Obesity - Possible Causes Behind it

Msc. - Food Science
Dietitian/Nutritionist, Bangalore
Obesity - Possible Causes Behind it

According to WHO a third of the world population is obese, obesity is caused by overeating, lack of physical activity, age factor, hereditary and sometimes malfunctioning of the endocrine glands causing excessive fat accumulation. Food is meant to generate energy in the body so as to enable each and every organ to discharge its individual functions. To avoid overeating one has be careful about portion control and eat at a fixed time. Each meal should have a combination of carbohydrate 50 to 60 % of total calories + protein 35 to 40% of total calories and fat 20 to 25% of total calories. Dietary fibre of 25 to 30 grams a day can obtained from whole grain, fruits and vegetables should also be taken in sufficient quantities as it imparts a sense of satiation and reduces the food intake.

Indian diets are usually high in carbohydrates leading to higher incidence of diabetes. This can be countered by increasing proteins in each meal along with fibre, which gives a controlled balance to sugar levels in the body. Use unsaturated vegetable oils like olive, rice bran, soyabean and sunflower oil to maintain and reduce cholesterol and maintain heart health along with Omega-3 from flax seed, nuts (almonds and walnuts) or fish oil supplements, including vegetable juices in your daily diet improves the antioxidant level and fibre in the body and helps in weight loss.

How to make meal plans customized to your BMR

Step 1 - Calculate your BMR

Women's BMR Men's BMR
655 66
+ (9.6* of your weight in kilos) + (13.7* your weight in kg)
+ (1.8*your height in cm) + (5*your height in cm)
- (4.7*your age in years) - (6.8*your age in years)
= ___________ Calories per day = ___________ Calories per day

Step 2 - After calculating your BMR ,and to start weight loss cut 200 kcal from your BMR.

For example if your BMR is 1600 kcal choose a meal plan that is 200 kcal lower I.e is from 1200 to 1400 calorie meal plan.

The main meals should be three meals like your breakfast ,lunch and dinner and the in between meals should be like fruits ,veg juice ,dry fruits, dry bhel , roasted chana, a slice of cheese ,which are not very dense in calories and high in nutrients.

STEP 3 - Eat every two hours with three main meals and three small meals.

STEP 4 - Drink plenty of water through the day at least 8 to 10 glasses.

Exercise - Lack of physical activity means there is no energy output and this results in weight gain over the period of time because as we age our BMR reduces (basal metetabolic rate : that is total number of calories your body requires for normal body functions excluding any activity factor like exercise, BMR is the energy used for metabolic process of maintaining life itself and usually accounts for two thirds of total daily expenditure ). To maintain a healthy weight increasing the BMR is essential and this means increasing the muscle mass over the fat mass as 1 pound of muscle burns 6 calories at rest and 1 pound of fat just 2 calories. Increasing muscle mass can happen only through physical exercise and adequate protein in the diet thereby, increasing fibre and reducing fat in the diet.

Advantages of Physical Activity

  1. Helps to keep digestive system in optimum condition
  2. Improves cardiac rhythm and output
  3. Maintains flexibility of body
  4. Energies blood vessels and does not allow cholesterol deposits and improved heart health as it does not allow fat accumulation
  5. Improves peristaltic action of intestine and rectum and removes constipation
  6. Helps in improving insulin resistance in the body especially in PCOD and PCOS (polycystic ovarian syndrome)
  7. Maintains the sugar levels in diabetic patients

