If controlled with tablets can delay surgery or else have to do, before that do a PSA and a biopsy to rule out malignancy.

My wife ultra sound report said Opinion rt sided mixed adenenexal mass? Twisted ovarian cyst. Please tell me what should we do?

MBBS
General Physician, Kolar
A twisted ovarian cyst is at a risk of getting congested with blood as the blood vessels also get twisted. She might have been experiencing pain from before. But now I guess the pain has become worse. Please take her to a gynaecologist as soon as possible with the report as she might need further evaluation and maybe surgery also.

Because his request are completed by non verbal actions. You have to make him speak by ignoring his activities. Leaving him in children of similar age group will be useful. Correct his words & pronunciations when needed. Never imitate childs voice & words for joy.

I am suffering from fever for last five day. How can I improve our health. Please help.

BAMS
Ayurveda, Bangalore
Hi, please go for a detailed blood investigation like cbc, widal test, mp and routine urine examination and then revert back with the reports. Till then keep recording your temparature. If it is too high, do tepid sponging. Wear loose cotton garments. Drink plenty of fluids. Consume light and easily digestable food. Continue to take tab. Paracetamol 1-1-1 till the fever subsides. Drink only boiled and cooled water.

Helo, i'd like to ask to you about something that really has been stressing me out and having nightmares about it. Recently I went to a trip to Baku, I had sex with an escort unfortunately but she had A HEALTh CARD so at first I was using a condom but last 5 minutes I took it off, She seemed really clean but am just really worried if I caught HIV, There were no symptoms showing too, I ran over HIV TOPICS recently and thoughts started to storm in my brain, It really causing me to stress and can't stop thinking.

MD - Psychiatry
Psychiatrist, Chennai
Cleanliness does not prove a person does not have HIV. You should have never taken off the condoms. The escorts are periodically tested for HIV, but during the window period they can have the virus, but may show negative results. Some escorts who are HIV +, deliberately can carry a duplicate card to fool unsuspecting customers. When was the exposure? Check for HIV antibodies 3 months after the last encounter, if negative do not worry. If you want to test early there are tests which can show the HIV antigens around 2 weeks from exposure. All the best, in future always use condoms.

Diabetes and Cancer: The New Epidemic?

MBBS, DNB (General Medicine)
Endocrinologist, Delhi
Diabetes and Cancer: The New Epidemic?

There was an era where we had deadly infections like plague and polio causing death of thousands of people. We still have occasional outbreak of swine flu, but by and large, infections are quite controlled. The new killer diseases are caused because of the lifestyle we have adapted and the damage we have done to the environment. High intake of processed foods, artificial chemicals in our foods, sedentary lifestyle with very minimal to no physical activity, couching over the computers; the list is quite long.

All these have led to diseases like diabetes and cancer that were not so common about say few decades ago:

Detailed observation has revealed that both these new epidemiologic diseases have a close correlation. There are factors, which induce diabetes and diabetes in turn and in some cases, diabetes inducing agents, can cause cancer also. It has also been observed that mortality is severely increased if diabetic patients are diagnosed with cancer. There are two types of diabetes. While type 1 is mostly hereditary, type 2 is lifestyle induced and the age at which this is being diagnosed is taking a severe plunge. Adolescents and teenagers are being diagnosed for diabetes. Cancer, on the other hand, is of various types (leukaemia, melanoma, myeloma, etc.) and can affect various organs (lung, breast, prostate, stomach, liver, etc.).

The medical community is yet to decipher the disease pattern of both these conditions. While there is no definite correlation between diabetes and all types of cancer, some types of cancer are definitely correlated with a definite reason identified, pancreatic and liver cancer for instance. The high amounts of insulin that diabetic patients are exposed to causes changes in liver and pancreas including fatty liver and cirrhosis, here the incidence of cancer is higher. The linkage is not very clear in lung and intestinal cancers and also there is no link between prostate cancer and diabetes.

Diabetes is considered as a state of chronic inflammation and leads to conditions like hyperinsulinemia (higher levels of insulin in the blood) hyperglycemia (higher levels of sugar in the blood). These are believed to aggravate the neoplastic process of cancer formation, thereby inducing cancer at a greater pace and also increasing the mortality rates.

The following are risk factors that are applicable to both age, physical activity, diet, obesity, drinking and smoking. It is also possible that onset of one can be followed by the other. As noted earlier, more detailed research is awaited to establish a definite linkage, but the correlation cannot be ignored at all.

Both these new epidemics are here to stay and since they have a common set of factors, we need to work on ways to contain them.

Dear lybrate-user, Do not change shampoos frequently. Use only coconut oil by mixing camphor three four pieces in 100 ml coconut oil twice in a week. Or Take 2 aspirin tablets Shampoo Crush the tablets. Add it to any normal shampoo. Let it lather and keep it on for 5 minutes. Wash your hair. If you feel the aspirin powder on your scalp, rinse again with plain shampoo.

