief from pain killers. I would advise you to take a combination of pantoprazole and domperidone and to take anti flatulent and anti acidity syrup like digene/gelusil for symptomatic relief. Don't consume any alcohol, or smoking in case you do. There is an infection called as h. Pylori infection which causes recurrent gastric and duodenal ulcers and such episodes of acidity.