Other Causes of Obesity

  1. Age factor: Obesity is generally prevalent in the middle age as the BMR reduces as a person ages especially if he or she does not exercise, but weight gain can be gender specific as women have more fat mass and the weight gain during pregnancy is common due to increase in fat in the diet, but in recent years there has been a rapid rise in obesity in children and teens due to over indulgence in junk and fast foods, which have high amounts of refined carbohydrates and hydrogenated fats. Heredity also plays an important role in fat accumulation. Children of obese parents do have high chances inheriting obesity from their parents as genetic factor could be a contributing factor rather than excessive food. In such cases physical activity plays a very important role for weight loss
  2. Untreated Hypothyroidism: Hypothyroid causes a drop in the BMR and increase in fat percentage and if untreated can cause weight gain, but once on medication they have improved metabolism.
  3. Drugs: Use of oral contraceptives, steroids, insulin and phenothiazines is generally followed by weight gain to stimulation of appetite.
  4. Alcohol: Alcohol is just empty calories and 1ml of alcohol gives 7 calories and this causes weight gain along with the fat laden food eaten while having alcohol, overeating is usually a consequence of drinking leading to weight gain and drinking coupled with smoking increases the lipid levels in the body affecting the heart health
  5. Fasting and Yo Yo Dieting: The process of losing weight and gaining weight several times throughout a life time is often characterised by increased fatness with each cycle (Krause food and nutrition therapy). Fasting is dropping calories to less than 600 to 800 over a period of time, which causes starvation and breakdown of muscle mass and lowering of BMR and once one gets back to normal diet there is sudden surge in weight which then gets difficult to rid off.
  6. Water: Incredible as it may seem water is a very important catalyst in weight loss, water suppress the appetite naturally and helps metabolise stored fat. Drinking water is best for fluid retention caused due to excessive intake of salt, the more water you take, more it dilutes the excess salt and reduces the fluid retention. Eight to twelve glasses of water in a day is the optimal intake.
  7. Stress: Stress is a another precipitating factor for weight gain which causes damage to the heart. Excessive stress causes an increase in cortisol and thereby, increase in fat percentage in the body and weight gain. An emotionally charged and highly strung nervous system can adversely affect the coronary arteries and deteriorate the heart. If you wish to discuss about any specific problem, you can consult a doctor.

MBBS, MS - General Surgery, DNB
Urologist, Bangalore
If it is slight pain during urination, it can be cystitis or urethritis (that is, inflammation of the bladder or urethra). If is persistent or severe you need to investigate with a urine analysis and urine culture. If it is reducing then you need not worry. If it happens repeatedly you may again investigate.

Pregnancy: The Second Trimester

MBBS, Diploma in Child Health
Pediatrician, Hyderabad
Pregnancy: The Second Trimester

The Second Trimester: What You Need to Know

  • During your second trimester prenatal visits, your health care provider will continue to check on your and your baby’s health, including monitoring the fetal heartbeat.

  • The second trimester is the most physically enjoyable for most women. Morning sickness usually lessens by this time, and the extreme tiredness and breast tenderness usually ease up.

  • Your fetus has now developed all its organs and systems and will now begin to grow in length and weight.

  • You may be able to feel the movement of the fetus for the first time at around 20 weeks. This phenomenon is called quickening.

  • A fetus born at the end of 24 weeks may survive in a neonatal intensive care unit.

Prenatal Visits During the Second Trimester

During your second and third trimester prenatal visits, your health care provider may check the following, depending on your current medical condition and the health of the fetus:

  • Any current symptoms or discomforts

  • Your weight

  • Your blood pressure

  • Urine test. This is done to find albumin, a protein that may indicate pre-eclampsia or toxemia, and glucose (which may indicate hyperglycemia).

  • Growth, size and development of the fetus

  • Size of the uterus. After approximately 12 weeks of gestation, the uterus can be felt through the abdominal wall.

  • Height of the fundus (top of the uterus), starting at 20 weeks of gestation

  • Fetal heartbeat

The Second Trimester: What to Expect

The second trimester marks a turning point for the mother and fetus. You will usually begin to feel better and start showing the pregnancy more. Your fetus has now developed all its organs and systems and will now begin growing in length and weight.

During the second trimester, the umbilical cord continues to thicken as it carries nourishment to the fetus. However, harmful substances also pass through the umbilical cord to the fetus, so care should be taken to avoid alcohol, tobacco and other known hazards.

During the second trimester, both your body and the fetus continue to grow.

The Second Trimester: Changes to Your Body

The second trimester is the most physically enjoyable for most women. Morning sickness usually lessens by this time, and the extreme tiredness and breast tenderness usually ease up. These changes can be attributed to a decrease in levels of human chorionic gonadotropin hormone and an adjustment to the levels of estrogen and progesterone hormones.

The following is a list of changes and symptoms that may happen during the second trimester:

  • Appetite may increase.

  • You may be able to feel the movement of the fetus for the first time around 20 weeks. This phenomenon is called quickening.

  • The uterus grows to the height of the bellybutton around 20 weeks, making the pregnancy visible.

  • The skin on the belly may itch as it grows, and there may be pain down the sides of the body as the uterus stretches. The lower stomach may ache as ligaments stretch to support the uterus.

  • The need to urinate often may decrease as the uterus grows out of the pelvic cavity, relieving pressure on the bladder.

