Sleep Apnea and Its Link to Heart Failure

DM - Cardiology, MD-Internal Medicine , MBBS
Cardiologist, Delhi
Sleep Apnea and Its Link to Heart Failure

We're all guilty of making fun of snorers, but sometimes this snoring can be a symptom of sleep apnea. Sleep apnea is a condition where breathing is not continuous and may start and stop many times while asleep. This affects the quality of your sleep and in turn can affect many aspects of your health including your cardiovascular health. Here's how:

High Blood Pressure: When a sleep apnea patient's breathing stops, the oxygen levels in the blood also suddenly fall. This can increase your blood pressure. High blood pressure means that the heart muscles need to work harder to pump blood through the body.

Cardiomyopathy: As a result of high blood pressure, the heart walls may become thicker and the heart muscles become stiffer. This is known as Cardiomyopathy. As this condition worsens, the heart becomes weaker and is unable to maintain a regular rhythm. This can eventually lead to heart failure.

Arrhythmia: An irregular heartbeat is also known as Arrhythmia. This may also be related to the changes in the heart's structure that follow the drop in blood oxygen levels. In many cases, Arrhythmia has no visible symptoms and can often go undiagnosed. This can result in the formation of blood clots in the atria which can lead to a stroke.

Sleep apnea is easy to diagnose. If the doctor feels that you show symptoms of sleep apnea, you may be asked to stay on the hospital overnight and undergo a sleep evaluation. This tests a variety of body functions including brain activity, eye movements, heart rate, breathing patterns and blood oxygen levels.

Treatment for sleep apnea depends on the causes for this condition. If you are overweight, regular exercise and a change in diet can help you lose the excess weight and cure sleep apnea. Similarly, if your sleep apnea is triggered by an allergy, treating the allergy can help cure the sleep apnea. Other forms of treatment for sleep apnea may include:

Continuous Positive Airway Pressure (CPAP): This is a machine which pumps air into the body through a nasal mask. By keeping the pressure in the machine higher than normal air pressure the upper airway passages are kept open and hence the quality of your sleep is improved. An auto CPAP machine can modulate the air pressure such that it is higher when you inhale and lower when you exhale.;

Oral devices: Your doctor may suggest oral devices that allow you to keep your mouth open while you sleep. This is easier to use than CPAP machine but less effective. Surgery is the last resort when it comes to treatment for sleep apnea.

Brain Cancer

IFAANS, DNB (Neurosurgery), MBBS
Neurosurgeon, Noida
Brain Cancer

Brain cancer is a disease of the brain in which cancer cells (malignant cells) arise in the brain tissue. Cancer cells grow to form a mass of cancer tissue (tumor) that interferes with brain functions such as muscle control, sensation, memory, and other normal body functions. Tumors composed of cancer cells are called malignant tumors, and those composed of mainly non-cancerous cells are called benign tumors. Cancer cells that develop from brain tissue are called primary brain tumors while tumors that spread from other body sites to the brain are termed metastatic or secondary brain tumors.

Prologue:

  1. Brain cancer is actually the abnormal growth and uncontrolled growth of the cancer cells in the brain that forms a tumor in the brain.Tumours can be either benign or malignant.

  2. Benign brain tumours are abnormal collections of cells that reproduce slowly and usually remain separate from the surrounding normal brain.

  3. Malignant tumours reproduce and grow quickly. Their borders are hard to distinguish from the normal brain around them.

Symptoms:

There are few early symptoms of brain cancer, but as the tumour grows within the confines of the skull, it causes increased intracranial pressure and exerts pressure on the brain, causing signs to develop.

