What are the symptoms of dengue and what precautions are to be taken in case if we suffer from dengue.

BHMS
Homeopath, Faridabad
What are the symptoms of dengue and what precautions are to be taken in case if we suffer from dengue.
Hi, Dengue fever, also known as breakbone fever, is a mosquito-borne tropical disease caused by the dengue virus. Symptoms include fever, headache, muscle and joint pains, and a characteristic skin rash that is similar to measles. In a small proportion of cases, the disease develops into the life-threatening dengue hemorrhagic fever, resulting in bleeding, low levels of blood platelets and blood plasma leakage, or into dengue shock syndrome, where dangerously low blood pressure occurs. Dengue is transmitted by several species of mosquito within the genus Aedes, principally A. Aegypti. The virus has five different types; infection with one type usually gives lifelong immunity to that type, but only short-term immunity to the others. Subsequent infection with a different type increases the risk of severe complications. As there is no commercially available vaccine, prevention is sought by reducing the habitat and the number of mosquitoes and limiting exposure to bites. The incubation period (time between exposure and onset of symptoms) ranges from 3 to 14 days, but most often it is 4 to 7 days. The characteristic symptoms of dengue are sudden-onset fever, headache (typically located behind the eyes), muscle and joint pains, and a rash. The alternative name for dengue, "breakbone fever" comes from the associated muscle and joint pains. Do’s and Don’t’s for Patients: If you or any other member is suffering from suspected dengue fever, it is important to carefully watch yourself or member for the next few days, since this disease can rapidly become very serious and lead to a medical emergency. The complications associated with Dengue Fever/Dengue Haemorrhagic Fever usually appear between the third and fifth day of illness. You should therefore watch the patient for two days even after fever disappears. Personal Measures: Use of mosquito repellent creams, liquids, coils, mats etc. Wearing of full sleeve shirts and full pants with socks during the transmission season. Use of bed nets for sleeping during day time to prevent mosquito bite What to Do: Give large amounts of fluids (water, soup, milk, juice) along with the normal diet. The patient should take complete rest. Immediately consult the doctor or report at the Health Centre if any of the following manifestations appear:  Red spots or points on the skin  bleeding from the nose or gums  frequent vomiting  vomiting with blood  black stools  sleepiness  constant crying;  abdominal pain  excessive thirst (dry mouth)  pale, cold or clammy skin  difficulty in breathing What Not to Do: Do not wait in case the above symptoms appear. Immediately consult a doctor. It is crucial to quickly get treatment in case of these complications.

I am in habit of biting my nails. I want to get rid of this habit but unable too. Kindly tell me some effective treat...
There is something that is bothering you (like stress and boredom) and nail biting helps to calm you down. It is a fixation around an oral need and acts as the best way that you have found to deal with anxiety. Do look back into your childhood and see what type of anxiety you have not dealt with and that you had to resort to this adaptation to reduce the tension. Obviously, this will not sort out the problem and so you will continue this behavior even as a habit, for all and any troublesome situation. Once you learn how to deal with life?s pressures, you will not need this self-consoling pacifier. If necessary you could seek the help of a counselor and with his/her directions you could resolve this for good. There are some material means to make it difficult but they are cosmetic: you could wear gloves or mittens, and you could buy a liquid like ?Thum? or some equivalent like that to paint on your thumb twice a day. It will leave a harmless bitter taste to dissuade your biting that digit. Sometimes nail biters have other serious problems too like ADHD, separation anxiety or bed-wetting and so on. I advise, for a permanent solution, to work with a counselor. Studies show 60% of children and 45% of teenagers bite their nails.

