if your pp sugar level is 300 mg/100 ml, then diabetes is not in control. You need to add some more drugs with the advise from your doctor. What is the hba1c%, is it below 7%? also how active you are, because exercise ad weight reduction helps in better control of blood sugar.

Will a person with Type 2 diabetes under control end up with the need for insulin?
Hello, When T2 diabetes goes on for very long time say 25 to 30 years or longer, then because of persistent hyperglycemia and lipotoxicity beta cells get affected and. Over the years cell mass is lost and most of the T2 patients end up on insulin in later years of their life. Thanks.

Dear lybrate-user, you should start her diabetic and hypertensive medicines in consultation with physician. The long term consequence of uncontrolled diabetes and hypertension is too costly to take a risk of managing them with diet alone. Lifestyle measures such as diet, exercise and weight reduction are no doubt benifitial but they are not that effective compred to medicine. At best they are additive to medicines. Thank you.

Dear friend, many of newly detected diabetics have sugars in range of 500-600 mg%. So you're not the odd man out although it's not good to see such high sugar readings. Please understand. That when sugar touches pancreas, insulin is released which takes sugar into the cell for energy production but with very high sugar pancreas becomes numb and doesn't release insulin in response to food. Under such condition pancreas doesn't respond to oral medications and hence insulin usage for initial one month or so helps immensely. Please follow your doctors advice without anxiety. Insulin is the safest and best drug provided dose is adjusted judiciously and diligently. Hope this helps.

Mr. lybrate-user, Diabetes mellitus occurs either due to deficiency of insulin or resistance to its action or both. Hence, the most suitable medicine for diabetes is insulin itself. Hence there is no question of that causing any bad effect on the body. In T1 DM only insulin is given as the treatment for lifetime. Thanks.

Hi doctor, My mom is 49 yrs, she visited a doc n she told to undergo thyroid check up. After the test TSH (ultrasensitive) observed value is 14.29 which is high and T3 (total) is 148.6,T4 (total) is 6.42 they are normal. So can you suggest what treatment should b done? And also what home remedies would b helpful?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
lybrate-user, Thanks for the query. TSH value of 14.29 mU/L is fairly high, suggesting presence of hypothyroidism. This needs to be treated with levothyroxine tablets. Dose of that medication has to be calculated based on the weight, height and TSH and T4 values. So you will have to give me her weight and height details. Plus there are a few other things which she has to follow, like regular exercise, some dietary restrictions and taking the medication on empty stomach early morning. Thanks.
