Good morning everybody. I am Dr Vandana Hegde. I am practising reproductive medicine, the department of Hegde fertility centre for the last 10 years. Today I would be speaking about the various aspects of fertility. So I will start from the basics to know what happens exactly and what are the problems that arise.

So here is a normal menstrual cycle of the female. There are certain things you need to know. What happens is during the onset of the periods, when the menstrual cycle starts, the eggs start maturing. Now, this egg grows in a wall filled substance which is called Follicle. So normally when the egg starts maturing, it reaches the complete maturity level on the 14th day of the cycle, an average of 12-16 days. As the egg starts maturing, the follicle size increases. Now when the maximum diameter of an egg is reached, the follicle burst and egg comes out. This process is called Ovulation.

If this ovulation happens on the correct 14th day of our cycle, over the next 14th day if there is no pregnancy then a girl will resume her menses which is a failure to conceive. If it happens regularly then the girl will have a regular menstruation cycle of 28-30 days. And if a girl is having the irregular menstrual cycle, then the ovulation is not happening correctly on time. So this is known as Ovulatory disorders. Now, what happens after the ovulation round.

Once the egg is released, it is caught by fallopian tubes. Fallopian tube has finger like projection and the eggs are normally kept on the last part of the tubes. Now when the couples meet, the sperm is released in the vagina. The sperms have basically the head, body and the tale. As I had mentioned earlier, once the ovulation happens, the eggs do not have any tale, so it needs a tube to collect it an d store it in the tube. The sperms will move into the uterine cavity and move into the fallopian tube and have to reach the other part of the tube. So at this point, the sperms will be reaching the egg.

There are certain criteria for normal semen. So when do we know that it is a fertile semen for which there are some criteria like the volume should be atleast more than 1.5 ml ejaculate.And the sperm count should be more than 15 million per ml of sperm. Because of the tale fraction of the mortality, there should be active mortality of the sperm for more than 30%. So when the sperm enters the egg, the process is called Fertilization. And the egg and the sperm will fuse to form an Embryo. The early embryo is called Zygote which is a single cell. So when this forms, the embryo has to divide. In that tube the embryo undergoes division. So in that cell, the embryo will divide into 2 cells, 4 cells, 8 cells. As the embryo is progressively dividing, the tube is going to push the embryo back slowly into the uterine cavity. During this process of egg maturation, the follicle growth, the ovulation, there is another process happening in the uterus.

So here is the uterus and this is the Uterine bed. So, as the follicles are increasing in size, so it produces a hormone called estrogen which is actually acting on the uterine bed. And the uterine bed is ready, there is an increase in thickness for an embryo to come here. And once the ovulation happens, there is a pregnancy favouring hormone called Progesterone which is increasing in the second half of the menstrual cycle. When this comes, there is a lot of blood flow coming to the uterine bed. So this is the time when the embryo is actually reaching the uterine bed. This process, when the embryo reaches the uterine bed and when the embryo sticks to the uterine bed, this process is called Implantation. Basically what is happening here is the blood collection between the uterus and the embryo. That is how the nutrients from the mother's body are transferred to the embryo which grows further and establishes a pregnancy.

So when the couples conceive, these are the things which have to happen in a natural way for them to conceive. Now what happens when they do not conceive when they try for more than a year after the marriage and still unable to conceive, so we evaluate the problems. Evaluation of problem depends on the steps which I have told.

The battle of supremacy between processed and whole foods has been raging on for quite some time now. Which is better? Which is more nutritional? The debate seems a never ending one. This article will tell you the benefits and drawbacks of both types of foods. It’s for you to decide which one is better for you and which isn't.

Processed Foods
Processed foods are foods which have been altered in some way or the other to make them healthier, to make them taste better or for storage purposes. Almost every day to day food that you consume is processed. Some examples of processed foods include breakfast cereals, cheese, milks, soft drinks, bread etc.

Advantages of processed foods
Here are some advantages that you drive home from processed foods:
1. Processed foods like different types of jams and juices taste very good and last longer, making them a part of every household.
2. Processed fruits, vegetables and herbs, when stored in cans, tend to be free from bacteria and other harmful agents.
3. They are easy to purchase because they are available in all seasons

Disadvantages of processed foods
Processed foods have their own set of drawbacks. Here are some of them:
1. Some processed foods are genetically modified, which may cause a negative impact on your health.
2. Foods tend to lose their nutritional value when processed.
3. Foods that contain added sugar and fat can lead to a lot of health problems like diabetes, heart diseases and other cardiovascular problems.

