Poor performance in studies day by day as compare to 2 years before. Roaming with friend late night. After noticing these changes we have doubt that his friend circle is not good and also taking drugs or alcohol also. We also go through his blood test report as under. Please suggest. Erythrocyte Sedimentation Rate (ESR) is 42 mm/Hr. Mm.Serum Uric Acid-6.2 mg/dl. Other things are normal in report. Please suggest as we are really worried.

Psychologist,
Hi, I can help you with the behavioral aspet of your brother. For medicine, I would suggest you to seek help of certified doctor. Chances are that there could be something bothering him, something which is unresolved in his head. Another possibility is peer pressure. Peer pressure is when we get influenced by people of our age, friends, collegues etc. Many a times in order to keep others happy, we end up doing things they expect from us. Most of the times there is disagreement within ourselves that keeps us frustrated, irirtated and angry. Try and know the reason if possible. It's been 2 years and he still hasn't shared it. I would suggest you to seek help of a counsellor. It will be easy for him to open up infront of a nuetral body and a professional will be able to give you better results.

Avoid triggers. If certain foods or odors seem to have triggered your migraines in the past, avoid them. Reduce your caffeine and alcohol intake and avoid tobacco. In general, establish a daily routine with regular sleep patterns and regular meals. In addition, try to control stress. Exercise regularly. Regular aerobic exercise reduces tension and can help prevent migraines. If your doctor agrees, choose any aerobic exercise you enjoy, including walking, swimming and cycling. Warm up slowly, however, because sudden, intense exercise can cause headaches. Obesity is also thought to be a factor in migraine headaches, and regular exercise can help you maintain a healthy weight or lose weight.

My child are addicted to ganja cigarettes,he had stayed at rehabilitation centre for 4 months and Again he is using ganja.what can be done for his addiction?

PDDM, MHA, MBBS

The following behavioral treatments have shown promise: cognitive-behavioral therapy: a form of psychotherapy that teaches people strategies to identify and correct problematic behaviors in order to enhance self-control, stop drug use, and address a range of other problems that often co-occur with them. Contingency management: a therapeutic management approach based on frequent monitoring of the target behavior and the provision (or removal) of tangible, positive rewards when the target behavior occurs (or does not). Motivational enhancement therapy: a systematic form of intervention designed to produce rapid, internally motivated change; the therapy does not attempt to treat the person, but rather mobilize their own internal resources for change and engagement in treatment.

Dear, Anger and aggression are emotions. Anger comes when you become irritated. Irritation happens when you don't like something, or when something is repeated. As anger is an emotion, it should be vent out. You should be able to throw anger out instead of controlling it. But more perfect will be, know the frustrating situations and stop being emotional. If practiced properly, you will not get irritated at those circumstances at all. Those techniques are much easier to understand.

People start smoking in teen age. How could we prevented them as we know it is an adolescence related problem.

MBBS
General Physician, Mumbai
It is true that people get habituated to smoking in teen age or adolescent life due to peer pressure or media temptations. Please remember that smoking is toxic to your body, mind and well-being. The earlier you can stop this habit, the healthier it can be for you. The lower taste and sinful habit of smoking can be stopped by replacing it with the higher taste. Simply read nice books which give you a higher meaning to life than chemicals in smoking. In place of smoking, spend your time by doing yoga and chant the names of god who you have faith in. It has been scientifically proved that by doing yoga and chanting, the habit of smoking can be stopped. By chanting with love, soon you would be swimming in an ocean of pleasure which is million times greater than flickering pleasure of intoxicants like tobacco, nicotine etc in the smoking, and automatically the attraction to the low taste of smoking would stop.

Inability to concentrate and easy irritability can be signs of underlying unresolved stress and anxiety. Increase in smoking and alcohol is also suggestive of inner stress. Try to find time fir relaxation. Do exercises like yoga and meditation for relaxation. In case you still find it difficult do not hesitate to consult a psychiatrist. In case of any queries and assistance with treatment required, you may consult me online.

I am 22 i have back pain and I want to quit smoking so please help thanks in advance.

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. I can advise you on your back pain issues but you will have to motivate yourself to quit smoking or join some de addiction centre for that. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

I am 57 years old, asthmatic, diabetic with psychiatric problems. I am taking ayurvedic treatment for asthma and Theo-asthalin 1-0-1 as a backup. I also use Asthalin inhaler and Budecort inhaler regularly and hence the attacks are under control. For a mild attack I use Albutamol tabs 1-1-1 for three days and I get relief. If there is an acute attack, inhalers do not work. Then I use a nebulizer with Asthalin and Budecort respules and get good relief. If there is an infection, like having fever, then only I rush to a doctor, who will put me on an anti-biotic (azithromycin 500 mg 1-0-1 for 5 days), bronchodilator syrup, a de-constant and paracetamol for fever. At the present moment, I am free from attacks even though the weather in Bangalore is un-predictable. Due to the history of 57 years of asthma, my lungs are not functioning to its full capacity and hence I feel doing breathing exercises a bit tiresome, even though I am on it. During an asthmatic attack, I usually sweat, probably due to the fact that enough oxygen is not supplied to my body. Once, the attacks are controlled, sweating disappears and I will be able to do my day to day duties without any problem. But this malady has certainly made me handicapped and my moments and activities are restricted. Ayurvedic medicines are of great help to me. Coming to my psychiatric problem - I am having Depression, Anxiety and Schizophrenia. For depression cum anxiety I am taking Venlor XR 150 mg 1-0-0, Mirtaz 15 mg 0-0-1 and Clonazepam 0.25 mg 1-0-1. The symptoms are under control. For Schizophrenia I am taking Sizodon plus 0-0-1 and Sizopin 0-0-1. The symptoms are under control. There is a certain relationship between my asthma and my psychiatric illness. I get asthmatic attacks usually when I am disturbed and depressed. This is due to the fact that I lost my mother at an young age and was brought up by my grand father whose death in 1986 meant a trigger for asthma. Once, I took psychiatric treatment in 1987, the attacks became less frequent. Is my asthma psycho-somatic in nature? Please advice. Do the above mentioned anti-psychotic drugs trigger an attack and which of them are habit forming? Please confirm.

