Age of Hormones in Women and Weight loss
In a study in obesity reviews unsw's st. George clinical school points out the paradox of weight management in women. From an energy balance point of view there is no explanation why women should be better than men particularly since men consume more calories proportionately. In fact women burn off more fat than men during exercise. Suggesting women are more efficient fat storers at other times
Oestrogen reduces a women's ability to burn energy after eating resulting in more fat being stored around the body

BODY TYPES & NUTRITION

BODY TYPES & NUTRITION

Not everybody’s created equal. Before you start your nutrition regimen, its always a good idea to figure out your Body Type. This will help you tailor your diet better and set realistic and attainable goals.

There are 3 basic human body types:

Ectomorph: They are distinguished by a lack of much fat or muscle tissue.

Mesomorph : This is marked by a well developed musculature.

Endomorph :  This is characterized by a preponderance of body fat.

This is how the 3 body types look like:

Everybody is different since we all have different genetic and cultural traits. If everyone ate the same things and started exercising similarly, we would not look the same. This is because each person’s genetic inheritance influences their bone structure, body size, shape, and weight differently.

Thus, every body type should eat differently as per what appeals to them. This will not only maintain one’s healthy weight but also prevent any kind of eating disorders.

ECTOMORPHS:

Ectomorphs are individuals characterized by smaller bone structures and thinner limbs. They are characterized by:

Narrow hips and clavicals
Small joints (wrists and ankles)
A thin build
Stringy muscle bellies
Long limbs
Proper nutrition is very important, especially for those who consider themselves as “Ectomorphs”. It typically means that they have a faster metabolism , a tough time gaining body weight and higher overall caloric needs.

An ectomorph must typically start his or her diet plan by following the main principles for big gains in muscle size and strength.

As an Ectomorph, your ideal nutrition consists of about 55% of complex carbohydrates , 30% good quality proteins and 15% healthy fats.

Ectomorphs’ Ideal Body Type Diet

Avoid fatty foods, especially animal fat, cold cuts, mayonnaise, full-fat dairy, rich desserts, packaged foods, chips.

As an Ectomorph, the ideal nutrition consists of about 55% of complex carbohydrates (whole grains, rice, nuts all vegetables), approx. 30% quality proteins (whey, chicken, eggs), and up to 15% healthy fats cold-pressed virgin olive oil, flaxseed oil and good omega 3 sources like fish and sea food.

MESOMORPHS

Mesomorphs have a medium sized bone structure and athletic body, and if they’re active, they usually have a considerable amount of lean mass. They are characterized as follows:

Wide clavicles
A narrow waist
Thinner joints
Long and round muscle bellies
As a mesomorph, you are likely to put on muscle easily and also easy to gain and lose body weight. However,  mesomorphs yet need a targeted diet plan that helps them avoid fat gain and improve overall health.

BALANCE  is very important for a mesomorph, who falls between an ectomorph and an endomorph. A typical diet for mesomorphs should comprise of 45% complex carbohydrates, 35% good quality proteins and up to 20% healthy fats.

Mesomorphs’ Ideal Body Type Diet

Avoid fatty foods (full-fat dairy, fatty meats, cold cuts, mayonnaise, very rich desserts,) and minimize as much as possible refined carbohydrates (sugars and white flour products), as they cause rapid weight gain triggering a fat-storage metabolism.

As a Mesomorph, you should eat 3 smaller meals favoring foods high in quality protein (whey, chicken, eggs) and complex carbohydrates (leafy greens and non starchy vegetables).

Approx. 45% complex carbohydrates, 35% quality proteins and up to 20% healthy fats; use cold-pressed virgin olive oil and flaxseed oil in salads and get used to cooking with rice bran oil.

Include fish and seafood, as they are good omega 3 sources-omega 3 and weight loss connection is well-known.

In between meals get a healthy snack: fruits (except the sweet ones, like bananas, pineapple, grapes, mango, watermelon), raw almonds, plain yogurt or various smoothies with the right foods for you, as above.

ENDOMORPHS

Endomorphs have a larger bone structure with higher amounts of total body mass and fat mass. They tend to be naturally less active and thus have more tendency to store energy as fat. Thus, they have a particularly easy time gaining weight but hard to shed kilos. The main characteristics of a Endomorph is:

They look blocky
Wider and thicker joints
Wider hips than the clavicles
Shorter limbs
Out of all the three body types, Endomorphs have the greatest tendency towards roundness. Being an endomorph, obesity is very common in males in females.

“Apple shaped obesity” is more commonly seen in males where there is excess fat in and around the belly, while “Pear shaped obesity” is commonly seen in females with excess fat on the hips and buttocks.