Brain cancer symptoms and signs are varied and depend on the area of the brain involved, but can include:

  1. Headaches.

  2. Seizures.

  3. Nausea and vomiting, which, CANSA reports, may be worse in the morning or after a sudden position change.

  4. Difficulty walking or clumsiness.

  5. Vision changes.

  6. Changes in alertness.

  7. Behavioural impairment.

Causes:

Brain cancer occurs when there is an uncontrolled growth of cancer cells in the brain that form a malignant brain tumor. The underlying cause of primary brain cancer, cancer that begins in the brain, is not known. Secondary brain cancer is caused by a cancer of another organ in the body, such as the breast, prostate, kidney, skin, or bone, that has spread to the brain.

What are the risk factors for brain cancer?

  1. Certain inherited conditions, including neurofibromatosis, Von Hippel-Lindau syndrome, Li-Fraumeni syndrome, and Turcot syndrome

  2. Personal history of cancer or family history of brain cancer

  3. Impaired immune system

  4. Radiation therapy of the head

Treatments Options:

Treatment

Surgery is the main form of treatment for brain tumors that lie within the membranes covering the brain or in parts of the brain that can be removed without damaging critical neurological functions. The goal is to remove the entire tumor, whenever possible, as a tumor may recur if any tumor cells are left behind. Radiation therapy and chemotherapy are generally used as secondary treatment for tumors that cannot be cured through surgery alone.

Stereotactic radiosurgery

Stereotactic radiosurgery is a treatment option that delivers a high concentration of radiation directly to the tumor in order to stop its growth, while delivering only a minimal dose of radiation to the surrounding tissue. Unlike conventional surgery, stereotactic radiosurgery does not require making an incision to remove the tumor. It can be especially effective in patients with many small metastatic brain tumors. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

COMT, MPTh/MPT
Physiotherapist, Surat
I can understand how disturbing an interuppted, inappropriate or disturbed sleep can affect your day to day life. In india it has been found 55% of patients are suffering from insomnia just because of stress, distractions and anxiety. So the best way to get rid from insomnia is to reduce the stress level. A physiotherapist can help you to give you the ways to relax your mind and give you comfort and stress full sleep. Consult some neurologist/neuropyschiatric and start taking medicine if it is due to some neurological discorder. And consult nearby physio who can make a perfect routine and daily life styl chart to make you relax so that you get an appropriate sleep. Please follow some points that will help you in such condition along with medical treatment: 1) use of ice packs over your eyes or frontal area for some time to give relaxation. 2) massage your head especially forehead area using levander oil or clove oil as it removes toxins from body 3) regular intake of healthy dietry food rich in proteins and vitamins. 4) avoid fasting - eat breakfast, lunch and dinner at fixed timings 5) make yourself busy by fulfilling your hobby as it will give you freshness and your mind will be diverted from normal life which help in pain reduction. 6) before sleeping time do some relaxation exercises. - sit on chair with both the hands placed on the pillow placed over thighs. - deep inhale from your nose, mouth should be closed. - hold your breath for the count of 5. - exhale completely from mouth making a loud sound. Continue to do this exercise for 10 times. Before going to bed make a habit of doing this daily. Modified technique of same exercise is while inhaling extend your arms, hold for 5 count and then exhale making your arms back in the rest position. 7) room modification while sleep - make sure room should be dark with no noise. Avoid sleeping immediately after dinner or food. Take a walk for 15 min and then come back, bed should be not too soft not too hard. 8) it is recommended to drink at least 8 to 10 glasses of water per day. It is also a very good habit to start and finish your day by drinking a good glass of purifying water. 9) exerices like aerobics (running) can be done to make your muscles active. Many other ways are there in order to maintain your life stress free and active so that you can continue to sleep on time and it does affect your life. For more help or instructions please consult. Good luck and get well soon.

I am suffering from bells palsy. I mean my half face is lighter and upside as compared to other side. Can it be cured...
Mr. Lybrate-user, bells palsy is usually a self limiting condition, if no specific cause can be attributed. There are few medications which can hasten the recovery. You also need to go for rehabilitation procedures like galvanic stimulation and facial exercises. The most important thing for you now is to protect your unclosable eye from exposure and dryness, to prevent loss of vision.