  • Your nose may become congested, and you may experience nosebleeds. This is due to the increase in hormones (estrogen and progesterone) and blood flow that affect the mucous membranes and blood vessels in the nose.

  • Your gums become spongier and may bleed easily. This is due to the increase in hormones (estrogen and progesterone) that affect the mucous membranes in the mouth.

  • Varicose veins and hemorrhoids may appear.

  • You may have a white-colored vaginal discharge called leukorrhea. (A colored or bloody discharge may signal possible complications and should be examined immediately.)

  • The increasing weight gain may cause backaches.

  • Skin pigmentation may change on the face or abdomen due to the pregnancy hormones.

  • Heart burn, indigestion and constipation may continue.

The Second Trimester: Fetal Development

Fetal growth from weeks eight through 40 is depicted visually in this chart.

Now that all the major organs and systems have formed in the fetus, the following six months will be spent growing. The weight of your fetus will multiply more than seven times over the next few months, as the fetus becomes a baby that can survive outside of the uterus.

By the end of the second trimester, your fetus will be about 13 to 16 inches long and weigh about 2 to 3 pounds. Fetal development during the second trimester includes the following:

  • The fetus kicks, moves and can turn from side to side.

  • The eyes have been gradually moving to the front of the face, and the ears have moved from the neck to the sides of the head. The fetus can hear your voice.

  • A creamy white substance (called vernix caseosa, or simply vernix) begins to appear on the fetus and helps to protect the thin fetal skin. Vernix is gradually absorbed by the skin, but some may be seen on babies even after birth.

  • The fetus is developing reflexes, like swallowing and sucking.

  • The fetus can respond to certain stimuli.

  • The placenta is fully developed.

  • The brain will undergo its most important period of growth from the fifth month on.

  • Fingernails have grown on the tips of the fingers and toes, and the fingers and toes are fully separated.

  • The fetus goes through cycles of sleep and wakefulness.

  • Skin is wrinkly and red, covered with soft, downy hair (called lanugo).

  • Hair is growing on the head of the fetus.

  • Fat begins to accumulate in the fetus.

  • Eyelids are beginning to open, and the eyebrows and eyelashes are visible.

  • Fingerprints and toeprints have formed.

  • Rapid growth is continuing in fetal size and weight.

  • The 20th week marks the halfway point of the pregnancy.

A fetus born at the end of 24 weeks may survive in a neonatal intensive care unit.

Folic acid does not induce sleep, continue taking it, pregnancy can be tested before periods and after conception through a blood test called hcg test, not very expensive. Heaviness in stomach can be due to gastric reasons but do not take medicines before pregnancy is confirmed. Eat a healthy, nourishing digestible food like toned milk, curd & vegetables and fruits to suit your constitution.

BAMS
Sexologist, Nashik
You have taken the medicines but the Infection is not fully cured. White discharge is due to Fungal infection. Take Anti fungal medicines. You should keep Hygeine of the private parts. Allopathy medicines have temporary effects. So try Ayurvedic medicines for your problem. You should also follow the diet schedule prescribed by us. Both medicines and Diet will fully cure your problem. Consult if require medication

Significance of Sexual Health

MS sexuality, M.Phil Clinical Psychology, PhD (behaviour modification), Certified in Treatment of Resistant Depression
Sexologist, Hyderabad
Significance of Sexual Health

Does a struggle with mental illness mean losing one’s cherished sexuality? No, it certainly doesn’t. But many people struggle far more than necessary because they don’t have the information and support they need to address the challenges that can arise around sex and mental well-being.

Sex is a matter for mind

We often think of sex as something our bodies are doing, but a lot of our sex life takes place in our brains. It’s important to realize that, for all genders, our thoughts and feelings play a vital role in getting us turned on and keeping us that way.

Anxiety or depression can strongly affect arousal and can definitely ruin the mood sometimes. Anxiety and other related mental health struggles can make it hard to be relaxed enough to have or enjoy sex, overshadowing it with a host of worries or intrusive distractions. When we are very unwell and struggling just to function, sex is rarely at the top of our mind.

The struggle with mental illness in a variety of forms can hurt a person’s self-esteem and make them feel unworthy of sexual attention. For example, a person may have an unrealistic view of their own body and may actively seek to deny or discipline the body as a way of coping. In these cases, it’s important to be critical of the beauty norms we are shown by the media, step away from the practice of measuring or defining ourselves, and to seek to rediscover our love and appreciation for our bodies and our sexual selves.

