Vaginal Vault Prolapse - Know Forms Of Tackling It!

Vaginal Vault Prolapse is the condition where the upper portion of the vagina may lose its normal shape and drop or sag down into the vaginal canal or maybe even outside the vagina. This may occur after a hysterectomy or even simultaneously with uterine prolapse. 

The treatment of vaginal vault prolapse varies with the severity and extremity of your symptoms. Your health care provider may suggest medications and physical therapy to treat your condition if you feel that the prolapse is not bothersome. However, you might have to undergo surgery later if your symptoms worsen and the prolapse affects your quality of life and hinders functional ability.  You must keep in mind that usually more than one area is affected by weakness in your pelvic floor. 

The symptoms of Vaginal Vault Prolapse include- backache, pelvic heaviness, bulging mass into or sometimes outside your vagina that hinders normal activities such as walking and sitting, vaginal bleeding and releasing urine involuntarily. 

 Treatments:

1.  Medication
Most of the women with this condition usually go through menopause around the same time. Since the estrogen levels are severely lowered during menopause, it causes vaginal dryness. Discussing estrogen therapy with your health care provider is a must if you wish to remedy your vaginal dryness. Not every woman should take estrogen so it is imperative to talk to a medical expert first. Usually, women are treated with estrogens before they go into surgery.

2. Physical Therapy
Most health care experts recommend pelvic floor exercises as physical therapy using biofeedback so as to strengthen the specific muscles of your pelvic floor. Biofeedback is the use of monitoring devices that contain sensors and are placed either in your rectum, vagina or on your skin. Biofeedback is very important because it tells you the strength of each muscle you are squeezing i.e. contractions, and if you are squeezing the right muscles in order to perform the exercise correctly. Long term usage of biofeedback in physical therapy leads to the strengthening of your muscles and a reduction in your symptoms. 

3. Surgery
Corrective surgery may be performed on you by the surgeon either through the abdomen or vagina. If the surgeon is performing the corrective surgery through the vagina, he will use the ligaments which support the uterus to solve the problem.

Infertility is the incapability of a couple to conceive after indulging in unprotected sex multiple times over a long period. It can also be referred to as the biological inability of a man to cause conception or a woman to conceive as well as being unable to carry the pregnancy for the whole duration. Research has shown that female problems contribute to over half of all the infertility cases.

Causes-

Causes of infertility in include:

  1. Ovulation Disorders – This is regarded as the most common cause of infertility in women. The disorders can be caused due to conditions like PCOS(polycystic ovary syndrome), Premature ovarian failure, poor quality of eggs, overactive or underactive thyroid gland and chronic conditions like cancer or AIDS.

  2. Problems in fallopian tubes or uterus – Abnormalities in the uterus or fallopian tubes render the woman incapable of conceiving naturally. This might be due to conditions like Endometriosis, previous sterilization treatments or surgeries to correct past problems.

  3. Medications or Treatments – There is a possibility of some treatments affecting infertility. Examples include NSAIDs (non-steroidal anti-inflammatory drugs), Chemotherapy and Radiotherapy.

Diagnosis-

Usually, the initial steps for diagnosing infertility involve a review of the complete medical history of the patients as well as a physical exam. Post this check-up, some diagnostic tests are conducted for infertility. This might include-

  1. Blood and Urine tests: For checking hormone levels.

  2. Pap smear:For checking the health of your cervix.

  3. X-ray: For outlining the internal shape of the uterus so that blockages in fallopian tubes can be identified.

Risk Factors:

  1. Age – Increasing age tends to lower the quality as well as the quantity of a woman's eggs

  2. Smoking – Besides damaging your cervix and fallopian tubes, smoking increases your risk of miscarriage. It is also believed to deplete your eggs at a premature stage, thereby reducing your chances of pregnancy.

  3. Weight – Normal ovulation is hindered by being overweight or even significantly underweight. This is because lower levels of BMI (body mass index) reduces the frequency of ovulation, reducing the chances of pregnancy.

