Diabetes in Pregnant women

The incidence of gestational diabetes is on the rise. At present 10 to 17 percent of pregnant women are developing diabetes during pregnancy. The chances of this are more if the woman is obese or if she had gestational diabetes in her previous pregnancy or a big baby earlier or if there is a family history mother father or siblings of diabetes.

This can be prevented to a great extent if a woman has a healthy lifestyle. Exercising regularly and maintaining body weight before and during pregnancy will minimise the risks of having diabetes in pregnancy.

Endometriosis - 4 Reasons Behind it!

MRCOG, MD -gynaecology, obstretition, MBBS
Gynaecologist, Ghaziabad
Endometriosis - 4 Reasons Behind it!

The abnormal growth of the endometrium tissue outside the uterus is known as endometriosis. Ideally, the abnormal tissue is supposed to break down and exit the body. However, in this condition, the tissue is unable to pass out of the body. As a result, the surrounding tissue tends to get irritated and leads to the formation of scar tissues.

The various causes of endometriosis include:

  1. Immune system disorder: Complications in the immune system might render it incapable of destroying the endometrial tissue that grows outside the uterus.
  2. Surgical scar implantation: Certain surgeries such as a C-section may result in the attachment of endometrial cells to the incision area.
  3. Embryonic cell transport: These cells may be transported to other parts of the body by the blood vessels which may lead to endometriosis.
  4. Retrograde menstruation: Instead of passing out of the body, under this condition, the blood flows back into the pelvic region via the fallopian tubes. When these cells attach themselves to the pelvic cavity, it causes endometriosis.

Symptoms
1. Pain during sexual intercourse
2. Pain during periods including pelvic pain, especially during periods, lower back pain and abdominal pain.
3. Excessive bleeding during periods
4. Fatigue
5. Constipation
6. Bloating

The major complication that results is fertility problems. Endometriosis may prevent the combination of the egg and the sperm and thus, impairs your ability to conceive. It is recommended not to delay pregnancy if you have endometriosis. This disorder also tends to increase the risk of ovarian cancer in some women.

How can it be treated?

Endometriosis is usually done using surgery or medications.

  1. Medication: It usually involves painkillers.
  2. Hormone therapy: Some of the hormonal therapies include:
    1. Hormonal contraceptives
    2. Progestin therapy
    3. Gonadotropin-releasing hormone (Gn-RH) agonists and antagonists
  3. Conservative surgery: If a patient wants to become pregnant, surgery is done as much as possible without harming the uterus and ovaries. Suggested procedures include laparoscopy and traditional abdominal surgery.
  4. Assisted reproductive technologies: An example of this form of treatment is In Vitro Fertilization.
  5. Hysterectomy: Under severe circumstances, total hysterectomy is conducted in order to facilitate the removal of cervix and uterus. If you wish to discuss about any specific problem, you can consult a gynaecologist and ask a free question.

My wife is having too much of bleeding during her periods. What is the solution? note: she is having copper t for the past 1year or so.

My wife is having too much of bleeding during her periods. What is the solution?
note: she is having copper t for the...
It can be a side effect in some women with copper t. I would advice her to have trapic mf tablets three times a day during her periods to see if that helps. Usually for three to four days a month. Also she can have swabs done to rule out infection.

When To Go For Intra Uterine Insemination(IUI)?

MICOG, MS, MBBS
Gynaecologist, Delhi
When To Go For Intra Uterine Insemination(IUI)?

For the benefit of couples suffering from infertility, modern medical science has introduced several innovative procedures. Some of the popular procedures are In-vitro Fertilization (IVF)Intra Uterine Insemination (IUI), Gamete Intrafallopian Transfer (GIFT), Intracytoplasmic Sperm Injection (ICSI), donor eggs and embryos and so on. In addition to these, there are several drugs and surgical procedures that help the couple in getting rid of infertility. Among all these procedures, IUI has gained popularity in the field of gynaecology and infertility treatment procedures. The IUI treatment is also popularly called as artificial insemination procedure. Although this is a popular procedure, it is appropriate that you should also understand its pros and cons.

IUI procedure in brief:

Sometimes nature needs help to start a pregnancy - and the doctor can do this by giving the sperm a piggy back ride through a fine tube into the body. This procedure is called intrauterine insemination (IUI) or artificial insemination with husband's sperm (AIH) - and effectively, the doctor is giving nature a helping hand by increasing the chances of the egg and sperm meeting.

In this method of IUI, the sperms are removed from the seminal fluid by processing the semen in the laboratory and they are then injected directly into the uterine cavity. It is not advisable to inject the semen direct into the uterus, as the semen contains chemicals (prostaglandins) and pus cells which can cause severe cramping; and even tubal infection.

Conditions precedent of IUI Procedure:

Before initiating the IUI procedure, the fast moving eggs are separated from the slow moving eggs. This separation is done in the laboratory. Further, in order to undergo IUI procedure, the women should be less than 40 years of age. On the other hand, apart from healthy fallopian tube, the women should also have higher ovarian reserves. Also, the sperm should have minimum mortality rate. However, IUI procedure is adopted only if the fallopian tube is healthy. IUI procedure is suggested in case the couple is having difficulty in vaginal intercourse, either because of psychosexual reasons or for reasons of physical disability.

The IUI procedure can be performed either with the partner’s egg or with the donor’s egg. Some of the other important aspects of IUI procedure are briefly discussed here:

  1. The IUI procedure is a short duration procedure and it can be completed within a few minutes. This procedure does not cause any discomfort or pain. The procedure does not require any hospitalisation or administration of anesthesia. Further, the procedure also does not cause any side effects. In fact, compared to the IVF procedure, IUI is cost-effective.
  2. In order to enhance the level of success, the gynaecologist may suggest IUI procedure every month. In some cases, the gynaecologist may also prescribe a few medicines to simulate the ovulation procedure. Except this, IUI may not involve extensive medication.

Summary:

You may undergo the IUI procedure under the supervision of sufficiently experienced gynaecologist. Further, you may also ensure the hospital is equipped with modern state of art machineries for carrying out the IUI procedure.

Approach a good art center (test tube baby center) in your area and seek their expertise. Low amh needs evaluation and some treatment. They will do some tests to assess your ovulation and hormonal status. If fine, and if you are ovulating they will help you. At times some medicines may be prescribed to you to help get good ovulation. This is taken for about 3 months and then ovulation attempted. Some patients may have to opt for donor oocyte too.
