 When a man becomes sexually excited (aroused), his brain sends signals to the nerves in his penis. The nerves increase the blood flow to the penis, causing the tissue to expand and harden.

Anything that interferes with the nervous system or the blood circulation could lead to erectile dysfunction.

Anything that affects the level of sexual desire (libido) can also cause erectile dysfunction because a reduced libido makes it more difficult for the brain to trigger an erection. Psychological conditions, such as depression, can reduce libido, as can changes in hormone levels (chemicals produced by the body).

Physical causes

There are four main types of health conditions that can cause physical problems resulting in erectile dysfunction. These are:

  • Conditions affecting the flow of blood to your penis – vasculogenic
  • Conditions affecting your nervous system, which is made up of your brain, nerves and spinal cord – neurogenic
  • Conditions affecting your hormone levels – hormonal
  • Conditions affecting the physical structure of your penis – anatomical

  • You must produce healthy sperm. Initially, this involves the growth and formation of the male reproductive organs during puberty. At least one of your testicles must be functioning correctly, and your body must produce testosterone and other hormones to trigger and maintain sperm production.
  • Sperm have to be carried into the semen. Once sperm are produced in the testicles, delicate tubes transport them until they mix with semen and are ejaculated out of the penis.
  • There needs to be enough sperm in the semen. If the number of sperm in your semen (sperm count) is low, it decreases the odds that one of your sperm will fertilize your partner's egg. A low sperm count is fewer than 15 million sperm per milliliter of semen or fewer than 39 million per ejaculate.
  • Sperm must be functional and able to move. If the movement (motility) or function of your sperm is abnormal, the sperm may not be able to reach or penetrate your partner's egg.

Medical causes

Problems with male fertility can be caused by a number of health issues and medical treatments. Some of these include:

  • Varicocele. A varicocele is a swelling of the veins that drain the testicle. It's the most common reversible cause of male infertility. Although the exact reason that varicoceles cause infertility is unknown, it may be related to abnormal testicular temperature regulation. Varicoceles result in reduced quality of the sperm.

Treating the varicocele can improve sperm numbers and function, and may potentially improve outcomes when using assisted reproductive techniques such as in vitro fertilization.

  • Infection. Some infections can interfere with sperm production or sperm health or can cause scarring that blocks the passage of sperm. These include inflammation of the epididymis (epididymitis) or testicles (orchitis) and some sexually transmitted infections, including gonorrhea or HIV. Although some infections can result in permanent testicular damage, most often sperm can still be retrieved.
  • Ejaculation issues. Retrograde ejaculation occurs when semen enters the bladder during orgasm instead of emerging out the tip of the penis. Various health conditions can cause retrograde ejaculation, including diabetes, spinal injuries, medications, and surgery of the bladder, prostate or urethra.

Some men with spinal cord injuries or certain diseases can't ejaculate semen, even though they still produce sperm. Often in these cases sperm can still be retrieved for use in assisted reproductive techniques.

The average penis size:
Studies done around the world, irrespective of race has shown that the average size of the human penis is about 3 to 5 inches flaccid and 5 to 7 inches erect. There are deviations to this as well, but this is the norm in over 90 percent of the cases. Medically a penis is only considered small if it is less than three inches in size when erect. This is a condition known as a micropenis. Penises tend to fall into two categories as far as erect sizes:

I. Growers: The ones who have a flaccid smaller length but tend to grow much more proportionately and thus have a much larger size after erection.
II. Showers: These are men who have larger flaccid sizes but don't grow proportionately as much after achieving an erection.

Normal size:
If you have an erect penis which is 5 to 7 inches long, then you have a normal penis as do most of other men. Most women have stated that they were happy with the partner's size, but what mattered to them more was how it was used. Also some women reported that the girth of the penis was more important as friction on the side walls of the vagina made them feel more pleasure.

The mechanics of sex:
Although there are some women who may like the visual appeal of a bigger penis, most women can't really tell the difference between a 5 inch and a 7 inch penis when it is inside the vagina. The fascination with a bigger size is mostly cultural, social or hyped up by the media. Most women want a variety of sex acts to help them reach orgasm and very rarely reach orgasm only through penetrative sex. Sex is an integral part of our daily habit (Dinacharya). The importance of sex in our lives has remained unchanged. The crucial points to be kept in mind in relation to this are:

1. Timing is key

According to Ayurveda, the best time to have sex is during the full moon, after 10 p.m. and at least 2 hours after a meal. The ideal timeframe for sex is between 10 p.m. and 11 p.m. This is because this particular time is when our stamina and passion are at their peak.

2. Digestion for distinction

As mentioned before, you need to wait at least 2 hours if you have a full stomach before your body is ready to roll. Having sex before that big meal is digested will create conflict in your mind and body. It's best to direct energy and blood towards digestion, as both of these are required for optimum performance.

3. Tick all the right boxes

Are you sick, tired, angry, stressed, thirsty or hungry? If you've said "yes" to any of these conditions then it's best to wait for the right time to be in the mood for sex. If you have sex in these conditions you can suffer from issues like lightheadedness, headache, bloating, gastritis, etc. Ayurveda says that we must abstain from sex if we are not fully present in the moment as we will not be able to seek pleasure from the act. Having sex when you are physically fatigued or mentally elsewhere can only make matters worse!

4. Seasonal sessions

The body is at its prime during winters so you can indulge in this pleasurable act every day during winter. 
In spring (vasanta ritu) and autumn (sharad ritu) our stamina is moderate so sexual frequency should also take the middle road - once every three days.

Symptoms

Many people don't notice symptoms when they have an STI, including most women with chlamydia. If it's left untreated, chlamydia can affect your ability to get pregnant.

Gonorrhoea can also affect fertility. Around 50% of women and 10% of men with gonorrhoea don't have symptoms.

Left untreated, STIs can affect your health. If you have any of the symptoms listed below, get tested.

In women and men: 

  • pain when you pass urine (pee) 
  • itching, burning or tingling around the genitals 
  • blisters, sores, spots or lumps around the genitals or anus
  • black powder or tiny white dots in your underwear – this could be droppings or eggs from pubic lice

In women: 

In men: 

  • discharge from the penis
  • irritation of the urethra (the tube urine comes out of)

These symptoms don't necessarily mean you have an STI, but it's worth seeing a doctor so you can find out what's causing the symptoms and get treatment.
