Hello doctor I have a question from you about my cousin and he is about 20 years old. He is well and fit physically but due to some unavoidable natural misfortune he got orphan at the age of 15 and from five years yet he is very disturbed physiologically and continuously in deep stress. I have a also pity and sympathy for him. The question is from about five to six month his behaviour is strange and abnormal. He often times does like a complete mad, after some times he becomes as usual. Please tell me what should be done to reduce his stress and to vanish his that memory of those incident from his mind. And to make his livehood better. Thanks and will be waiting for reply.

Hello, welcome to lybrate sorry to hear about the tragic incident that happened to your cousin. Such traumatic life events are a source of immense stress. Such stressful conditions can trigger the development of an abnormal personality, an underlying mental illness, which might remain hidden till a new triggering factor is encountered in life. Also such life events makes an individual more vulnerable for stressful situation in future, reduces stress coping abilities and can lead to maladaptive changes in ones behaviour. As you told that since last 5-6 month he had started behaving abnormally, with increased aggression. This might be triggered by some other incident which need to be explored. Also it can be the beginning of an underlying mental illness. Hence before going for managing these symptoms, a thorough assessment is necessary in order to understand his emotional and psychological state. This will help in understanding him better, & would help in making a better management plan for his issues. Many Faces of Depression : Could that unexplained headache be Depression?
Shruti (name changed) aged 25 years, BA, homemaker and mother of two kids was a good cook and managed her house to perfection, much to anybody's envy until one day she started experiencing sudden severe headache and backache.
These symptoms made her incapable of carrying out her household duties as efficiently as she used to earlier.
Concerned with her pain & apparent symptoms severity, her husband took her to a general physician who prescribed a battery of tests including MRI Brain only to find no abnormality.
Attending Physician prescribed some multivitamin syrups & pain killer medicines & also tried to assure the family that NOTHING was wrong with Shruti.
However, she continued to complain of body aches and headaches, with resultant chaos on home management front compelling her husband to hire a maid to look after all the work including health of Shruti.
Shruti started feeling guilty about this & she started remaining aloof and also had trouble sleeping at night.
Frustrated with the added financial pressure and Shruti's ever increasing health complains, her husband accused her of faking it in order to avoid doing work.
Her in-laws added fuel to the fire citing it was all a drama. This further added to Shruti's sense of guilt and her appetite significantly reduced. 4 months after onset of these symtoms, Shruti lost 10kg weight.
She started remaining on the bed for hours and hardly interacted with anyone. She also started voicing ideas of ending her life in order to relieve everyone off her responsibility & enabling them to lead a happy life!
Now concerned, her parents took the matter in their hands & took her to a psychiatrist.
At the psychiatrist’s clinic, after detailed evaluation , Shruti was diagnosed to be suffering from depression (severe without psychotic symptoms) and the psychiatrist pointed out that the headaches & unexplained body aches were a the starting points of depression itself. She was started on medications and counselling therapy and within 2 months time she recovered completely & once again happily assumed her normal HOME MINISTERS ROLE!
Depression is one of the most common mental disorder and seen in every 4 out of 100 persons as per World health organization (WHO) data
When one thinks of depression; a sad, tearful face comes to mind. But depression is more than just that. unexplained headaches, body pains, sleeping difficulties are often the presenting complaints of depression. Other presentations of depression are chronic tiredness, joint pain,back pain, gastrointestinal problems including frequent acidity, indigestion, appetite changes etc. without any probable detectable physical cause
Depression contributes greatly to the disability caused by headache and backaches. More than 50% of patients with depression who visit general practitioners complain only of physical symptoms and in most cases, these symptoms include pain. If physicians test all pain patients for depression, they might discover 60% of currently undetected depression in their OPD.
Complete Recovery from an episode of depression is possible & achievable to everyone after proper treatment with medicines, counselling sessions & lifestyle modifications!