Endomorphs’ Ideal Body Type Diet

As an Endomorph, your nutrition for losing weight should be roughly up to 30% complex carbohydrates (starchy vegetables, brown rice, whole grains), about 45% quality protein (chicken breast, whey protein) and about 25% healthy fats: omega 3 sources (fish, seafood) and ricebran oil, which helps to increase your sluggish metabolism. Also, to keep your metabolic rate up, divide your daily meals into smaller portions so that you have something to eat handy every 2-3 hours.

Avoid as much as you can all types of sugar (including all fruits except berries) and simple carbohydrates like white flour products and pasta, white rice, potatoes, because they cause a rapid rise of your blood sugar level, which, in turn triggers the release of insulin, the fat-storage hormone. Never end your meals feeling completely full.

Endomorphs typically do best on a higher fat and protein intake with carbohydrate intake being controlled and properly timed (e.g., after exercise). More fat and protein, less carbohydrate.

A nutrient distribution for this body type might be around 25% carbs, 35% protein, and 40% fat.

KNOW YOUR BODY TYPE – YOUR DAILY NEEDS ARE AS IMPORTANT AS YOU ARE !!

The weight should be around 5.6 kg. But 5 kg is not bad. Do not worry mothers milk is never insufficient. Peoples wrong advice makes it insufficient and that too in mothers mind. If the child gains atleast 15gms/day than do not worry. Also if the child passes 6-7 times urine and goes back to sleep after feeding than he is fine and geting enough milk. Always feed and empty one breast fully before going to another breast as the breast foremilk is rich in water and the hind milk is rich in protein and fat. Happy Feeding.

I have tried 7 times iui 2 times ivf still my wife has not conceived I am now 40 and my wife 35 her amh now is 2.3 and my sperm count is 20 million I am thinking of now going for once again for ivf which is best centre what precautions should we both take her tubes are patent and no endometriosis or cist also let me know how shall I improve my sperm count

20 million count is normal, provided the morphology and motility are within normal limits. Again, for IUI and IVF, even lower sperm counts are sufficient. SO, you need not worry about your count. Now, AMH is marginal. SO, I think you should consider IVF with ovum donation as after 36 years, the success rate of IVF is lower. Again, she is not having endometriosis, tubal block etc. So, the chance of having pregnancy is good, if ovum quality is normal. All the best.

Blood Clotting During Pregnancy - Why is it Dangerous?

DNB (Obstetrics and Gynecology), MBBS
Gynaecologist, Mumbai
Blood Clotting During Pregnancy - Why is it Dangerous?

Clotting of blood is important for us in case of an injury or wound. It prevents the blood to ooze out of the body from any opening. Our blood releases cells called blood platelets, which initiate the process of clotting. However, the clotting of blood inside the veins is not a good thing to happen. This condition is called thrombosis. Moreover, clotting during pregnancy is a serious condition that poses threat to the health of both the mother and the child. In case of pregnant women, clotting of blood starts in the deep veins. These veins are usually the ones in the legs or the pelvic region. This condition is termed as deep vein thrombosis (DVT).

Why there is a clot during pregnancy?
Blood can clot during pregnancy due to multiple reasons. Clotting occurs to prevent too much loss of blood at the time of labour.

How will you know that it’s DVT?
DVT comes along with certain symptoms. Some of these have been listed below:

  1. The pregnant woman may notice tenderness or swelling in one of her legs. She might feel the pain in one of the legs.
  2. The skin colour of the woman starts reflecting changes. The skin might turn a little reddish.
  3. The patient’s skin will be warmer at the site of clot.
  4. The veins in the affected region might look dilated or larger in size.

What are the dangers linked to this clotting?
Whatever may be the reason, clotting during pregnancy is always attached to some dangerous consequences. If the blood clot is left unattended and untreated for long, it may have some serious consequence. The clot may start shifting from your legs to one or both of your lungs. This condition is called pulmonary embolism (PE). It is acute condition that can cause death of the patient as well. The symptoms of PE are as follows:

  1. The patient suffers from sudden breathlessness.
  2. The patient may feel that there is some tightness around the chest.
  3. The patient may find blood in her cough.
  4. The patient may show signs of collapse.

Clotting of blood can also prove detrimental to the baby’s health and growth. The blood is supplied through placenta to the baby and a clot in the mother’s vein can decrease the amount of blood supplied to the baby. Sometimes, the blood supply to placenta might be entirely snapped, leading to the death of the child inside the womb. However, blood clotting at the time of pregnancy happens to only a few women. It has been found that one or two in thousand die due to clotting of blood during pregnancy. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Headache in pregnancy? I'd rather not take medication.

