Shoulder pain can be a result of injury or disease of the shoulder joint. Injury can affect any of the ligaments, bursae, or tendons surrounding the shoulder joint. Injury can also affect the ligaments, cartilage, menisci and bones of the joint. Some minor shoulder pain can be treated at home. Icing the shoulder for 15 to 20 minutes, three or four times a day for two to three days can help reduce pain. Use an ice bag or wrap ice in a towel (putting ice directly on your skin can cause frostbite). Rest the shoulder and avoid any movements that might cause pain. Simple shoulder exercises can help to stretch and strengthen muscles and rotator cuff tendons. A physiotherapist can show you how to do them properly. Please consult a physiotherapist for detail assessment and management of your condition. Best wishes.

When I take my bp readings my first reading is about 145/73 and after 5 min it is 130/75 both taken at rest. Why it is so? And my right arm bp is higher compared to left hand by 10-15 points of systolic. Above readings is of left hand.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Kindly show me photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take biod3 max 1 tab dailyx10 paracetamol 250mg od & sos x5days do back (spine)/shoulder/knee exercises make sure you are not allergic to any of the medicines you are going to take do not ignore it. It could be beginning of a serious problem. If no relief in 4_5 days, then contact me again.

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 1 wk, contact me again. Do not ignore. It could be beginning of a serious problem.

Taking Care of Your Joints as You Age

Skin is one of the most obvious parts of our body, which ages with an increase in the number of candles on our birthday cake. As you age, joints tend to become stiffer and less flexible. The body does not recover from bumps and bruises as easily as it did before, and it becomes difficult to stand for extended time intervals. This is caused mostly by a breakdown of cartilage and inflammation of the synovium.

What is cartilage?
Cartilage is a flexible connective tissue. It connects the ends of two bones in a joint. It is softer than bone tissue but less flexible than muscle tissue. Cartilage is responsible for the smooth movement of joints. 
With age, cartilage begins to degenerate. As the cartilage gets thinner, movement becomes more and more difficult. If left untreated, the cartilage may completely wear off leaving no buffer between the bones. This is one of the leading causes of knee replacement surgery. 

What is synovium?
Synovium is a membrane responsible for keeping the cartilage healthy. It produces a thick fluid that keeps the cartilage and joint lubricated. In cases of osteoarthritis, this membrane can swell up and produce excess fluid. This in turn causes the joint to swell as well. 

Along with age, factors like being overweight, genetics, gender and lifestyle also play a role in the condition of your joints. Being obese point towards a sedentary lifestyle and puts extra pressure on joints, leading to the thinning of cartilage. While men have higher chances of becoming osteoarthritis patients before 45, women have higher chances of developing this disease after 50. A healthy lifestyle is the easiest way to ensure that your joints work well even when you're old and grey. 

Exercise
Movement of the joints keeps the fluid in them moving. It also prevents the cartilage from shrinking and stiffening up. Exercise also makes the bones stronger, and slows the rate of bone tissue deterioration. Stretching exercises are also a good way to maintain the flexibility of joints. 

Healthy diet
Foods that are known as natural antihistamines, antioxidants and foods that fight inflammation promote healthy joints. Some of these are:

  1. Green tea: It prevents the breakdown of collagen and cartilage
  2. Nuts and seeds: These are rich in omega 3 fats that are essential for good bone health
  3. Ginger: It relieves pain and joint swelling
  4. Yogurt: It helps soothe tender joints and fights inflammation
  5. Pomegranate: It protects the cartilage against damage

Hot and cold therapy can also help relieve joint pain and reduce inflammation. As you get older, avoid putting too much pressure on your joints and give them adequate rest. Lastly, avoid smoking, as people who smoke have an increased risk of cartilage loss.

My grandmother has pain in whole left leg permanently since last two years, what was the cause and how to treat it.

There are actually many causes of pain in the legs and we will have to find out a cause in your grandmother: - we have situations in which leg pain actually comes from issues in lower back region when a nerve can get impinged there - pain in calf/leg region can also be due to many other reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. So, I need to know whether there is any back pain, any swelling in legs, does she suffer from blood pressure or diabetes issues? also whether any recent blood tests like vitamin d or b12 etc have been done for her or not? do not hesitate to contact me if you need any further assistance.

This could be due to lumbar spondylosis with nerve root compression. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take paracetamol 250mg od & sos you will need other supportive medicines also. Do back (spine) exercises make sure you are not allergic to any of the medicines you are going to take you may have to use lumbosacral support for some time. Avoid painful act/activities. Take medicines as prescribed it may have to be further investigated. Do ask for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

I have spondylitis in lower back. What should I do?

Tips for cervical spondylitis: 1. Peel two cloves of garlic and crush it. Add this to a glass of milk. Boil it on a mild flame till one-fourth glass remains. Filter and make two equal parts. Drink one part in the morning and the other in the evening. 2. Warm mustard oil or sesame oil (1 teaspoon) with 3-4 cloves of crushed garlic and 1 tsp of ajwain (carom) seeds until it roasts to a reddish brown colour. Ensure that it does not get burned. Massage this oil on your affected areas. After that do hot fomentation with salt potli (bag). 3. Take 1 tsf triphala churna daily. N practice yoga daily twice.

What is the severity of pain? is the pain continuous or intermittent? is it pulsating? since pain is a complex phenomenon, there are many treatment options medications, therapies, and mind-body techniques. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.
