All About Renal Hypertension

Renal hypertension is a disorder, which is characterized by a rise in the blood pressure that results from kidney disease. The blood flow to the kidney is impaired due to the narrowing of the arteries and this leads to renovascular hypertension. 

Symptoms
The various symptoms of renal hypertension are: 

  1. You may experience symptoms of high blood pressure.
  2. Your kidneys may not function properly due to the impaired supply of blood
  3. It may lead to presence of blood in your urine
  4. You may be affected by pulmonary edema that results in accumulation of fluid in the lungs
  5. It may result in severe headaches and confusion
  6. You may experience blurred vision
  7. You may have nosebleeds
  8. The impaired kidney function may also lead to chronic kidney damage. 

Causes
The various causes of renal hypertension are: 

  1. Accumulation of cholesterol in the body may lead to blockage of the artery due to plaque buildup
  2. Smoking may increase your chances of getting affected by narrow arteries

The narrowing of the arteries causes a reduction in the blood supply to the kidneys. This results in the kidneys to release various hormones that instruct the body to hold on to water and sodium. This causes the fluid to accumulate in the blood vessels, thus resulting in high blood pressure. 
The various risk factors renal hypertension are: 

  1. Excessive alcohol consumption
  2. Substance abuse
  3. Diabetes
  4. High blood pressure
  5. High cholesterol
  6. Aging

Treatment
Medications used to treat high blood pressure are used to treat renal hypertension. It is important that you get your blood pressure levels checked on a regular basis. You need to make certain lifestyle changes such as: 

  1. Exercise on a regular basis to keep your heart and body healthy
  2. Limit consumption of alcohol and reduce smoking
  3. Eat well balanced meals to keep obesity at bay
  4. Keep your mind free of stress
  5. Restrict consumption of salt
  6. Maintain optimal weight levels

You can go for a device called iucd or multiload. This is inserted into the uterus by a trained gynaecologist. Using methods like withdrawal or pull out method or safe days is riskier, and have higher chances of pregnancy.

Causes, symptoms and treatment of Infertility

Hi, I am Dr. Jayanti Kamany. I am an IVF Consultant practicing at Srishti Fertility Centre and Women's Clinic in Mumbai. So, today i'll be talking about Infertility. The problem of infertility is on a steep rise. Some of the causes for the rise in inferility are increased age at the time of marriage for women, postponement of child bearing. Even if a women gets married earlier she postpones her child bearing for various purposes like her career.

Increased stress levels today, increased levels of sexually transmitted diseases and of course Tuberculosis, which is a very common disease in our country. So, who is to be blamed for the infertility. We have found that 3 out of 10 couples are battling with this problem of infertility. 30% of the faults is found in the female, another 30% is found in the male partner, again 30% is found in both partners and about 10-15% cannot be explained because inspite of all the tests being normal, there is no problem which can be found and we call it unexplained infertility.

There are two types of infertility:

1. Primary infertility- Primary infertility is when the couple has never conceived

2. Secondary infertility-  Secondary infertility is when the couple had a child but cannot conceive again, or the couple has conceived, has had an abortion but there is no conception again.

When should a couple consult or take treatment for infertility? 

Usually if the ovum is young, if she is below 28 years she can wait for one year after trying to consult an Infertility Specialist. Even after one year if the couple doesn't conceive inspite of having regular sexual intercourse without having birth control measures they should see an Infertility specialist.

Now there are certain conditions where a couple irrespective of the duration of marriage or age should see a doctor immediately. So, what are those conditions? In the female, if she is having any problems regarding her menstrual cycle, suppose her menstrual cycles are irregular or she is getting very scanty bleeding or scantimensis or she has very severe pain during mensis or otherwise if she had a surgery where her one ovary is removed or a problem of polycystic ovarian disease which is very common today the she needs to see a Gynaec immediately. If she has recurrent abortions, that is, she can conceive but cannot carry the pregnancy to 9 months then definitely there is something wrong with her so she has to consult an Infertility specialist.

What are the causes in a male?

A male, if he has had a surgery in the testes or if he has a history of a disease called mumps in his childhood, or if he has a low sperm count or he cannot feel his testis in his scrotum then he needs to see a Fertility specialist.

Get your Ultrasound done, to confirm for any issues with foetus, passing a white transparent thing could be just accumulation of normal vaginal fluid which you might not have probably washed properly, so suggest you maintain personal hygiene as well, and drink lots of water.

Mam its my first pregnancy although every thing is normal m dilated 3cm as well but I did not feel lightening of the baby that is why m a bit concerned about how to help baby engage as I do not want a c sec. M not sure if the baby has engaged or not but not feeling that much of pressure on blaďder as people say they feel after lightening. M taking regular 20 min walk even squatting 5 times in a day. What else could I do. M feeling my stomach still high.

There is nothing that can help your baby in engaging. Everything is dependent on various circumstances and situations at the time of labour pain that you will deliver normally or by c section. So let everything be decided by god.

As you r perceiving pain like menses then you must be getting menses. Hold on for 4 more days. If menses r missed by 7 days then only you can do urine test to find out pregnancy.

U need to repeat scan after 1 week. If heart is absent after 1 week then pregnancy needs to de terminated. Normally heart appears between 6-7 weeks. So ur usg report may be normal

The pregnancy tests done nowadays are very sensitive. However I suggest just to re confirm that she is not pregnant you could do a blood test for serum b-hcg. If that's also negative then you should seek a gynaecologists advice to see if she has any other reason for not having periods.