Well, thanks for seeking help and I appreciate the way you detailed your issues out. It seems, your asthma is controlled with conventional medicines. Your occasional depression/ anxiety attacks, despite the symptoms being majorly in control, could be triggering your asthma attacks. I do empathize with your irreparable losses. Life does present us with varied challenges which sometimes are unpleasant to deal with. We, however, need to take every challenge that we are faced with, positively and think it as being an opportunity life has bestowed upon us mostly to evolve us and bring the best out of us. You certainly have dealt with this challenge and moved forward in life already. I would hereby advise you to think positive, visualize pleasant stuff, de-stress regularly and stay relaxed. This practice will not only help avert and reduce the incidences of future asthma attacks but also help you be in self control while an attack has precipitated. Also, learning well & practicing yogasanas like'bhujangasana'savasana' and pranayams like'kapalbhati'anulom-viloma'bhramari' etc all on a daily basis are highly beneficial in this condition unless advised otherwise by your consulting doc. Simultaneously, make sure your diet/ nutrition, lifestyle including bowel habits etc all are streamlined. Its advisable to eat in moderation and less than your full capacity than eating to your full. Try avoiding tea, coffee, spices, chillies, pickles, extremely hot & cold/ chilled foods generally. Do consume citrus fruits/ vegetables rich in vitamin c as they help strengthen your immunity. Warm saline gargling, at least once daily, shall help protect you from throat related afflictions that often trigger asthma in certain people. Mostly, fresh air, dry climate, regular exercise go a long way in helping avert precipitation of an attack. Daily consumption of ginger tea helps a lot, unless you are allergic to it. Inhaling the scent of honey during an asthma attack helps breathe easier too. Having said the above-mentioned, you must continue with your allopathic medications as already advised by your doc and attempt the natural remedies as suggested by me above, only as an adjunct or complement to the ongoing conventional treatment. Hope the above-mentioned helps. Do take care and all the very best. Sincerely, Dr. Pritam (nd) bbsr.

Depression is characterised by persistent sad mood and feelings of negativity, helplessness, hopelessness and feeling that life is not worth living. This may lead to having feelings of self harm. This also suggests that your depression is of a severe type. It is advisable that you a consult a psychiatrist with one of your family member for treatment. Depression like any other illness is treatable with medications and counseling. Do not stay alone. Always be with a family member or close friend. Speak out your thoughts and share them whenever you feel like. Do not self medicate. For further help and queries consult me online.

I am a 19 years old girl. I've been going through major mood swings and I just feel empty in my heart sometimes. Even though I'm surrounded by so many people I still feel lonely. I am really close to one of my friends and I have realised that only when he's around in the morning I wake up with this positive energy. But even if everything going right I always feel somethings not right. I have suddenly lost interest in talking to my relatives or family. I am a photography student and I have kind off lost my interest and dedication in it too. I just can't figure out whats wrong. I have no answers to my questions. I feel low most of the time. When someone asks me whats wrong I get irritated even more cz I myself can't answer that question. Sometimes I feel I should consult a psychologist. I feel worthless and directionless. I feel like I won't be able to find work once I pass out of college. There are just too many things going on in my mind and I end up over thinking. I don't want to take any sort of anti depressants or something. I'm just worried about when things would get back to normal. I get lost in my own world and sometimes feel like going on rides or going out alone. No one lets me be. I don't know what to do.

Dear lybrate-user, you are having depression now. Normally depression increase to a particular severity level and then improve on its own in around 9 to 12 months. So if you do not take treatment it will take 9 month for improvement. With treatment you will find improvement in your symptom in 3 weeks. I would seriously recommend you to consult psychiatrist and start treatment. You can start brisk walking or exercise daily, drink approx 4L of water daily. Maintain your sleep and wakeup time. You will get some improvement with these measures. All d best.

I went in for love marriage against my family and now things are not working fine. What should I do?

Reparenting Technique, BA, BEd
Psychologist, Bangalore
Love marriage, in addition to going against your family, have been bad portends for marriage success. I can say that since you have made your own bed, sleep in it. However, I think we need to be reasonable: if it appears that you have tried all means to resolve the differences and it has not worked, then discuss the best way to end this union amicably. But you both must meet with a marriage counselor and talk about the issues. If you are a boy, it may be easy for you but you need to think of your wife and whether this is fair to her. And if there are kids, you have a lot of thinking and responsible decisions to make. Whatever, it must be made with all of them in perspective. Do not do what is convenient for you alone that would be very selfish. If you are doing this because of family pressure or any other unacceptable reasons, your conscience will not leave you in peace – and you cannot run away from your conscience all of your life! My advice: be responsible, be respectful and be functional in whatever you do.

These problems are either they are too busy or due to anxiety and stress. Busy people use organizers or employ a personal assistant because they can't remember every task.