3 Ways a Dialysis Patient can Cope with Depression

DM in Nephrology, MD in Internal Medicine, MBBS
Nephrologist, Ghaziabad
3 Ways a Dialysis Patient can Cope with Depression

A diseased kidney is one that has lost its ability to carry out the filtration process in a proper manner. This process of filtration usually sends the waste and toxins out of the body in a routine manner. In such cases, a kidney disease manifests which requires a number of treatment measures. Dialysis is something that enters the treatment plan at a later stage as chronic early stage kidney disease can remain in the patient’s body for many years. When the kidney function comes down to 15%, then most doctors recommend the start of dialysis which is basically a process that removes the excess waste and fluid from the blood. This can be a process that will leave the patient fatigued and in a mental state that is not exactly stable and positive at all times. It is common to see dialysis patients battling depression due to the chronic nature of the treatment as well as the low chances of recovery.

So, let us find out how dialysis patients can cope with depression.

  1. Professional Help: Depression is said to be like a common cold when it comes to mental diseases. The state of a person’s body can very well affect the balance in his or her mind and lead to depression. Starting from pressing circumstances to hormonal changes as well as chronic ailments, depression can affect people due to a number of reasons. Dialysis patients who are undergoing depression must consult a professional specialist like a psychiatrist who will help the patient understand and cope with the situation with the use of therapy based counselling.
  2. Medication: The patient can also be given medication that will prevent the build up of unnecessary stress by blocking the production of those hormones that result in a negative state of mind. This medication must be prescribed after speaking the patient’s nephrologist about what kind of medication will suit the patient specifically.
  3. Psychotherapy: Long term psychotherapy is also known as talk therapy that can help the psychiatrist or clinical psychologist in talking the patient through the problems. Besides giving the patient the scope and platform to air out the problems and reach solutions regarding normal day to day functioning despite the troubles, this kind of therapy also aims at equipping the patient with a better perspective and improved attitude.

While going through this kind of therapy and medication, it is important to keep a team of nephrologists and psychiatrists in the loop so that the patient may reach out at any time in order to avail help in the most trying situations. This will give a blanket of security to the patient as well. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Hi! My son is of 8 years .we are dstrbd about his sexual organ which measures only an inch. It gets erect while passing urine so ,doctor told its nt a matter. But from long time its not at all growing .plzzzz suggest.

BASM, MD, MS (Counseling & Psychotherapy), MSc - Psychology, Certificate in Clinical psychology of children and Young People, Certificate in Psychological First Aid, Certificate in Positive Psychology
Psychologist, Palakkad
Dear lybrate user. I can understand. SIZE, THICKNESS AND HARDNESS of penis never matters in a sexual intercourse. Love and passion matters and determines the sexual pleasure. Usually, in flaccid condition, the size will be less than 2 inches. Average size of erect penis is 4 inches. But even a two inch erect penis can satisfy any woman across the world just because, woman sexual nerve endings which are the reason for pleasure for women, are available only up to two inches from the opening of vagina. Therefore, please don't worry. Your son is just 8. He has lot of time and it will grow. Take care.

I realized that from 6 month my sex desire is not like that earlier like I am feeling not excited or there is no any feeling with sex, 2 month back I engaged with my gf for the first time in my life. My sex was less then 1 minute if I say it was for only 30 to 40 second after few days I again try but again same problem I thought may be because I am new or due to nervousness, there for I tried again after few days but again same result every time I also feel that my penis is not tight as it has to be, one day I tried with Viagra 50 mg this time I stand for 3 to 4 minutes and penis was also tight with good erection I do not know how this problem came with me because I realised this problem from 6 to 7 month before that when ever I did masturbations I had good erection and no issue with penis. Now I am worried about my future as I am only 25 year old. Is I am suffering from ED & premature ejaculation.

MD-Ayurveda, Bachelor of Ayurveda, Medicine and Surgery (BAMS)
Sexologist, Haldwani
Hello- While lack of libido is sometimes related to hormonal imbalances, it's important to understand the other factors which can lower libido and contribute to erectile dysfunction and premature ejaculation. Two of the most common sexual dysfunctions that affect a large percentage of men are Premature Ejaculation (PE) and Erectile Dysfunction (ED). When it gets added by lack of libido the situation get more complicated. Due to reduced or hampered blood flow (critical to achieve a penetrable erection) to the genital areas is one of the main reason for premature ejaculation and erectile dysfunction, the logic of 'one reason one problem' gets applied. It can be corrected and recovered permanently with the help of ayurvedic medications and exercise schedule.

General Physician, Kolkata
Mr. Lybrate-user, sorry for your friend's loss. 1 month grief/depression post death is very normal. The best thing you can do is to be there for your friend. If it continues post a couple of months, you can try consulting a therapist. In the meantime, she can use paracetamol (crocin) 500mg once or twice a day as necessary. Regards.
