Hi I can understand your problem 1) avoid activities that make your symptoms worse 2) apply ice packs 3) start physiotherapy treatment as electrotherapy will reduce pain and inflammation 4) exercise and manual therapy will help in pain free movement. My mother has been suffering from chikungunya for 2 weeks. Her age is 45 and has weight of 70 kg. She has swallowed legs, joint pains in her legs and hands. We have consult a doctor at safdurjung hospital. They have advised to give her a lot of fluid items and paracetamol tab. But I have recently read in newspaper about unani medicine for chikungunya which has been given at ram manohar lohiya hospital. I want to know that is unani medicine is effective and can it cure the disease? What should we do? And the treatment which we are following now, is it right or not?

Dear lybrate-user, namaskar, The treatment advised by Dr. S of safdarjung hosp. Is correct and scientific and evidence based. So stick to that regimen. The disease has it's natural history and takes it's own course to settle. So better have patience and faith on your doctor. Take care.

You should go for a walk first two km. Increase it slowly. After that you may do jogging slowly. Do massage of your legs daily at night. Take vitamin d.

Irregular periods r quite common in this age group, But you should use effective contraception as still there are some chances of getting pregnant. Do a urine pregnancy test yourself. If its negative. Then let me know I will prescribe you medicine for white discharge and morning sickness.

Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. Typhoid can Def take a lot out of a person. possibly it might have caused some lesions in the intestine that could be causing malabsorption of nutrients. I recommend just take your medicines regularly so that the infection completely resolves. give yourself sone rest and let the body heal and recover. And eat well. only eating good food will GIVE u your strength back. medicines won't.

It could be due to deficiency of vitamin d3/b12, calcium, hemoglobin. Get your blood examined for above and consult physician with report for advice and treatment 1. Take good nourishing diet with plenty of green leafy vegetables and, fruits 2. Drink plenty of water with electral/ors powder 3. Take multi vitamin supplement like one capsule of polybion daily 3. Massaging cramped muscle, take pain killer like one tablet of crocin pain relief sos after food up to a maximum of three tablets per day 4. Hot fomentation by heating pad, warm water bath 5. Stretching exercise etc 6. Take adequate rest, avoid over exertion and dehydration 7. Consult physician for further advice and treatment.

Coccydynia (Tailbone Pain)- Physiotherapy Treatment Approach

We all have lost our tail with evolution…but did you ever think that the vestigial (redundant) part of that tail which has been left over in us in the form of tailbone can mess up with our daily activities and torture us to the level of not allowing us to sit!

If you are not able to sit or if you are experiencing pain when sitting right at the tail bone region, you may be having ‘Tailbone Pain or Coccydynia’.

What is Coccydynia?

The coccyx, also referred to as the ‘tailbone’ is the last segment of one’s vertebral column. Coccydynia is referred to a disabling pain that occurs in/around the coccyx. This type of pain usually starts when one sits all of a sudden or rises up from the seat after having been seated over a long period of time. Also known as coccygodynia, this condition can mar one’s quality of life. The pain, often described as ‘stabbing’ or ‘piercing’, can radiate to the buttocks, the lumbar spine, and rarely, to one’s thighs.

Anatomy

The coccyx is the final part of one’s vertebral column. The vertebral units are fused together. The frontal portion of the coccyx is the fusion site of the muscles and ligaments which control the functions of the pelvic floor. The coccyx also supports the anus’s position. The gluteus maximus is attached to the posterior part of the coccyx. Ligament or muscle damage, or muscle weakness can cause the coccyx to assume an abnormal position, thus causing pain.

Common Causes:

  1. Sitting for extended periods of time, especially on hard surfaces

  2. Effect of a direct trauma, such as a fall

  3. Giving birth

  4. Bone fracture, tumors or an infection can also cause this condition

How do you know whether it’s Coccydynia?

  1. You will experience pain while sitting, especially on hard surfaces

  2. Localized pain in or around the tailbone that worsens with touch or any pressure applied on it

  3. The pain will become severe when you stand from a seating position after considerable amount of time

  4. The pain might also start during sexual intercourse

  5. The pain increases in women during their menstrual cycles

Diagnosis

  1. Local Coccydynia

    • Traumatic

    • Idiopathic

  2. Radiated or referred Coccydynia
  3. Psychogenic Coccydynia

  4. Chronic proctalgia

Physiotherapy Approach For Treating Coccydynia

Patients diagnosed with Coccydynia are advised to avoid factors which might provoke the pain. The initial line of treatment will include making certain adjustments such as applying gel cushion when one is sitting for extended periods of time. This helps reduce localized pressure and improve his posture. Other modes include:

  1. Mobilizations: This can help realign the posture of the coccyx. Initially, there might be tenderness; hence, it is advised to start with rotational mobilization. To start with, either side should be mobilized first.

  2. Manipulation: This can be done intra-rectal and when the patient is lying in a lateral position. The coccyx is repeatedly extended and flexed with the help of the index finger. However, care must be taken so as to not hurt the rectal mucosa.

  3. Massage: Massaging the coccygeus muscles has also been proven to relieve pain. Biofeedback can also be integrated with it in order to avoid incidences of muscle pull on the coccyx. If you wish to discuss about any specific problem, you can consult a physiotherapist.

Improve Your Posture Before it’s too Late

A good posture makes you stand tall. The right body posture is crucial for a healthy and strong, disease-free body. A correct body posture can help to alleviate all kinds of problems that might arise due to the practice of wrong posture such as chronic neck pain, back pain, and headache among others. Here’s how to avoid some of the common posture blunders:

1. Hunchback

You may know that sitting for long hours is bad for your health, but sitting hunched over the computer screen is especially harmful for your spine. When you’re in this posture, the chest muscles of your body tighten causing the top of the spinal column to form a C-shape. This can lead to pain in the back, shoulder and neck. To prevent your spine from curving in this manner, you can do the bhujangasana pose to strengthen your back.

2. Forward head

Likewise, sitting in a stooped manner in front of the computer can cause the problem of forward head and associated neck pain. Performing head nods on a daily basis can provide respite from the ache. For this, move your head towards the neck until your chin touches your neck. Hold on to this position for 5 seconds before going back to your original position. Repeat as needed.

3. Rounded shoulders

Intense training of the chest muscles or typing while not sitting straight can cause your shoulders to swivel forward, leading to rounded shoulders. This can prevent your arms from working at their full capacity while running or hamper your breathing ability while performing strenuous activities. One of the ways that you can fix this postural blunder is by doing some stretching exercises to relax your chest muscles.This workout can be easily done anywhere, as all you need is a doorway. Position your arms against one side of the doorway with your elbows bent at 90 degrees. Then, stretch your chest and shoulder muscles by placing your foot through the doorway. Hold the posture for about 30 seconds Whether your postural problem is mild or severe, it can affect your overall health and performance.

Wrong posture can cause you to experience skeletal system disorders such as kyphosis (outward curving of the spine) or lordosis (inward curving of the spine). If not corrected in time, the wrong body posture can also impact your self-confidence.

I had near accident while driving two wheeler around month of march , not visible wound but I stretch my right leg to avoid accident. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.
