Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Treatment includes intensive physical therapy, short course anti inflammatory medications according to one's suitability and maintenance of calcium/ vitamin d metabolism. If there is no relief, then we might have to consider arthroscopy of your shoulder to improve your movements. Do not hesitate to contact me if you need any further assistance. I am 36 years old male suffering with hypermobility of joints. It was diagnosed 2 years ago with stiffness and pain in lower back and sacroiliac joints. My joints are very weak and get hurt easily since my childhood. Its been more than a month that I took stairs to office for a week instead of lift. I used to climb 15 floors without lift. My knees started paining and then I stopped doing it. Now my knees are swollen and they are not getting better even after icing or using hot water bottle. All the tests in the past were negative. The tests done were RA Factor, HLAB27, BL17, CRP, cbc, MRI and complete spine screening and Bone scan of lower back. My lower back is fine now but I have swelling in my knees and pain in left shoulder and left wrist.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Your knee pain could be due to certain reasons but common ones are chondromalacia, pain due to knee instability, inflammation etc. If there is no history of any injury, and no s/s of inflammation or infection like swelling, skin redness or local warmth on touching, then I will like to advise you for conservative management initially in form of: - intensive physical therapy with focus on strengthening of quadriceps and stretching of other group of muscles - gentle anti inflammatory medications like tab etoshine or tab paracetamol can be taken in small doses for 5-7 days - adequate calcium and vitamin d supplementation as per requirement - ice packs at home for 3-4 times in a day and see how it goes for around 2 weeks time. If your condition does not improve, then we will have to get certain investigations done. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Most of us will suffer from back or neck pain at some stage during our lives. The good news is, however, in most cases the pain is not serious in nature and can be remedied. Postural related back pain generally stems from sitting for long periods with overloaded or tight back and pelvic muscles. For example, if your rear thigh muscles (hamstrings) are tight, when you sit they will pull the pelvis out of position and increase tension on the lower back muscles. This in turn overstretches your back muscles and with lengthy periods of sitting will cause pain. This is one example of many variations of postural related back pain. However, one thing will be in common, progressively developing back pain from long periods of overstretched and overloaded back muscles and ligaments. When you first start to develop postural back pain it manifests itself as a low grade ache which tends to be relieved with light exercise or avoiding sitting. But after a while the pain increases and tends to be there most of the time regardless of what you do. It should be noted that a small percentage of back pain is caused by more serious conditions like a prolapsed disc or nerve compression. Symptoms that suggest a more serious back problem are leg pains, pins and needles or weakness in your leg or foot. These symptoms need to be properly assessed by a Chartered Physiotherapist So what can you so to reduce back or neck pain? Firstly, break the cycle of sitting for long periods by getting up and moving around every 20 minutes or so and by introducing simple movements that can by Done at anytime, even when sitting (see below). Secondly, a daily routine of stretching and relaxing tight thigh (hamstring) and buttock (gluteal) muscles will help to prevent overloading your lower back muscles when sitting. Thirdly, reduce back or neck pain by relaxing the surrounding muscles and ligament by introducing the following routine 2-3 times a day: 1.Apply heat to the back muscles. A shower, massage or hot water bottle works best. This will help to relax the muscles. 2.Release and relax back muscles through massage, use of a tennis ball or foam roller for 5 to 10 minutes twice daily – purchase a foam roller from www. Physiosupplies. Com/exercise/foam-rollers Here is a video of foam rolling on YouTube 3.Apply movement to the lower back muscles using the suggested exercises below. About a minute of each 3 or 4 times a day will help to recover the overloaded lower back muscles. The components of the word'osteoporosis' literally mean'porous bones' -'osteo' is for bones, and'porosis' means porous - helpfully describing this condition that results in reduced bone density and increased fragility of the bones. The thinning of the bones in osteoporosis, combined with the formation of weaker bone crystals, puts people at a higher risk of fall and fractures. It can affect anyone of any gender, but it is particularly prevalent in postmenopausal women due to the sudden decrease in protective oestrogen levels after menopause. Signs and symtom Bone loss that develops slowly, leading to osteoporosis, does not cause any symptoms or outward signs. As such, a patient may only discover that they have osteoporosis due to an unexpected fracture after a minor fall or a slip or strain - or even a simple cough or sneeze - may result in a fracture. Breaks in the spine can lead to altered posture, with compressed vertebrae creating the stooped appearance often seen in older people Diagnosis Osteoporosis may be diagnosed directly through the use of a bone scan that measures bone mineral density (bmd). Central or peripheral dxa. Osteoporosis is diagnosed when the t-score is -2.5 or below.
