Many common foods and supplements contain compounds that interfere with thyroid functions. In general, it’s best to avoid the following foods and supplements: Soy: Studies suggest that phytoestrogens in soybeans and soy-rich foods may inhibit the activity of an enzyme that makes the thyroid hormones. One study found that women who consumed soy supplements were three times more likely to develop hypothyroidism. Iodine-rich foods: Some forms of hypothyroidism are caused by a lack of sufficient iodine. In such cases, using iodized salt or iodine-enriched foods can be beneficial. But eating too much iodine can have the opposite effect and suppress thyroid gland activity. Check with your doctor before taking supplements. Iron and calcium supplements: Taking iron or calcium supplements can also change the effectiveness of many thyroid medications. High-fiber foods: Although a high-fiber diet is usually recommended, too much fiber may interfere with the absorption of thyroid medicines. Certain vegetables: Cruciferous vegetables that are rich in fiber like broccoli, cabbage, spinach, kale, and Brussels sprouts can inhibit thyroid drug absorption. Reducing the amounts of such produce in your diet may help. Caffeine, tobacco, and alcohol can also influence the effectiveness of thyroid medicine. Ask your doctor for tips on how to regulate or reduce your consumption. The thyroid is a small gland at the base of the throat that makes hormones to regulate growth and metabolism. These hormones influence a variety of organs, so they can affect body temperature, weight, cardiovascular health, and fertility. Hypothyroidism occurs when the thyroid gland produces too little of these hormones. If your thyroid hormone levels are too low, you may experience symptoms like fatigue, digestive problems, sensitivity to cold temperatures, and menstrual irregularities. Although there may be no cure for hypothyroidism, the condition can be managed with medication. If left untreated, hypothyroidism can lead to many complications, such as heart problems, nerve damage, and infertility.

Rotating the Fetus - Understanding the Procedure!

MBBS, DNB - Obstetrics and Gynecology, DGO
Gynaecologist, Delhi
Rotating the Fetus - Understanding the Procedure!

In a normal pregnancy, the baby develops with its head pointed down, and the head is usually the part which comes out first during normal delivery. However, in many cases, the baby could have its legs, feet, or buttocks pointing to the cervix. In a majority of cases, the baby may have this position, but rotates to have its head pointing down before the third trimester. However, this may not happen, and this is referred to as breech.

In a lot of cases, the doctor would try to move the baby’s head downwards usually around the 37th week, and this is referred to as external cephalic version (ECV) or even as version. The process is done externally by manipulation and hence the name external. It is done before labor and may allow for a vaginal birth. In very rare cases, it may be done during labor, but before the amniotic sac has ruptured. As a backup, there should be a provision for the patient to undergo C-section, if ECV is not successful.

Indications:

  1. Single pregnancy, into 36 weeks of pregnancy, with no complications, and preferably not the first pregnancy
  2. No engagement of the fetus (any part) in the uterus
  3. Adequate amniotic fluid, which will provide a good environment to move the baby with minimal injury

Contraindications:

  1. Suspected/known birth defects
  2. Multiple pregnancies (twins/triplets)
  3. Ruptured amniotic sac
  4. Fetus with a hyperextended neck
  5. Mother’s health is not optimal and is on cardiac medications
  6. Condition that mandates a cesarean section (placental separation from the uterus, placenta covering the cervix, etc.)

Procedure

The fetal position is first estimated using an ultrasound. The position of the placenta and the amount of amniotic fluid are also closely monitored. Under constant monitoring, the uterus is relaxed through medications. With one hand on the fetal head and another on the buttocks, the doctor tries to rotate the fetus. Depending on how much pressure the mother is able to tolerate and how flexible the uterus is, version may be successful (success rate is about 60%).

A second attempt under epidural anesthesia may be done, if the first one did not succeed. However, the chances of success with subsequent attempts is very doubtful. The fetus is constantly monitored through ultrasound and fetal heart rate monitoring. A fetus is considered healthy if the heart rate moves up during this procedure. However, if the heart rate seems abnormally high, the procedure would be abandoned.

After the procedure, the mother and the fetus would be monitored for a while before being sent home. As the fetus is constantly monitored throughout pregnancy, the doctor would be able to tell if this procedure is required. If you wish to discuss about any specific problem, you can consult a gynaecologist.

As I am in 3rd semester on my pregnancy. So pls provide me the food intake chart.
1. Proteins: Especially essential during 3rd trimester (7th to 9th months of pregnancy). for growth, repair of muscle, skin, hair and body tissues. Protein deficiency leads to low immunity, hairfall, feeling of tiredness. Proteins are not stored in our body as any excess protein is converted to energy or fat. (a) Major protein sources: Egg white(white portion), Milk, Soybeans/Soymilk, Fish, Cheese, Tofu. (b) Other protein sources: Beans with wheat, Nuts, Curd, Yogurt (Dahi) 2.Carbohydrates Our main source of energy. Stored in liver and muscles. Carbohydrates are used the most by our body duringexercise. Hence, it is important during pregnancy to consume a diet rich in carbohydrates. (a) Instant/rapid carbohydrates energy sources: Banana, Milk, Sweets, Sugarcane juice. (b) Slow carbohydrates energy sources: Beans, Bread, Cereals, Potato, Pulses, Rice. 3.Iron : Especially essential during 6th to 8th months of pregnancy). Helps in production of haemoglobin and transportation of oxygen to blood cells. 1. Largest mineral found in our blood which also helps in producing energy. . 2. If taking Iron and Vitamin C supplements, do not take them at the same time but after a gap of about 3 hours. Taking Vitamin C tablet helps in absorbing iron. Sources of Iron: 1. Vegetables: Spinach, Dark green leafy vegetables, Lentils(Daal). 2. Fruits: Avocados(Makhanphal), Peach(Adau), Pear(Nashpati), Pumpkin(Kaddu), Raisin(Kishmish), Pomegranate(Anaar). 3. It is not life threatening unless not growing in size. It may take time for her to conceive naturally or may cause miscarriage but not necessarily, so need to consult gynecologist before conception.
