I am 65 years old. Iam having morning back pain in the lower right side along with the shoulder and right hand pain further frequently my legs also swels. I also deel sleepy always. Kindly advice the precautions that I must take.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. I think it is degenerative issues in your lower back region and other joints like shoulder and hands which may be a cause of pain in your case. So for that, intensive physical therapy, gentle anti inflammatory medications and adequate calcium and vitamin d intake is important. There are many causes of swelling in the legs. Most common reason is postural and many patients develop it when they stand for a long time, but as soon as they elevate their leg up, the swelling subsides. It can happen after any injury which is a priority to diagnose and treat accordingly. Also there could be infection or inflammation which can lead to swelling. Sometimes heavy physical activity may also lead to swelling in your calf muscles. But we have to look at medical causes as well, chronic cardiac conditions can lead to swelling, any pathology which leads to low proteins in the body like deranged lft, kft etc can also lead to swelling. Hypothyroidism can also lead to generalized swelling like appearance in the body. So for accurate diagnosis we need to clinically evaluate and get appropriate tests: cbc, esr, crp, lft, kft, serum electrolytes and thyroid profile. It must be sciatic pain ie. The nerve gets compressed in the low back. For that conservative management would be ice can be kept in the low back where you have back pain. It is suggestible to wear lumbo sacral belt which would help you to build the abdominal muscle tone which itself would make you feel better. You can also wear mcr chappals which would help you to have less weight falling in the painful back.

X-ray must and physiotherapy treatment must. Sit straight maintain your posture avoid long sitting and toward bending. After relief do strength exercise of spine.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. As a general rule, first step is to rule out a bony injury and for that x ray of the affected shoulder is required. If there is no bony injury, I would advise to start with conservative management in form of ice packs, gentle anti inflammatory medication, arm sling and local analgesic spray for application for 5-7 days. If your pain, swelling and other associated symptoms do not subside after 5-7 days time, then we need to evaluate you and look for what soft tissue injury is there exactly? do not hesitate to contact me if you need any further assistance. Yes. But if it's a complete tear and you are experiencing instability of knee, giving way feeling then you will need ACL reconstruction surgery.

You need to correct your posture Avoid being in one position for long duration. Use thin pillow while sleeping. Avoid too much neck bending while working. Whenever sitting, ensure your back is straight, keep a cusion behind you lower back (lumbar region). Shoulder pain can be due to pain radiating from neck. Avoid slouched posture Apply hot fermentation at times when pain occurs & do self stretching of your body

Heat fermentation twice a day and rest must for 2 days and apply pain relief ointment after relief start stretching exercise of spine if it work then ok otherwise tk physiotherapy treatment for 5 days avoid long sitting and standing.

I am 32 years old, I have wrist pain in little finger side vein what should I do?

Kindly show me x rays of wrist & hand before I can suggest any thing. Any way take multivitamins & other antioxidants along with. Ibuprofen 200mg od & sos x 5days do ask for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

Wear knee cap so that you will feel firm while walking or climbing stairs. You have to strengthen the knee by doing quadriceps muscle strengthening exercises.

I am having pain in calf in leg from last 2 years and now from last 1 year I am having pain in my hand and lower back also and now the pain turn into burning and numbness. Done all diagnosis but all are ok and not able to find out the cause of pain.

Have you got your Vitamin D levels, serum uric acid, serum vitamin B12 levels and sugar done, if not they should be done too. Besides the pain in the leg may be due to a nerve compression in the back, that will have to be evaluated clinically and you may need an MRI of the spine if that is the case.

DO take IFT and Laser Therapy for 12 days followed by Strengthening Exercises from Neuro Physiotherapist Best Wishes, and use firm even and hard Mattress for sleep.

Why do you want to go for knee replacement surgery? knee replacement surgery is the last resort if all necessary actions fail. First of all the diagnosis of knee pain has to be made. At the age of 52 years the most common cause of knee pain is osteoarthritis which can be diagnosed by an x ray. If it is osteoarthritis then along with medications few life style modifications are extremely important. You must avoid sitting on the floor, squatting, stair climbing, prolonged standing or prolonged walking, and you must do exercises for quadriceps strengthening. During walking you shoud use functional knee brace with hinge. If there is swelling rub ice for 15 mins 4 times a day. You can also modify your shoe to reduce pain in knee. Shoe modification is based on knee deformity. Most of the cases need shoe with lateral wedge insole for genu varus and calcaneovarus deformities. If all these measure fail to resolve your pain (provided the case is osteoarthritis) you can go for injections like steroid or high molecular weight hyaluronic acid before going for knee replacement surgery as mentioned earlier knee replacement surgery is the last resort. I answered for osteoarthritis knee, but first of all the diagnosis has to be confirmed. X ray knee joint, routin blood test and few special tests will help to get the doagnosis. Thank you.

knee pain in your age group is usually due softening of the cartilage of the knee cap . try quadriceps strengthening exercises and if there is no improvement then you should consult an ortho surgeon to make sure there is no other pathology.

This is basically classified as early degenerative changes in the knee joint when the cartilage in between your joint space starts getting worn away due to age related changes and regular friction of the joint, I would recommend you to avoid too much folding of your knees (like sqatting, sitting cross legged & doing vajrasana) it causes your bones to rub against each other and worsen the condition. You need to start with regular heating of your knee joint with warm water and 1 tsp of epsom salt mixed in it, it is highly beneficial, stat doing stretching of tensor fascia lata muscle & it band stretching, start with quadriceps strengthening exercises twice a day and wait for a month to start noticing changes.