Substance use may put limitations or restrictions upon one’s sexual interest. Some drugs can affect your brain in ways that make you less able to feel pleasure from sex for periods of time after their use.

Substance use can be a problem when it leads to sexual behaviours one may not feel proud of. Under the influence of drugs, you may do things that you regret, such as having sex with someone you wouldn’t have while sober, or doing things you normally might be uncomfortable with; such as being filmed or photographed during sex, or having sex in public spaces. These personal-boundary transgressions can lead to shame and loss of self-esteem and cause conflict in relationships.

Additionally, addiction or mental health problems like mania may be associated with intentionally seeking risky situations such as having unprotected sex with strangers or seeking ever escalating levels of violence, humiliation, and bodily harm (both in real life and in the pornography one is consuming). This can impact a person’s ability to find interest in having sex with their steady partner, because the sought-after thrill or risk is no longer present.

When you become an ally to your partner and deepen your communication, your sex life is certain to benefit. Great sex is often based on great connection, and by working on your communication you are nurturing that connection.

We are all entitled to knowledge about our sexuality and that good scientific knowledge about sex is necessary for us to be able to make informed sexual choices. By educating ourselves, whether we are young or old, we can make sex better for everyone.

We all have a right to enjoy and appreciate our sexuality. Do your best to not let mental illness steal that away from you and the ones you love!

Due to sex, my wife feels her breast became large and heavy, as compared to her normal size. What should she do to get her normal size back?

DHMS (Diploma in Homeopathic Medicine and Surgery)
Homeopath, Ludhiana
The breat size in females changess due to hormonal changes that take place after marriage and Sex and Mental health plays an important part in these hormonal changes. There is no any medicine to reduce the breast size but yess, I will advise you below given exercise which will help in reducing breast sizeBelow I have listed down some best exercises that will help you Reduce breast eventually Pushups Dips Barbell Bench Press Pec Deck Butterflys Using a Pec Deck machine, seat yourself in it accordingly. Perform the exercise by squeezing your forearms and elbows in together so that you virtually touch them together at the peak of the movement. Be sure to really squeeze your pecs at the peak of this movement for a one-count. Return to the start position and repeat. Decline Dumbbell Flyes Position yourself on a decline bench, flat on your back. Have your spotter hand you each dumbbell. When you begin this movement, you want your arms to be stretched out wide to your sides with your elbows slightly bent and your palms facing inward, toward one another. When you lift the dumbbells up together, visualize hugging a giant tree trunk. At the peak of the movement, really squeeze your pecs together for a one-count. Be sure to lower the dumbbells in a slow and controlled fashion. Repeat this movement for as many repetitions as you can until failure. Remember to always use a spotter when performing this and almost every other free weight exercise! With all this you must take care of your diet too – eat healthy and apt food, do not go for crash diet as it may lead to weakness as you have taken up a exercise plan and you need to take some calories to burn them. Big size Breasts although attract a lot of attention- they may cause you embarrassment sometimes too so to avoid physical and mental pain from your big bust you must stick to some given food choices: 1.Fruits – Fruit diet is what is always recommended, a bowl full of fruits that are rich in nutrients and water content should be your first preference. Fruits contain antioxidants that help you reduce weight and remain healthy. This will help you maintain your body fat proportion and thus a natural way to reduce breast fat. 2.Vegetables- Having lot of vegetables is also recommended as like fruits Vegetables are also good source of nutrients and antioxidants that keep you metabolically active. Being active means you will not tend to put on weight and managing weight is the key to your over grown bust. Veggies like Spinach, Brussels sprouts, broccoli, cauliflower etc. That have no starch are best. 3.Fish- best protein source- For your daily protein source you must rely on fish only this is the lowest calorie protein source. This will help your muscles in getting healthier and toned. Fats are minimal in fish. Fishes like Tuna, trout &salmon are best to use. 4.Nuts- nuts are also source of energy and necessary antioxidants and nutrients which are necessary for a female body to cope up with everything. Best nuts that you must include in your diet are walnut, almond and Brazilian nuts along with macadamia nuts that are neither salted nor coated with sugar. The intake of nuts helps you maintain healthy metabolism again the key to weight management. The food management is an important part. If you have a healthy diet you tend to manage weight and have less body fat which leads to automatic and natural reduction of breast. This regime if combines with exercise works best.