I am suffering to allow teeth problem from 1 year which has medicine and tooth past is useful for me.
Tooth brush to be changed every 2 months. Changing the toothbrush alone donot keeps your teeth clean. All toothpaste have nearly similar ingredients like Abrasives, Fluorides, Surfactants, Antibacterial agents, Flavourants, Remineralizers, Fluoride, Diethylene glycol, Triclosan etc. Only the wrong brushing technique that we follow harms our teeth and gums. So consult your dentist, learn proper brushing technique, Brush your teeth twice a day, up & down short vertical strokes, with ultra-soft bristles, indicator brush to protect your teeth and gums from plaque and calculus. Floss your teeth inter dentally once a day. Consume a healthy diet rich in vegetables and lean protein. Reduce your intake of sugary foods and beverages. Gargle your mouth thoroughly after every meal. Drink plenty of fluoridated tap water. Drink not consume alcohol drinks. Don’t smoke.

MBBS, DNB (General Medicine)

Sir, my brother facing some stomach problem since 15 days with vomiting give me some treatment.
It is possible that your brother is suffering from Acidity.Problem of acidity is a tendency in which your stomach starts to produce more acid than required for the digestions of your food. In such a case when there is irregularity in the timings of the meal or there is intake of spicy and oily food items then there is production of even more acid. In such a situation only by the help of medicines it is difficult to achieve relief. A balance should also be made between type of diet, Regular timings of diet and good healthy lifestyle with emphasis on proper sleep. For diet - Fruits are especially recommended because they have an excellent role in prevention of acidity.

Skin whitening

Skin whitening is the practice of using substances, mixtures, or physical treatments to lighten skin color. Skin whitening treatments work by reducing the content of melanin of the skin. Many agents have been shown to be effective in skin whitening.

Uses

Specific zones of abnormally high pigmentation such as lentigo spots, moles and birthmarks may be de-pigmented to match to the surrounding skin. In cases of vitiligo, unaffected skin may be lightened to achieve a more uniform appearance.

Discovery and design

Melanogenesis inhibitors have been discovered and developed through several methods, including: screening of synthetic chemical libraries (high-throughput screening is occasionally used), screening of plant extracts, computational (in silico) search, found as a side effect of previously known drugs and exploration of structural analogues of previously known tyrosinase inhibitors based on knowledge (in varying degrees) of their structure-activity relationship. Thus, the development and discovery of melanogenesis inhibitors illustrates many of the methods used in drug design. Some of the most potent competitive reversible tyrosinase inhibitors are synthetic compounds with a potency hundreds of times that of kojic acid.

Mechanisms of action

Melanin is the main substance responsible for the color of the skin. Melanin is class of dark polymers generated by the body through the process of melanogenesis. Among the melanin pigmenting the skin and hair, 2 types can be distinguished based on its chemical composition and biological route of synthesis: the black/brown eumelanin and the red/yellow pheomelanin. The variation of skin color among individuals is mostly because of variation of the content of melanin in the skin. Skin with little or no melanin is almost white. Other factors influence skin color in a lesser degree, including the amount of blood in blood vessels (because of the color of blood), skin thickness and content of carotenoids in skin.

Melanin in synthesized in melanosomes which are organelles produced in melanocytes. Melanocytes are cells dedicated to this function that are present in the skin, hair follicles, and other structures of the body. The synthesis of melanin (also called" melanogenesis" and" melanization") involves a chain of enzyme-catalyzed chemical reactions and non-enzyme-catalyzed reactions. The main precursor to melanin is l-tyrosine. The first step of melanogenesis is the conversion of l-tyrosine to l-dopa; this is the first and rate-limiting step and is catalized by the enzyme tyrosinase. Other enzymes involved in the synthesis include tyrosinase-related protein 1 (trp1) and tyrosinase-related protein 2 (trp2); trp2 is also known as" dopachorome tautomerase" (dct). L-tyrosine is taken by the melanocytes from the intercellular medium, then transported to the melanosomes. L-tyrosine is also synthesized within the melanocytes from l-phenylalanine by the enzyme phenylalanine-hydroxylas (pah)

Melanosomes are transferred to keratinocytes (the most abundant cell type in the skin). Most of the melanin of skin is found in keratinocytes. Additionally, melanocytes interact with keratinocytes through chemical signaling.