  4. Sexual history – Sexually transmitted diseases like gonorrhea and Chlamydiacan cause damage to the fallopian tubes, resulting in infertility.

Treatments:

Even though it is possible to restore fertility in women using only one or two therapies, a number of treatments might be required before conception is possible. Some of these treatments include:

  1. Intrauterine insemination (IUI): deliberately introducing sperm into the uterus of a woman for achieving pregnancy.

  2. Stimulating ovulation with fertility drugs.

  3. Surgery to restore fertility.

In situations where pregnancy does not happen spontaneously, Assisted Reproductive Technology (ART) can be used by couples to achieve a pregnancy. It is any form of fertility treatment which involves the handling of sperm and egg. The entire ART team consists of psychologists, physicians, embryologists, nurses and lab technicians.

One common ART technique is In vitro fertilization (IVF). It is a process where an egg and sperm are manually combined in a laboratory dish, followed by transfer of embryo to the uterus.

Male infertility basically refers to the inability of a man to result in pregnancy in female, who is fertile. In human beings, this problem accounts for around 50% of infertility. Almost 7% of all men get affected by infertility. Change in status and current stress, which is on the rise, leads to making a man infertile. When a man has the problem of obesity, this, in turn, tends to affect his fertility as well, with the sperm quality going down along with the sperm count.

Causes of Male Infertility:

There are quite many reasons due to which a male can become infertile. They include:

  • Varicocele: This is a situation where an enlargement of veins occurs in the testicles. It results in blood to pool, thus creating an increased temperature. This rise in temperature in the scrotum leads to the reduction of sperm count, hence affecting fertility.
  • Undescended testicles: males with an undescended testicle remains at the increased risk of an inguinal hernia.Infection: An infection occurring in the testicle or prostate can make a male infertile.
  • Chemotherapy for cancer: Chemotherapy for cancers or any kind of radiation can have an adverse impact on a male's ability to produce quality sperm and also affecting the sperm count.

Normal Range of Sperm Count:

  • The normal volume varies from 1.5 to 5 ml per ejaculation.
  • The sperm count tends to vary from 20 to 150 million sperm per milliliter.

Different Issues Related to Male Infertility:

There are certain issues relating to male infertility. They include:

  • Oligospermia: This is an issue mainly characterized by low sperm count.
  • Asthenozoospermia: This is the medical term used for reduction in sperm motility.
  • Azoospermia: Azospermia is a medical condition wherein a man.s semen has no sperm.
  • Teratospermia: Teratospermia is condition mainly characterized by the presence of sperm having different shapes (Morphological defect)

Risk Factors for Male Infertility:

  1. Consumption of Alcohol
  2. Smoking
  3. Stress
  4. Tight clothing (Jockeys) affecting the circulation of the blood in the groin area
  5. Hormonal imbalance
  6. Indulgence in too much exercise
  7. High temperatures- hot saunas
  8. Caffeine
  9. Toxin and pollutants

How to Prevent Male Infertility?

Males can prevent themselves from becoming infertile by following a healthy lifestyle:

  1. They must not get addicted to things like smoking, drugs or in consuming alcohol.
  2. Exposure to radiation and toxic substances must be avoided.
  3. Avoidance of long hot bath or saunas in a must.
  4. Cold water seat bath is very useful to increase sperm count and motility.

Various Kinds of Investigations Include:

  1. Ultrasound of the scrotal
  2. Testing of hormones
  3. Analysis of the urine after ejaculation
  4. Some genetic tests
  5. Biopsy of the testicles
  6. Semen analysis

Ascoril d and augmentin are quite effective for dry cough, an anti allergic may be added as dry cough are often an allergic manifestations. Also try betadine gargle with warm water 3 to 4 times daily. There is severe vit d deficiency and vit d at a dose of 60,000 units per week for 8 weeks and than monthly need to be supplemented.
