Dear lybrateuser, - In take if red wine in moderate quantity can provide antioxidents in the form of resveterol which can help in prevention of cardiac disease as it prevents deposition of harmful fats in the lining of blood vessels of the body, can help in improving the good cholesterol (HDL) levels in the blood - intake of too much quantity is harmful as can cause acidity, increase in bad cholesterol levels due to alcohol content in it, increase in calories leading to increase in weight.

1. Do steam inhalation 2-3 times daily with karvol plus inhalant capsule 2. Put otrivin nasal drop one thrice daily 3. Take a course of antibiotic after consulting doctor 4. If you are suffering from chronic sinusitis, take plain xray pns/ct scan of pns or nasal endoscopy after consulting ent specialist to confirm diagnosis 5. If you are having recurrent symptoms, you may have to undergo endoscopic sinus surgery.

E. Avoid anxiety/stress/depression/fear/boredom/lack of motivation f) check your hemoglobin, bp, do a thyroid function test to rule out hypothyroidism g) consult physician for advice and treatment 33) headache 1. Take one tablet of crocin advance as and when required after food up to a maximum of 3 tablets daily, 2. Drink plenty of water 3. Take adequate rest 4. Have adequate sound sleep in the night for 7-8 hours if you have recurrent attack of headache1. It could be a tension headache due to anxiety/stress, depression inadequate sleep, low bp/high bp, migraine, prolonged use of cell phone/computer, chronic anaemia, refractive error, chronic sinusitis, organic brain lesion 2. Avoid stress, physical and mental exertion, have adequate sound sleep for 7-8 hours daily in the night 3. Go for regular exercise 4. Practice yoga, meditation and deep breathing exercise to calm your mind, control your emotion and relieve stress 5. Check for refractive error, sinusitis hemoglobin, bp 6. Avoid prolonged use of cell phone/computer 7. Consult neurologist to rule out other causes of headache, if required take a ct scan of head to exclude any brain lesion 8. If it is migraine you have to take migraine prophylactic drug after consulting neurologist.

DIABETIC DIET CHART: Morning: 4-5 Almonds soaked overnight in water or 1-2 walnuts 6:30 a.m.: Tea + 2 mariegold biscuits(only for those who can’t skip their morning tea) 8:00-9:00 a.m. : 2 Roti + subzi + daliya /upma/poha/raw cheese/sprouts + 1 glass skimmed milk 11:00-12:00 a.m.: Fruits (only those permitted), salads, guava, papaya, pomegranate, cucumber, carrot, sprouts etc 1:00-2:00 p.m.: 2 roti +dal + salads+curd/raita/buttermilk 4:00-5:00 p.m : tea + roasted chana/bhelpuri(only if hungry) 7:00 p.m : salads/coconut water/lemon water(without sugar) 8:00-9:00 p.m: 2 roti +dal/khicdi + skimmed milk Try to sleep daily on sametime till 11:00 p.m. SPECIAL PRECAUTIONS:  Avoid potato/sweet potato/arbi banana/cheeku/grapes/mango/litchi papads/ chatni/pickles/fried foods groundnuts/cashew/pista/raisins sweets/honey/cold drinks/jaggery Note: For preparing chapatis, use whole wheat flour mixed with chana flour or besan in 3:1 proportion alongwith small amounts of soyabean

All About Laser Prostate Surgery!

One of the most common problems faced by ageing men is benign prostatic hypertrophy. Prostate is the gland at the base of the urethra near the bladder and when it enlarges it can lead to symptoms, mostly related to urination. 

  1. Frequent urge to pass urine 
  2. Prolonged urination 
  3. Frequent nocturnal visits to the toilet 
  4. Intermittent urination 
  5. Difficulty to start urinating 
  6. Inability to completely empty the bladder 
  7. Urinary tract infections 
  8. Blood in urine 

There are medications available to manage this, but offer only temporary and partial relief. Many men therefore prefer the surgery to manage these bothersome symptoms. However, like any surgery, the risks and benefits need to be considered along with other conditions like age, overall health status, other comorbid conditions, etc. Surgical removal of the enlarged prostate gland is a more definitive approach to manage these symptoms. 

In addition to providing a quick cure, it also is used in the following cases: 

  1. Patients who do not respond to medications 
  2. Refractory urinary retention 
  3. Presence of blood in the urine 
  4. Associated bladder stones or Hernia 
  5. Frequent infections of the urinary tract 
  6. Associated damage to the kidneys 

Procedure of Surgery 
During the procedure, a tube fitted with camera is passed through the tip of the penis into the urethra towards the bladder neck. Once it is in the desired position, laser is passed through it to deliver energy that acts on the prostate to resect or evaporate it. 

There are two methods by which laser acts on the enlarged prostate and making way for free flow of the urine. 

  1. Ablation: Excess prostate tissue is vaporized by the laser by using photosensitive vaporization of the prostate. This is also known as Greenlight laser therapy or KTP laser vaporization. Alternately, Diode or Thulium LASER can be used as the source of laser energy to ablate the prostate tissue. 
  2. Enucleation: Entire adenomatous prostate tissue is cut and teased out into the bladder by using Holmium laser. Morcellator is used to grind this enucleated prostate into smaller pieces to enable easy retrieval. 

More men now opt for laser prostate removal as it has the following advantages: 

  1. Reduced risk of bleeding: This becomes essentially important in patients who are on blood thinners. 
  2. Minimal hospitalization: This can be done with minimal one or two days stay at the hospital 
  3. Immediate symptom relief: As compared to medications, the relief is felt almost immediately after the surgery 
  4. Minimal catheter: With laser surgery, a catheter may be required for less than 24-48 hours unlike in open surgical cases. 

As noted above, as with any surgery, once enlarged prostate symptoms set in, have a detailed discussion with your doctor to identify if you are a suitable candidate for laser surgery.
