IVF Protocols For Low Ovarian Reserve / Poor Egg Quality!

The available eggs in the ovaries at a time are collectively called an ovarian reserve. Low reserve happens when the production of eggs reduces. This affects the chances of pregnancy. The general cause of low reserves can be aging ovaries. In such cases, the ovary may be healthy and functioning even if the reserve is low. Production of eggs lower as a woman ages. A woman starts with 250,00 to 500,000 eggs at puberty and ends up with 1000 eggs at menopause.

Causes:
Low reserves are caused by

  1. Production of eggs decreases: Chromosomal abnormalities like Turner syndrome (lack of two X chromosomes) and genetic anomalies like Fragile X can decrease egg production.
  2. Ovarian tissue damage: Rough torsion, endometriosis triggered ovarian cysts, malignant or benign tumours, surgical removal of ovary or any other part of it, chemotherapy or radiation, pelvic adhesions, immunological problems or high BMI (Body Mass Index) can destroy the ovarian tissues.

IVF Protocols:
If a woman has low ovarian reserves, then she will be put on the either of the following protocols:

  1. Short GnRHa Flare: Gonadotrophin releasing-hormone-agonist (GnRHa) like Lupron, Nafarelin, Synarel or Buserelin is administered. This therapy is initiated at the onset of menstruation. The goal of the treatment is to stimulate the release of the follicular stimulating hormone (FSH) which augments the ovarian follicular growth.
  2. Combined Clomiphene or Gonadotrophin/Letrozole Stimulation: Older women are mostly administered this protocol. But it is strongly advised not to, as this protocol can potentially harm the egg or the embryo.
  3. Mid-follicular GnRH-antagonist protocol: Once the GnRHa-agonist protocol is commenced, the GnRH-antagonist is given several days later. Once the follicles reach the size of 12mm, GnRHa antagonists like Ganirelix, Orgalutran, Cetrotide and Cetrorelix are added. These drugs stop the pituitary from secreting the luteinizing hormone (FSH leads to secretion of this hormone).
  4. Long GnRHa Pituitary Down Regulation Protocol: This is the usual approach to deal with low ovarian reserves.
  5. Agonist/Antagonist Conversion Protocol: This protocol inhibits FSH production. This protocol generally yields good results.

Laparoscopic Surgery - A Complete Guide On What To Expect!

Laparoscopic Surgery, also known as Keyhole Surgery, is a minimally invasive technique in which operations are performed through small incisions made far from the location. Laparoscopic Surgery has many advantages over the traditional open surgery procedures. It is reliable, risk-free and reduces the chances of huge pain and hemorrhaging. Smaller incisions are made while performing this operation which enables a faster recovery.

How is it performed?
Laparoscopic Surgery is performed with the help of an instrument called Lparoscope. It has a thin tube-like structure and is extensively long. The camera at the front of a laparoscope is inserted into the body through a hole that allows doctors to perform surgery by watching real time video on the TV screen and it is much more convenient than open surgery.

Reasons Behind Performing Laparoscopic Surgery
Abdominal Problems are usually solved by using techniques such as MRI, CT scan, and Ultrasound technology. If these fail to provide enough information, then laparoscopic surgery is performed for a better diagnosis of the condition. It can also be performed in a case when the doctors need a sample of tissue from a part of the abdomen. Laparoscopic surgery is famous and most preferred in Bariatric Surgery and Gallbladder Removal.
This minimally invasive technique is widely common for weight loss.

Laparoscopic Surgery is safe and trusted by many surgeons worldwide, and it can be performed on patients who have undergone laparoscopy surgery beforehand as well. The only thing that is needed to be kept in mind is that the keyhole is to be made should be from a different site. Laparoscopic Surgery allows the patient to start having food normally after a couple of days of the surgery. It reduces hospital costs as minimal days are spent in the hospital, and it helps the patient to continue with their normal life early.

Types of Laparoscopic Surgeries

  1. Hand-assist laparoscopy: Hand Access Devices are new Laparoscopic devices which help the doctor perform various functions having their hand inside the surgical location. Such hand-assist surgeries are called the hand-assist laparoscopy and are used in surgeries like colon removal and kidney removal.
  2. Robot Assisted Surgery: This is the most commonly used laparoscopic technique which surgeons perform in a less invasive way. This computer based system is known as the Da Vinci system. The surgeon performs this type of surgery with the help of two masters which are similar to joy sticks. These masters control the two arm-like instruments which they insert into the body.

Preparing for Laparoscopic Surgery: There are no such preparations that are required to be taken before laparoscopic surgery as your doctor might help you out with everything. If you are taking any prescribed medicine, your doctor must know about it. Before laparoscopic surgery, blood test, urinalysis, ECG, and other required tests might be advised by your doctor. It is best not to eat or drink anything 8 hours before laparoscopy.

There is no fix time... it is just when both the partners are willing and happy to do it... And before marriage just use condom to avoid pregnancy or contracting any disease.... they are best....

We had sex without condom. It was her 9th day of the cycle. She had taken I Pill after that on the same day itself. But on the 18 th day of the cycle, she had minor bleeding per vagina. We thought its spotting. But the following days there was heavy bleeding which continued for 2 more days. Can this be pregnancy?

If she's had heavy bleeding, it can be period or a Miscarriage Get ultrasound abdomen done Pregnancy test remain positive after MTP and Miscarriage Do urine test for pregnancy Negative results from 2 labs will be confirmatory.

Hi, due to vidyaarambam for my kid I have delayed my periods by using a tablet primolut N. I started on 13th Oct and had 6 tablets but after having a tab on 18th Oct, I got my period on 19th Oct. Kindly let me know whether this kind of tablets really works to delay periods. The process didn't work out for me. As intimated by the pharmacist I had it on a correct time without delaying or missing the timing. Kindly suggest on this.

Yes, they do work but nothing is 100 % in life. Also there are other tablets which work better but may have extra side effects. Also if taken much before expected period has better effect compared to when patient goes for advise very close to next period.
