I am a 40 years old male having Hypertension for the past 5 years I was taking Telma 40 morning and amlong 5 mg evening for the last three years a week ago my BP suddenly shot up to 190/100 I was feeling dizzy and my heart was pounding very fast this lasted for half and hour. The next day I got and ECG and 2D Echo done which was normal but the TMT test showed positive for induced ischemia. On the Doctor's advice I got a coronary angiogram done which showed mild coronary artery disease but no blocks there was slight plaque build up in the arteries the doctor changed my medicine to cilamet xl 10/50 to be taken morning and ecosprin av 20/75 to be taken in afternoon. It has been 3 days and I still get sudden panic attacks where I feel chill feeling on my hands and feeling of irregular heart beat this last for about 15 minutes and subsides. The doctor has now added ZAPIZ 0.5 to be taken in evening. With this I am not feeling the panic/anxiety symptoms. What is wrong with me should I see a Neurologist or Psychiatrist? Is this sudden feeling of panic/anxiety attack due to cilamet xl?

Dear Friend Not every person with abnormal TMT test required Coronary angiography. You could do stress echo or stress thallium to confirm coronary artery disease (CAD). It again indicate that in future you r not right candidate for TMT as it was false positive & will remain false positive foreever. You said that you had bp problem since 35. It should be evaluated. High bp in young may be bcz of thyroid problem or some kidney problem or kidney tumour which are curable type of hypertension you may have anxiety panick attack thatswhy you have some relief after giving antianxiety medication. To be summerize YOU NEED TO EVALUATE CAUSE OF HIGH BLOOD PRESSURE AT THIS YOUNG AGE.

It appears you are suffering from high blood sugar, (diabetes), high bp and dyslipidemia. You seem to also have fatty liver. These conditions must be properly taken care of. You also are obese (bmi 31.227). Please let me know are on any medication now if so what are taking? further how much exercise do you do every day? please furnish this information to help you properly. As regards the food you have to restrict deep fried foods, sweets and even red meat. Total caloric intake also has to be curtailed to reduce excess weight. Avoid tobacco in any form and alcohol completely. I await your information.

Heart attack can present in variety of ways ranging from chest pain ,neck pain,jaw pain ,to abdominal pain or palpitations with profuse sweating and feeling of impending doom.... At time of suspected attack ask the pt to cough voluntarily so that blood supply to brain is maintained even by failing heart...give him sorbitrate and ecosprin to be kept beneath tongue ..then take him to hospital immediately

What is the maximum systolic pressure a hypertensive patient could take without it getting fatal? Age is 50 years.

That varies from person to person. Some people can have SBP beyond 200 and remain asymptotic but some may become symptomatic with 180. But that's not the point. The point is you should maintain your BP as close to normal as possible.

I am unable to eat little spicy food and some reduction in mouth I have habits of eating tobbaco n betl nuts. Now what can I do. And from last 2 month I am having pain in my chest. Which type of symptoms is this or what treatment I should take.

If you have reduction in opening the mouth, it could be due to sub mucus fibrosis because of your tobacco consumption. Get your mouth examined by surgeon and follow the advice and treatment accordingly.

Hello Lybrate user according to ayurveda, Hypertension is referred to as Rakta Capa Vriddhi and it sees the vitiation of vata and pitta doshas as the main cause. Home Remedy- Mix 1 teaspoon of onion juice with an equal amount of honey. Take once a day for 1 week. Upon noticing improvement, continue for several more days. Treatment- 1-take sarpgandha vati twice a day. 2-yogendra ras. Akik bhasm works very good in case of hypertension. 3-Take pranacharya no tens capsule twice a day Diet-avoid salty diet. Oily food. Hello, CHEST PAIN in your case may be due to 1.acidity. 2. severe anxiety 3. muscular problem A. You can take Tablet Pantocid 40 mg (if no drug allergy) one tablet before food for 3 days B. high cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Avoid fast foods,oily foods,junk foods,alcohol,smoking,fatty foods, non-veg.,avoid stress, take green veg., whole grain. Take anti cholesterol medicines .

Signs and symptoms include ; dizziness or lightheadedness. Fainting (syncope) lack of concentration. Blurred vision. Nausea. Cold, clammy, pale skin. Rapid, shallow breathing. Fatigue. When dealing with low blood pressure, it is usually suggested to increase your salt and fluid intake. Before increasing yoursalt consumption, do check with your doctor. Also, cut down on alcoholic beverages and high-carbohydrate foods such asbread, rice, pasta, and potatoes.
