It seems you are suffering from nerve compression. Whenever a back nerve compresses or impinge it creates pain over its nerve root. After looking to your pain I am recommending you something: Precautions: 1. Avoid forward bending, if required then bend your knees first and then bend forward. 2. Avoid weight lifting. 3. Sit in chairs or car seats with proper back support. 4. Place pillow under your legs while sleeping or can opt sidelying position for sleep. 5. Get enough vit d from sun as vit d keep bones strong including spine and other bones. 6. Drink plenty of water to enhance the intervertebral disks. 7. Avoid prolonged standing. 8. Change your sitting position every 15 minutes so that it does not pain. If possible visit some neuro-physio who can give you several modalities like SWD, IFT, traction etc to cure the pain. Along with it we have a special program designed in case of sciatica that is Butler’s mobilisation and Mckenzie exercises which a physio gives with the proper technique to get cure from disease. I understand your pain so I am suggesting you the following treatment that you can take at home and can get some relief from pain and discomfort. Treatment: 1. Use of Cold packs- Sometimes piriformis muscle gets tight which cause impingement of sciatic nerve leading to tingling and numbness sensation down into the leg. So ice massage can be given over the area of piriformis muscle or the entire back for 15 min. Once done with cold therapy take rest for 2-3 min to relax the muscles. 2. Since the pain is radiating you can do some below mentioned exercises which will give you immediate relief from pain if done correctly. A) Lie on bed on your stomach gently push up with your arms, resting on your forearms. Hold for 5 secs. Now slightly stretch your back muscles and count till 10 and relax. You might feel a slight pain initially but do not worry. Repeat it 15 times. B) Now that pain must have gone. And now you can push up onto your hands fully. Slowly lower down back to floor. Perform this atleast 15 times. Again a slight pain will be there initially. Once you will complete this exercise program your pain will be relieved right away. If pain is severe while doing the exercise then stop doing the exercise and immediately consult some physio to take care further. Other exercises that can be done in sciatica pain are: 1. Straight leg raising 2. Knee to chest. 3. Hamstring stretch. 4. Back extensors stretches and strengthening. 5. Gluteal stretch. All the above can be done for 10-15 times with the hold (stretch) time of 5 -6 secs Make sure to perform the exercises correctly which will give you relief from your pain and strengthen your muscles. Slipped Disc Problem - Why You Get It?

Are you prone to slipped disc problem? + the ideal way to treat it

A slipped disc, also known as a prolapsed disc or herniated disc, is the term given to the phenomenon that occurs when a cartilaginous disc found between the vertebrae in the spine becomes slightly displaced or protrudes, thereby pressing on the nerves in the adjacent areas and causing back pain.
This happens when the soft inner part of the disc, known as nucleus pulposus, bulges out through a weakness in the exterior part of the disc. A prolapsed disc usually occurs in the lower region of the spine and can be of varying sizes. There might even be the development of inflammation around the disc in some cases.

Who is likely to get affected?
Most cases of herniated disc occur in patients between the ages of 30 and 50. The number of men affected is twice as many as the number of women.
Although the root cause of this problem is not yet known, there are a number of factors that may trigger it. These factors include working at a job that requires a lot of lifting, an occupation that involves remaining in a seated position for a long time, weight-bearing sports, obesity, smoking habits and increasing age.

Effective ways of treating the problem
The different methods that are commonly used to treat the problem are:

  • Medications - There are many different kinds of medicines that are effective in providing relief. These include painkillers, muscles relaxants, anti-inflammatory pills, etc.
  • Spinal injections - A number of spinal injections are used to treat slipped discs. The injection is done at the opening where the inflamed nerve roots are found and reduces pain, swelling and irritation in the area.
  • Radiofrequency ablation - This procedure makes use of an electrical current produced by radio waves to reduced pain and sensitivity in the affected area.
  • Ozone discectomyAs opposed to the surgical removal of the prolapsed disc, this new process makes use of an ozone-oxygen mixture that is injected into the spine to treat the problem. Many international studies have proved its superiority to traditional methods and surgeries.

Those affected by this problem are advised to keep exercising and continuing with their everyday activities as best as possible.

Sir. My mother is a rhutd- arthrities patient and taking prednisolone, hcq, leflunomide, shelcal 500 regularly. My question is if she stops taking prednisolene for some time will it affect? another one is, taking regular calcium tablet, can it create stone in the stomach? your advice please.

Prednisolone should be weaned off gradually. Continue with other medicimes as before until all symptoms are controlled. Kindly show me photograph& the x rays of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take biod3 max 1 tab dailyx10 paracetamol 250mg od & sos x5days do back (spine)/shoulder/knee exercises make sure you are not allergic to any of the medicines you are going to take do not ignore it. It could be beginning of a serious problem. Contact me again, if needed.

• Apply Hot Fomentation twice daily • . Avoid bending in front. • Postural Correction- Sit Tall, Walk Tall. • Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times.

Hello doctor, I slapped my brother 2 days back, after continuous complain about ear ringing, we went to doctor today and doc told that it might be due to eardrum rupture but he is not sure. So he prescribed OTOREX, NUROKIND PLUS, CELIN. After giving ear drops, he is complaining that his ringing sound is more. Pls help me in this matter. Thank you in advance.

Stop all the medicines that you are taking. There could have been a traumatic perforation of the ear drum or the nerves in the inner ear might have got temporarily damaged. Get an ent consultation done to rule out if there is any perforation of the ear drum. Do not allow any water to enter the ear. Put cotton inside the ear & keep the ear dry for about 15 days. The ringing sensation should go on its own.

Take rest avoid lifting heavy weights and bending forward take analgeics and muscle relaxant with ppi for 2-3 days if your symptoms does not improve, pls visit an orthopedic surgeon nearby.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. If there is pain in whole upper limb with any other associated symptoms like numbness, paresthesias etc, then there can be a possible impingement of a nerve. You are advised to start with gentle anti inflammatory medications, physical therapy, if radiating pain is severe, then medications for neuropathic pain etc can be started and normally your symptoms should settle in next 5-7 days, if they do not subside, then a detailed neurological examination and appropriate investigations need to be done.
