Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. I need to know the duration of your back pain and leg pain and mode of onset. I would also like to ask you if it is associated with other neurological symptoms like paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. Please also send me your mri films and report as an attachment so that I can see and advise you accordingly. Do not hesitate to contact me if you need any further assistance.

Respected sir, my father is 59 years old suffering from spinocerebellar ataxia. Progressive in nature. 1. Slurring of speech since 2007 2. Gait abnormality since 2009 3. Writing abnormality since 2009 he is not having any comorbidities neither any family history. Please help me if you have any suggestion for him.

DM - Neurology
Neurologist,
Progressive sca at 59 years age is unusual and should trigger a search for various conditions: cerebrovascular disease, demyelinating disease, infectious diseases, vitamin deficiency, toxic exposure, sarcoidosis, autoimmune diseases (eg, sle, sjogren syndrome), and alcohol-induced cerebellar degeneration. Other diseases that can mimic this condition include late-onset spinocerebellar ataxia with or without a family history, olivopontocerebellar degeneration, and other degenerative diseases of the brain. The most problematic is paraneoplastic cerebellar degernation, even though it is slightly more common among women; in men, it may signal an occult small cell ca of the lung, or hodkgin lymphoma. Apart from the tests for the other conditions mentioned, a test for anti-yo antibody and a search for occult malignancy should be done. Treatment is by physiotherapy/gait therapy, and treatment for the underlying condition if possible.

Migraine is a kind of relapsing headache. Its symptoms include - episodic unilateral or bilateral, severe and pulsating headaches, associated at times with nausea and vomiting, relieved momentarily on rest or taking medications and aggravated by precipitating factors like sleep deprivation, loud noises, continued sun exposure, allergy to some agents etc. The treatment of migraine is mainly done for relief from acute headache attacks and later treatments to prevent future relapses. Also treatment of factors which tend to cause relapses is also important. Consult a headache specialist for treatmnet. You may contact me online for an further queries and management.

DM - Neurology, MD - General Medicine, MBBS
Neurologist, Chennai
Hi Mr. Narasimha Rao, You can't decide with single nerve conduction value. Also NCV is different for different nerves. One need complete nerve conduction report and full history of the problem and examination of the person to come to a conclusion.

Better see a neurologist in your area. You need good painkiller (like naxdom) which ever suits you. Some prophylactic medicines also required.