Natural foods
Natural foods are those which don’t undergo any change in the form of added color, sugar or flavor. Natural foods go straight from vegetable farms and poultry farms to the market where you can purchase them.

Advantages of natural foods
Fresh fruits, vegetables or meat are categorized under natural foods, including a host of other foods as well which are not processed. Here are their advantages:
1. Natural foods are as nature intended them to be. They contain no added ingredients or flavor.
2. A diet comprising of natural foods keeps many diseases at bay.
3. Natural foods are free harmful flavor enhancers like MSG
4. Natural foods are rich in vitamins and minerals which are necessary for a healthy you.

Disadvantages of natural foods

To lose weight, you have to maintain an input-output ratio. Whatever food you eat contains the input calories which gives you the output calories in the form of energy. If your input calories is too much, you are likely to put on weight. The best way to lose weight is to exercise or spend the additional calories accumulated in your body in the form of fat. The other most important thing is to eat wisely. Try to cut down the excessive calories you are eating and make sure whatever you eat is healthy. Some tips to lose weight - 1. Have more fruits and vegetables than junk food and juices. 2. Avoid sugar as much as you can 3. Exercise 3-4 times a week. 4. Have an early dinner, preferably before 8 p. M. 5. Do not starve yourself. Eat small meals 5 times a day. 6. Have barley soup in dinner and avoid rice at night. 7. Drink lots of water daily.

Hello, Get in touch with your knee surgeon and take a green knod from him to resume exercises. Start doing mild exercises and have a customised diet to support your exercise. Go slow with exercise.

Eat healthy, don't skip meals, avoid all forms of sugar (cola/juices/cold drinks/sweets/chocolates etc) have more of fiber rich food (whole grains instead of refined) have 2-3 fruits a day, avoid eating too much at one meal, and stop sitting or lying down immediately after having food, keep atleast 1 hour gap. Above all combine this with exercises atleast 4-5 times a week.

I think I am suffering with social phobia. When I go to office, I am thinking that everybody is watching me and judging me. It hurts me mentally and it disturbs a lot from doing my job at that moment. Hello, welcome to lybrate from your account, that whenever you go out you feel like that people are watching you, judging you. Till this it seems like that you might have the fear of facing the public, as if they would judge you. This can account for social phobia. However for the next part of your complain, that once you come back at home, you still feel afraid of them, keep thinking that they might be commenting behind your back on you. This doesn't happen in social phobia. These kind of symptoms and the symptoms of social phobia can be seen in the beginning or florid psychosis as well. Both of the conditions have different treatments as well. Hence to reach a definite conclusion regarding your diagnosis, a detailed assessment by an expert psychiatrist is required who will further decide the treatment as well for your condition. Aged 21 two years back I love my Junior and having sexual relationship with her. Due to this she got pregnant but her parents not allowed our relationship and done medical abortion to her. After few months she got arranged marriage with another person aged 32, she is only 18 at that time and living in Bangalore. But she is not happy with him because of he is impotent and can't satisfied her. Hence he is allowed some of his friends to sex with her, by using sleeping tablets. Treatment of piles 1. Apply proctosedyl ointment 2-3 times daily before and after passing stool. 2. Sit in a bath tub containing warm water with little salt for some time twice daily (sitz bath). 3. Take two tea spoonful lof softovac/naturolax powder with a glass of warm water/milk in the evening or syrup duphalac 3 tsf (15ml) twice daily if there is constipation. 4. Take fiber containing diet, plenty of green leafy vegetables and fruits. 5. Consult doctor for further advice.

Hi. Treatment for hyperthyroidism was initially start of with medicine like neo marcazole, methimazole or propyl thiouracil .etc .based on patient symptoms the dose of the drug may be adjusted accordingly. If patient still had symptoms even with maximum dose then advice to go surgery like thyrodectomy.

Dear , if you don't have diabetes and till nocturia 3 times a night then one needs to find out the cause urine examination to see if there is infection, usg to find the cause of nocturia like stone, obstruction in flow of urine, uroflowmetry tee your voiding pressures and pattern.