Because of stress negative thoughts may come. In stress our mind can have loss of positive manner. So it is possible of negetive thoughts. So then you follow stress relief methods. Take the help of friends or elders. Share your feelings with your closed friends, recreate your own entertainment channels. Take a special space for yourself. Visit temple's, movies, new places, new taste of food. Change the lifestyle management. Be friendly with others. Be happy all the time. All the best.

Dear Doctor, I have a problem of unnecessary thinking or daydreaming. It is positive & slightly confidence boosting. But, the Problem is that I am wasting a lot of time doing for last few months. When I am doing something important, after sometime I find myself thinking about unrelated topics. I tried concentration boosting exercises, but till now the problem is intact. I am a hard working person, but this is becoming a drag. Could you please tell me about how I can solve this? Thanks.

Hi lybrate-user, Try mindfullness breathing exercise. The primary goal of mindful breathing is simply a calm, nonjudging awareness, allowing thoughts and feelings to come and go without getting caught up in them. Sit comfortably, with your eyes closed and your spine reasonably straight. Bring your attention to your breathing. Imagine that you have a balloon in your tummy. Every time you breathe in, the balloon inflates. Each time you breathe out, the balloon deflates. Notice the sensations in your abdomen as the balloon inflates and deflates. Your abdomen rising with the in-breath, and falling with the out-breath. Thoughts will come into your mind, and that’s okay, because that’s just what the human mind does. Simply notice those thoughts, then bring your attention back to your breathing. Likewise, you can notice sounds, physical feelings, and emotions, and again, just bring your attention back to your breathing. You don’t have to follow those thoughts or feelings, don’t judge yourself for having them, or analyse them in any way. It’s okay for the thoughts to be there. Just notice those thoughts, and let them drift on by, bringing your attention back to your breathing.  Whenever you notice that your attention has drifted off and is becoming caught up in thoughts or feelings, simply note that the attention has drifted, and then gently bring the attention back to your breathing. It's okay and natural for thoughts to enter into your awareness, and for your attention to follow them. No matter how many times this happens, just keep bringing your attention back to your breathing. If problem persists show it to a psychiatrist.

Stammering could be because of ENT cause or psychological. However even if it is purely ENT also, nervousness, fear of embarrassed make it worse. You need therapy like desensitization to get over it. Try speaking in front of mirror, select the words which are difficult for you ,try replacing with alternate words, give pauses, try speaking like a newsreader etc. Finally, it is something which is not your fault so be fearless. You can also choose a career where speaking is not so important. Try also to use body parts to increase effect of communication.

The changing life style, nuclear families, pollution, very demanding jobs, dissatisfaction in relationships are the major causes of stress. It is advisable that you share your worries with someone you trust. By doing this you will lighten your burden and also might get a different perspective about the difficulties in your life. At the same time, do not take chest pain lightly. The pain may be because of stress or any other physical ailment. Please get yourself checked. Consult a doctor at the earliest. Take care.

Here are the symptoms that indicate you're suffering from anxiety disorder. 1. Excessive worry - This condition persists if you worry about the day to day activities a bit too much on a regular basis. It is identified when you start taking too much stress about very petty things, which tends to affect you in an adverse manner. A noticeable sign of excessive worry can be too much fatigue. 2. Sleep problems - Problems falling asleep at the right time and problems in maintaining a sound sleep are two signs that indicate you have an anxiety disorder. 3. Irrational fears - This symptom is not generalized; instead, it is specific and subjective in relation to a particular thing or situation. The fears that are experienced under this category are sudden and unexpected in nature and there is no fixed rationale behind these fears. 4. Muscle tension - Muscle discomfort and pain is one of the leading physiological causes of an anxiety disorder. I am very depressed. I think I cannot do any thing. All the time I am thinking to die. I have a little daughter of 4 years. Previously I was working in a bank. After my daughter I had to resign. Till now unable to work. We have no family support at all. Wat can I do.

You should be able to distinguish between disappointment and depression. Disappointment also brings on symptoms identical to depression but they are short lived. You are disappointed due to your loss of job. Now you have to pull your pants, get up and go to job. Because there is no other alternative. I will help you.
