Management of Endometriosis

When tissues that usually grow inside the uterus, start growing outside the organ, it is called endometriosis. Painful periods, pain during sex, pain while urinating or bowel movements, extreme bleeding, fatigue, infertility, diarrhea, bloating and nausea are some of the symptoms of endometriosis.

Treating endometriosis usually depends on the following factors

  1. Age
  2. Severity of symptoms
  3. Severity of the disease
  4. Future plans for children

Depending on these the doctor might opt for any of the following treatments

  1. Medication for Pain: If the symptoms are mild, then the doctor will prescribe some pain killers like Nonsteroidal anti-inflammatory drugs. 
  2. Hormone Therapy: Hormone therapy is effective, but once you stop them, your symptoms may come back. Some of the hormone therapies include: 
    • Hormonal Contraceptives: Birth control pills, vaginal rings and patches are useful in controlling the hormones that are responsible for tissue build-up outside the uterine cavity. 
    • Medroxyprogesterone: This drug stops menstruation, which in turn stops the tissue build-up and put an end to all the symptoms. But on the other hand, gaining weight, low bone production and depression are some of the common effects of this drug. 
    • Gonadotropin-releasing hormone (Gn-RH) antagonists and agonists: Hormones that stimulate ovaries are blocked by these. They bring down oestrogen levels and hence prevent menstruation. 
    • Danazol: This drug prevents menstruation and symptoms of endometriosis by blocking the hormones that stimulate the ovaries. However, Danazol may not be a favourable option because of its side effects. The drug should be avoided during pregnancy.
  3. Conservative Surgery: If you are trying to get pregnant, but you already suffer from endometriosis, then removing endometriosis surgically is a viable solution. Your uterus and ovaries will be preserved, which might increase your chances of becoming pregnant. Even suffering from severe pain might find relief from conservative surgery. 
  4. Hysterectomy: In extreme cases, the only way to deal with the symptoms is to completely remove the uterus, ovaries and cervix. Since a hysterectomy means you cannot have children, it is kept as the last option for women in their reproductive age. Hello It is important to know what pills you have taken and in what sequence. Usually pain starts within 3 -4 hours followed by bleeding. Pills for medical abortion if not taken the right way may not be effective. You can wait till tomorrow morning but if the bleeding does not start pls get in touch with gynaecologist.

Planning on getting pregnant can involve taking steps that will help you in having a safe and fulfilling pregnancy and subsequent childbirth. It is important to indulge in preconception planning so as to ensure that you are in the pink of health and ready to go through the process of bearing and birthing a child in the best possible manner. Indulging in this stage is also a way of looking out for your unborn child and ensuring that he or she gets the best conditions during and right after pregnancy. So how can you tell if your body is ready for pregnancy? Here’s a quick list for you!

  1. Type of Birth Control: In the preconception stage, it is important to ask what kind of birth control you have been using. This will make it easier for you to estimate your cycles and know when you will be due for your period once you stop the pill, so that you know roughly when you will be ovulating. Usually, for most birth control pills, the time lag is about two weeks which gives you plenty of time for the menstruation cycle to get back to its normal course.

  2. Vaccinations: You will need to check whether or not all your vaccinations are current and up to date. Before you start trying for conception, it will be best to get these vaccinations done as many of them cannot be taken once you have conceived, due to side effects like fever and more, which cannot be treated with regular allopathic medication once you are pregnant. Remember to specially check about vaccinations for German Measles and Chickenpox, which can prove to be fatal if they strike during pregnancy or the post-natal period, when the immunity of the body dips.

  3. Swelling can be due to either less haemoglobin in your circulation, kidney issues, mumps infection, insect bite etc. It needs to be seen to sussest any remedy.

My friend suggested NT scan is good at this point in time to know the internal structure of baby and any abnormalities as well. Can you please suggest how effective is that scan. PMS - 5 Ways To Help You Manage Better!

What is premenstrual syndrome (PMS)? In a layman's language, it is that time of the month you don't really want to unnecessarily bother a woman or you might have to face the brunt of an emotional outburst. In medical terms, it is a combination of emotional, physical, psychological changes that occur in a woman after ovulation and culminates with the ending of her menstrual flow. The most common mood-related symptoms are irritability, depression, crying, oversensitivity, and mood swings.

The most common physical symptoms are fatigue, bloating, breast tenderness, acne, and appetite changes with food cravings. In severe cases, anger, irritability, anxiety and tension leads to loss of the functioning ability of a woman. If you've been there or are suffering, you are not alone. There are scores of women around the world who experience these symptoms. Here are ways to minimize and get rid of PMS.

1. Control PMS with diet changes: Certain food types in general are not a healthy choice but can cause more harm than good during PMS. Depending on what factor affects you the most, cut down on these types of food.

  • Salt: Excess salt can lead to bloating
  • Caffeine: Can lead to anxiety, sleeplessness and irritability
  • Alcohol: The concoction for depression
  • Sugar: Can cause mood swings

As always, eating more fruits, veggies and grains at the right time can keep you healthy and prepared for those dreaded days.

2. Exercise: Exercise is a mood lifter. It gets you into shape, adds to your stamina and slowly but steadily boosts your confidence to become slimmer, stronger, and fitter. 3. Stock up on vitamins: Hormonal imbalances are the primary cause behind PMS. Though it is not proven, stocking up on some essential vitamins like calcium, potassium and minerals can give you more strength to fight the symptoms.

4. Herbal remedies: They've been around for hundreds of years and you are likely to have an aunt or grandma swear by one of these cures. From ginger to primrose oil, these cures can balance your hormonal activity in a natural way. You will need to experiment to see what really suits your need.

5. Take a break: No matter what physical or emotional trauma you go through, it can double up because of stress and lack of sleep. Knowing your 'off' days, it's best you find a way to take some time off for yourself at a spa or try meditation and deep breathing. Do what you love to do to. Keep your mind calm and clear. Listen to music, take a walk with your dog or simply read a good book.

Abortion pills can be used only till 7 weeks of pregnancy. As you said, it is already 2 months, means beyond 8 weeks already. It can not be done with medicine. It has to be done surgically under anaesthesia. Copper t also can be inserted at the same time to prevent further pregnancy till you want. There is no pain and it is extremely safe if it is done with mva technique.

This bleeding could be bcz of low lying placenta or some other issue, it would be better if you get a dopplers Ultrasound done to confirm for babies well being and thereafter decide for the course of treatment to be taken. You may though immediately start with pregnancy support pills to avoid any complications.
