What is the best medicine for the migraine problem while I have severe head pain? Please help me.

MBBS, MD - Community Medicine

What is the best medicine for the migraine problem while I have severe head pain? Please help me.
It is a type of headache which is initially unilateral (right/ left side of head) and localized in the front side and eye area of head, but pain can be felt anywhere around the head or neck. There may be associated nausea, vomiting, intolerance to light, and many others. If you have this type of symptoms, follow the advice: 1. Take tablet paracetamol 500 mg within 15 min of starting headache. If headache does not respond to paracetamol, you may take tablet headset one tablet at the start of headache, you can repeat again after 2 hours of previous dose but maximum 2 doses in a day. 2. Yoga, meditation, stress relieving techniques help you in relieving symptoms. 3. Identify the trigger which causes this type of headache, and avoid that i. E. Coffee, alcohol, chocolate, any particular fruit or nut, or else. Consult again with answer (yes or no) to following questions for further prescription to manage your migraine: 1. Unilateral location (right or left side) 2. Pulsating or throbbing quality of headche 3. Moderate or severe pain intensity 4. Aggravation by or causing avoidance of routine physical activity (eg, walking, climbing stairs) 5. Nausea and/or vomiting 6. Intolerance to light or sound while headache.

I am suffering from insomnia. It is difficult for me to fall asleep and I wake up tired in the morning.

PDDM, MHA, MBBS
General Physician, Nashik
I am suffering from insomnia. It is difficult for me to fall asleep and I wake up tired in the morning.
First thing that should be done in this problem is to improve sleep hygiene. - Fix your time to go to sleep. - Avoid alcohol, tea, coffee in evening - Light exercise will also help in getting good sleep - Use your bed for sleeping only and don't watch TV, read books on bed - Avoid day time naps - If you fail to sleep within 10 minutes of going to bed, get up and start to distract yourself. - Bath with warm water in evening will help. Try to improve sleep hygiene and if condition don't improve consult a psychiatrist or sleep specialist. Medicines like Benzodiazepines like Clonazepam, Alprazolam etc help in getting good sleep. Z-drugs like zolpidem or zopiclone also help in getting good sleep. These drugs have abuse potential so should be taken for short time only and under proper prescription. Thanks, hope this helps you.

What is the solution of my 9 years daughters neurogenic bladder and bowel incontinence, Dr. Please advise me.

C.S.C, D.C.H, M.B.B.S
General Physician, Alappuzha
Neurogenic bladder is bladder dysfunction (flaccid or spastic) caused by neurological damage. Symptoms can include overflow incontinence, frequency, urgency, urge incontinence, and retention. Risk of serious complications (eg, recurrent infection, vesicoureteral reflux, autonomic dysreflexia) is high. While neurogenic bladder can't be cured, necessarily, it can most definitely be managed. Most cases of neurogenic bladder can be managed with medication and intermittent catheterization. The minority of children with the condition need major reconstructive surgery.
