Diabetes affects all organs in the body ? Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient and doctor should be n constant touch for regular follow up and advice. It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose ? SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS),& HbA1C ( inform me ) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For further assistance you can concern me privately

Treating Plantar Warts

MBBS, Diploma In Dermatology And Venerology And Leprosy (DDVL), DNB (Dermatology), MNAMS
Dermatologist, Delhi
Treating Plantar Warts

Plantar warts are a common occurrence in children, so are palmer warts. These warts are named so according to where they appear in the body. Palmer warts occur on the hands, and plantar warts on the bottom of the foot.

What causes plantar warts and palmer warts?

Plantar warts and palmer warts are caused by a viral infection in the top layer of the skin. The virus strain here is called human papillomavirus or HPV. (Take note that while many strains of the virus exist, those that cause genital warts are different from the ones that cause plantar warts). The resulting warts are non-cancerous growths but are often mistaken to be malignant. In fact, most warts go away in a year or two if you leave them alone without treatment. However, depending on the location they may cause minor pain or irritation. For a self-conscious person, the wart may cause people to shy away from clothes and situations that require their exposure.

How do you identify a plantar wart?
Think of any yesteryear movies where the villains had a distinct look to make them look malicious. Such villains were often planted with a plantar wart! Both plantar and palmer warts are generally small to the size of a small thermocol ball, although some tend to grow bigger. Sometimes, the warts appear in clusters called mosaic warts. Sometimes, a seed like dot appears on the wart as a result of a blood vessel growing into it. Such warts are also called seed warts. Because of the constant pressure from walking and standing, planter warts don't tend to stick about the skin as much as palmer or other warts.

Here are 3 ways to get rid of plantar warts

  1. Try some home remedies: Home remedies described here are quite effective but may take a while before the effects are visible. Also the warts may be painful during the treatment period making it difficult for you to walk. Start by soaking your feet in warm water to soften the wart. Use an isolated nail file or pumice stone to remove the top layer of dead skin. (Make sure you don't reuse the tool for anything else). Next, go for a salicylic acid treatment in the form of liquid, gel or patches. You can also follow this up with a duct tape treatment for 7 days and then remove the duct tape. You can also try freezing the wart with an over the counter medicine like Compound W Freeze Off and Dr. Scholl's Freeze Away. During any home remedy, if the wart starts bleeding, oozes blood or pus or becomes painful you may need an intervention from a medical specialist.
  2. Prevent plantar warts: You get infected with plantar warts through exposure to skin scales that are infected by the virus. Watch for contagious spreading in closed rooms with skin exposures like changing lockers and common showers.
  3. Consult a doctor to try a strong salicylic acid treatment, laser treatment immunotherapy or even surgical options to remove the wart.

Understanding Retrograde Ejaculation

MBBS, PGDUS, Fellowship In Aesthetic Medicine, Advance Course In Diabetes - USA, MD - Medicine

Understanding Retrograde Ejaculation

A very uncommon and rare condition, which can occur in case a person is diabetic or has undergone a surgery, a medical disorder known as retrograde ejaculation. It is a condition in which semen enters the bladder rather than going out of the penis after orgasm. This causes a problem as very little or no semen is ejaculated.

That is why this is sometimes also called as 'Dry orgasm'. The only treatment for retrograde ejaculation is to make the male fertile again because retrograde ejaculation is not particularly harmful to the individual. It has the following symptoms:

There are only three main symptoms associated with retrograde ejaculation.

  1. Dry orgasms
  2. Urine that is cloudy immediately after an orgasm
  3. Male infertility

The possible causes of retrograde ejaculation are:

  1. Surgery such as prostate surgery
  2. Certain medications which cause side effects. These medications are usually administered for high blood pressure, prostate enlargement and mood disorders.
  3. Nerve damage

It is worth noting that Dry orgasms have other causes as well, which includes:

  1. Prostatectomy, which is the removal of the prostate by surgery
  2. Cystectomy, which is the removal of the bladder by surgery
  3. Radiation therapy, which is especially used to treat cancer in the pelvic area

Here is how retrograde ejaculation may get more complications

There are only two main complications which occur due to retrograde ejaculation. These include:

  1. Male infertility
  2. Worries about retrograde ejaculation which may lead to less pleasurable orgasms

Treatment for retrograde ejaculation.

It is worth noting that certain medications, which are used to treat retrograde ejaculation can lead to serious complications and others may lead to a higher heart rate and blood pressure.

Medications which are usually used:

  1. Imipramine which is also called Tofranil
  2. Chlorpheniramine and Brompheniramine
  3. Ephedrine, Pseudoephedrine and Phenylephrine

It is also worth noting that the treatment for male infertility is different and more advanced.

My dad - 71 years - he had ptca done in 1997 and was also diagnosed with diabetes. Till aug 2014 it was all working well with minimal drugs - diabetes under control through diet etc and cholesterol levels quite normal. However, last year in aug-14, he had high fever and got unconscious (10-15 minutes)/fallen down on the way to bathroom and fractured his leg. He does not remember what happened. Got admitted and was administered with antibiotics (novosafe). Meanwhile got catheter inserted as he was not able to pass urine and/or move because of fractured leg. Got released from hospital after 6-7 days but soon after two days again got high fever again and fell unconscious again. Gave bit of sugar and took to hospital - he felt much better by the time he reached to hospital and wsa hospitalized. Doctors suspected uti and diagnosed hypoglycemia. After that he had high fever 2 times and novosafe, piptaz courses for 6-7 days each time worked. Doctor suggested urine culture this jan 2015 and" susceptible to colistin" was found. We got 10 day course of colistin followed by 1 month course of niftas. He did not get high fever of 4-5 days after that. However got fever 5-6 times -- 100-101 degree - for a day or two with normal antibiotics gets over. Each time normal urine test showed few puss cells and couple of times off late not even those. Urine test was quite normal. We observed that high fever occurred after some physical exertion earlier 3-4 times but later 5-6 times after colistin course no such co-relation and got urine test done and puss cells were found very low/none. Usg and uriflowmetry was done in may-15 and not found any issues except that prostate slightly enlarged (which was noticed even in usg in aug 14) and for which doctor advised not to do any procedure except have tamdura (1 at bed time for 2 months) and take nurokind (1 every day for two months). Urologist does not think its recurrent uti case at least anymore. Diabetologist does not think its due to diabetes. Heart specialist also thinks nothing wrong there as lipid profile and color doppler showed normal. However, fever has been repeating every 15-20 days though puss cells are not found now. Every time he gets bit unconscious (5-10 minutes) - starts with some fever. Heavy eys. Getting unconscious. Expert suggested to consult now neuro-physician and cardiologist after usg and carotid doppler tests. Also, another expert doctor suggested it may be yers of diabetes causing the same. However, he suggested urine culture with sensitivity analysis including test for suspected fungiurea and atypical mycobacterial uti to rule out its uti. Not sure what to do (tried to keep it as brief and can give more details).

Dear please give reports of cbc, esr, and usg (kub) with post-void film and also that of uroflow. Please post pictures as well. One more possibility is t. B. As t. B. Is very common in diabetic patients specially with irregular and recurrent fever.
