Know The Procedure Of Epidurals!

MD, FIPP
Pain Management Specialist, Kolkata
Know The Procedure Of Epidurals!

Are you worried about the labour pain? It is perhaps the most difficult phase of a woman's life. But today, there is no reason to worry for. Thanks to epidurals, you can give birth to your baby without experiencing the pain involved.

Epidural is the new age anaesthesia. It is used to block pain in a specific region of the body. A patient is not made unconscious fully at the time of any surgery. He or she is given epidural for regional anaesthesia. Its use has become popular during childbirth. A pregnant woman is given epidural at the time of labour or c-section. It provides relief from the pain rather than cutting off the patient's feelings or senses.

Although it has become a widely accepted technique, women worry about the safety of epidurals. They are concerned about the safety of the baby and their health. But it is absolutely safe.

  • How do these work?: There are nerves in our body, which carry the pain signals to the brain. These nerves pass through epidural spaces. Therefore, anaesthesia is injected into these spaces to stop the pain signal from reaching the brain. This blocks the pain. If you are pregnant and thinking whether to go for epidural or not, you will have a lot of questions in mind. Here's all that you need to know about epidurals:
  • Effect on baby's and mother's health: Use of epidurals tends to increase the time of labour by 20 minutes approximately. The mother may develop fever. However, it blocks the pain. This is some relief to the mother. As there is no pain, stress hormones are not released. Thus, blood will not be diverted from uterus and placenta. Baby's supply of nutrients and oxygen will remain intact.
  • How painful it is?: Women mostly worry about the painful insertion of the epidural needle. However, this will not hurt you. A small needle is first inserted to make the area numb. Thereafter the bigger needle is inserted. You may feel the discomfort for five seconds. The epidural medicine starts functioning just after 5 minutes of insertion.
  • Procedure: First the patient is made to take the suitable position. While some may be asked to lie sideways, others may be asked to sit straight. The area is cleaned with a solution. The small needle is first inserted to create numbness in the area. Once the area is numb, the bigger needle is inserted. You might feel a pinching sensation. Thereafter, a catheter is threaded to the epidural space, where the nerve carrying signal of pain passes through. Epidural medicines are pushed into the region via the catheter. Epidurals don't affect your nerve endings or spinal cord. They are pushed into the epidural space.

Pregnancy is possible only on one day of cycle, day of ovulation when ovum is released from ovary of woman. This is often deranged in PCOS that is why Fertomid type of treatment is given for ovulation. For pain you need to be reexamined by Gynecologist. Remeber in PCOD treatment for pregnancy may be long term.

Health Facts

Gist of rujuta divekar's talk today organised by borivali medical brotherhood:

1. Throw out your weighing scale cause it is not a reflection of how healthy you are.

2. Bmi is not an accurate representation as each of us is different. Instead two new terms have come into play- tofi and foti (thin outside and fat inside and vice versa) means you are unhealthy even if you look thin on the outside.

3. New research is showing that cholesterol is not directly the cause of heart disease.

4. Eat 4-5tsps of ghee everyday. It is good for the gut bacteria and will actually help in reducing abdomen fat.

5. Bring back rice on your plate. Not brown not white but handpounded. Eat rice at night too if you have grown up eating it. Leave the quinoa to the south americans

6. Stop looking at food as carbs, protein & fat. Instead look at it from farmer, trader, consumer point of view. So eat what takes the least time to come to you - eat local!

7. All fruit is the same. All have fructose- again eat local grown seasonal fruit.

8. Bananas are good for you.

9. Eat before you drink alcohol - eat early, drink early.

10. Weight loss industry has somehow managed to get everyone to believe that what tastes yuck is healthy and good for you. Don't equate healthy with yucky tasting food.

11. Keep moving. For every half hour you sit, move at least 3mins.

12.150mins of exercise a week is mandatory.

14. Weight bearing exercise is what one should be doing to keep bones strong and for stronger muscles.

15. Oil your hair once a week, rub ghee on your feet and drink plenty of water to alleviate bloating.

16. Salt is not bad for you. Don't go salt free. Use rock salt.

17. Sugar is also not bad for you provided you eat it the right way. Indian sweets are better than your pastries and brownies etc.

18. Work on strengthening your legs.

19. Don't believe that drinking green tea will make you lose weight. A myth that google and weight loss industry has promoted just because chinese are thin.

20. Red meat is absolutely fine as long as you watch the quantity.

21. Latest research is linking wine to breast cancer

22. Eat so much that when you get up from the table, you should feel light.

23. Don't count calories

24. Most important - remove fear from your head while eating. (we all know about the mind-body connection by now)

25. Make lifestyle changes and don't follow fad diets which rob your body of essential vitamins and minerals)

Hi Theoretically, precum can cause pregnancy but chances are very slim. It becomes a too long journey to survive for sperm from precum to finger to vagina. Sit back & relax

Hi what are the precautions to take when we have sex first time and any precautions to be taken to get pregnant in 3-...
Pl see that hygiene should be maintained. As far as sex success is concerned, simultaneous arousal, foreplay, orgasm should be coordinated. For getting pregnant in 3-4 months, sex should be done between 7-14 days of m. C. Your husband's shukra dhatu should be perfect qualitatively and quantitatively. So drink milk, ghee to be used in food, fruits, dry fruits.

Lupus anticoagulant is an autoantibody associated with excess blood clot formation. It is done when you have had signs and symptoms of a blood clot in a vein or artery (known as thrombosis or thromboembolism); when you have a prolonged PTT test; when you have had recurrent miscarriages. Heparin is given to as a treatment is such case so the clot formation is not there.