There's much you can do to prevent or relieve headaches during pregnancy without taking medication. Start with simple prevention tips:

Avoid headache triggers. Keep track of your meals, activities and headaches for several days to help pinpoint your headache triggers — then do your best to avoid your triggers.
Include physical activity in your daily routine.Try a daily walk or other moderate aerobic exercise.
Manage stress. Find healthy ways to cope with the stressors in your life, such as delegating tasks on your to-do list and spending time with people who lift your spirits.
Practice relaxation techniques. Try calming activities such as deep breathing, yoga and visualization.
Eat smaller, more-frequent meals throughout the day. Regular meals will keep your blood sugar on an even keel, which can help prevent headaches.
Drink plenty of fluids. Staying hydrated can keep you feeling your best.
Follow a regular sleep schedule. Fatigue and lack of sleep can contribute to headaches during pregnancy. Go to bed and wake up at about the same time every day, even on weekends.
Consider biofeedback. With this mind-body technique, you learn to control certain bodily functions — such as muscle tension, heart rate and blood pressure — to prevent headaches or reduce headache pain. If you'd like to try biofeedback to treat headaches during pregnancy, ask your health care provider for a referral to a biofeedback therapist.

When a headache strikes:

Rest. Lie down in a dark, quiet room with your eyes closed.
Use a compress. Apply a warm compress (such as a hot towel) to your face, eyes and temples — or try a cold compress on the back of your neck.
Try massage. Ask someone to massage your shoulders and neck to relieve tension. You might rub your temples, too.

If these steps don't help, check with your health care provider about other treatment options for headaches during pregnancy.

Most pregnant women can safely take acetaminophen (Tylenol, others) to treat occasional headaches. Your health care provider might recommend other medications as well. As with any medication, though, make sure you have the OK from your health care provider first.

How to Control Knee Joint Pain?

Commonwealth Fellow Joint Replacement, DNB - Orthopedics, MS - Orthopaedics, MBBS
Orthopedist, Delhi
How to Control Knee Joint Pain?

An elementary knowledge of biology tells you how important joints are for the overall functionality of a human body. If not for biology experience teaches us the inevitable importance of joints. The link between a tooth and the jawbone is also a joint. Without joints the human body would have been in bits and pieces.

Having known that, you should also try to remember or understand how disabling joint pains can be. They hardly let you move a limb and can also cripple you for life. Knee joint pain can hinder a person's free movement. Knees bear the entire load of your body; they support your frame. Knee joint pain should therefore be a cause for alarm.

Going by the following ways can relieve you of knee joint pain:

  1. Maintain a healthy weight: Carrying extra pounds can exert additional pressure on your joints and contribute to knee pain. If you're overweight, losing as little as 5 percent of your body weight can help relieve the stress on your knees. Talk to your doctor about developing a healthy eating plan and exercise program to help you lose weight sensibly.
  2. Find a low-impact exercise you enjoy: Talk to your doctor before beginning an exercise program. Good choices for people with knee pain include walking and swimming. Be sure to warm up before and cool down after exercising. Avoid hilly terrain and high-impact activities like running and jumping, as these can worsen knee pain.
  3. Give physical therapy a try: Physical and occupational therapy often are helpful for people with knee pain. A physical therapist can help design an exercise program that fits your individual ability level and teach you proper techniques to spare your joints. Occupational therapy can teach you how to reduce strain on your knees in your daily activities.
  4. Get enough rest and relaxation: Sure, physical activity is important, but rest and relaxation can go a long way to promote good health - and reduce pain. Achieve a healthy balance in your life by learning stress-relief techniques like deep breathing and meditation.
  5. Make sure you're getting enough sleep: No question-arthritis pain can interfere with a good night's sleep. However, proper sleep is necessary for overall health, so if you are having trouble sleeping, talk to your doctor. Proper pain management can help break the cycle to help you slumber soundly.
  6. Use Ice and/or Heat: For many people with arthritis pain, ice can help relieve pain and swelling and heat can help ease stiffness. Ask your doctor about how to safely use an ice pack and/or a warm towel or heating pad. A hot shower in the morning or warm bath before bed at night also may be helpful.
  7. Apply a topical pain reliever: A number of over-the-counter and prescription creams, gels, sprays and patches are available to help relieve arthritis pain. These pain relievers contain ingredients like capsaicin, salicylates, menthol, or a combination of medicines. Ask your doctor if one of these products might be right for you.
  8. Consider Injections: Persistent arthritis pain may respond to treatment with injections. Available options include corticosteroids, which can be used up to 2 or 3 times a year to relieve severe pain, and hyaluronic acid, which can help replenish lubricating substances in the knee. Talk to your doctor about the benefits and risks of injectable treatments for knee pain.
  9. Talk to Your Doctor about Surgery: If you have severe osteoarthritis symptoms that interfere with daily life and do not respond to conservative treatment, surgery may be necessary. Ask your doctor about arthroscopic procedures or total knee replacement, may be necessary.
