Mother (aged 74 years) History: Diabetic, High BP, Thyroid, Parkinson, Kidney (swelling in legs). occasionally coughing (both dry and with phlegm). On Meditation. On 24th Mar, 2013, admitted to ICU, MKW Hospital (J Block, Rajouri Garden, New Delhi). Did not show much breathing problem but there was heaviness in stomach and rib side pain. Chest x-ray suggested fluid accumulation (Pleural effusion). First need extracted 1500 ml fluid, then tube inserted. Fluid continued coming (approx 6500 ml). Still coming till 28th Mar. Mother feeling better and is able to breathe without mask. However, on 27th late night, the tube was blocked and mother had breathing problem. Quick x-ray showed the blockage in tube which was corrected. On 27th, CT Scan was done. The finding is as under" IMPRESSION:Right side pleural effusion with subsegmental collapse in bilateral lower lobes. Chest tube noted on right side. Few subsentimeteric sized mediastinal lymph nodes seen. Patch opacities seen in right upper and middle lobe -?Infective [Tubercular] Mild to moderate ascites with mild diffuse wall thickening noted in caecum, terminal ileum and few small bowel loops with few subcentimeter sized mesenteric lymphadenopathy -- likely infective [? Tubercular. Since the CT Scan report was not conclusive, doctors suggested bronchoscopy. Not done so far. Based on above facts and findings, what could be suggestion (whether TB, cancer, lymphoma, any infection or another thing). Since volume of fluid is very large and is keep coming from the tube. It seems it is also keep accumulating very fast now? Otherwise, mother is stable, easily taking breathe and eating. Active in thinking, listening and speaking. Keep demanding for going back home.

Sir, I am 35 year old, I have pain in right solder, (not accident), 20 days back I am sanding on the chair for some work, the chair position on the bed. The chair slip my right hand go back a speed. Do hot fomentation any way take bio d3 max 1tab odx10days ibuprofen 200mg od & sos x 5days do neck & shoulder exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take.

I have problems of forgetting names of persons. Whenever I am talking with someone or about any topic I lost the words. Couldn't find out the appropriate words. But the word or the name is very easy and known for me. Requires detailed examination and investigation. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Kidney Donation - Things You Must Know About It!

Whenever we choose to undergo a surgery, there are a lot of questions that we try to seek answers for. Here's a list of things one must know before he or she opts for kidney donation!

What is the rate of recovery?
The recovery period of the donor is usually in the range of 5-7 days. However, it depends on how fast an individual recovers and what is the kind of procedure performed on the patient. It can greatly vary from person to person. The donor might feel an itching sensation and experience pain as the incision heals. Activities such as contact sports and heavy lifting are not advised, post 6 weeks of surgery.

How does the transplant affect the donor?
Living with one kidney is not unusual and it is possible to lead a normal life with some to no problem at all. The donor’s kidney has a tendency to increase in size after the transplant. This happens to make up for the lost kidney. While some physical exercise is healthy for the body, activities that require heavy body movement should be avoided. Donors are required to go through extensive medical check every six months to avoid any possible complications.

Does donating a kidney affect life expectancy?
Life expectancy does not decrease after kidney donation and the chance of the existing kidney failure is limited too. Studies have revealed that a donor has a tendency of developing a high level of blood pressure. Having said this, a thorough discussion with the transplant team should be done to discuss any possible complications that might occur post the surgery.

Mental state after kidney donation:
A donor can go through a mixed set of emotions after donating a kidney. Overwhelming joy, depression, relief and anxiety are some of the common emotions a donor goes through. Studies have shown that less than 1% of all donors show any signs of regret. On the contrary, more than 80% donors reveal that they would have done the donation anyways. The donation experience is generally positive. If, however, a donor is going through emotions he cannot handle, he should join a self-help group or report to a doctor.

Is there any side effect of kidney donation?
Apart from the scar a patient receives after the surgery, there aren’t too many complications. Having said this, there are tendencies to develop high blood pressure, proteinuria and a reduced functionality of the kidney, resulting in pain. Long-term diseases such as nerve damage, obstruction, and hernia can catch up as well.

Is there any dietary restriction?
It is entirely possible to go back to your old and regular diet. However, in the case of any complications, a patient might have to skip few things from the diet. It is a good idea to get a clear picture about the dietary requirements from the kidney transplant team.

Does donation hinder in getting pregnant?
The medical advice from a doctor is that it is not a good idea to become pregnant till at least six weeks post the surgery. However, it is entirely possible to get pregnant after kidney donation. But it should be ensured that a patient receives very good prenatal care to reduce any possible complications. If you wish to discuss about any specific problem, you can consult a Nephrologist.

To gain weight you should eat more calories than you burn. Plus not only you have to eat calorie dense food but you have to keep in mind that what you are eating is healthy too. In the process of weight gain you do not want to put weight in an uneven manner and look unhealthy rather than looking bright and healthy. 1. Start with changing your lifestly. Start working out if you are not active. The biggest myth that prevails now a days is that thin people do not need to work out. This is wrong. When you challenge your body, it is forced to grow. But this growth can only be possible if you are eating enough food to let the body absorb and grow in size and muscles. 2. Eat Healthy – Stop drinking and smoking too much if you are indulged in it. Limit your intake of junk food, pepsi, sugary stuff etc. Do not at all starve yourself. Make it a habit to eat something healthy every 3 hours. This means you have to eat 6 small meals in a day. Your Daily food intake should ideally consist of – a. Have plenty of nuts, dry fuits and seeds. Almonds, walnuts, sunflower seeds, pumpkin seeds, figs, dates and raisins. They are full of healthy fats needed by a human body. b. Good amount of fruits and vegetables – Bananas, apples, pomegranate, green leafy vegetables. Fruits and vegetables helps in absorption of food in the body and together, completes your daily nutrition need. c. If you are a non- vegetarian - Eat eggs, chicken etc. d. Eat whole grains – Try to have multi grains spread throughout the day. Wheat, brown rice, oats, bajra, daliya etc should be taken in multiple meals in a day. e. Eat Sprouts – Chana and moong which are full of protein and nutrients. f. You can supplement your diet with good quality multi-vitamin capsules. If you are lacking in protein, you can buy a high quality whey protein and have it with banana shake twice a day. g. You can take natural food items like wheat grass which is full of essential nutrients which helps in weight gain. Try to be patient while you are on the process of weight gain. Results take time to show. Just follow the regimen of weight gain religiously and you will soon start seeing the results.

Get x rays both hips ap and lateral views and x ray dorsal spine ap and lateral views asking with vitamin d levels, serum calcium, phosphorus, alkaline phosphatase and serum uric acid levels ( blood tests) fasting and revert back with reports. Also get her bone densitimetry 3 sites.
