I am 51 years old man working in a Govt. Office. My nature duties are of sitting 8 hours doing stationary job. Off late, I have been suffering with pain or burn under my right sole as well as left sole. I feel more pain/burn in the right sole. I am even not able to walk properly. I got tested myself whether I am suffering from diabetes but, the report was negative. Kindly advice.

Knee pain can be cured by strengthning the muscles supporting your knee joint. Replacement is not the only option. Start quadriceps strengthning exercise. Sitting on chair and raising your leg. For 20 times. Slr, knee bending, foot strengthning will also help. Use some analgesic gel and knee cap for pain relief. Hot pack can also be used. 82 years my father suffer with backach and low backach since last 10 years and he takes number of medicines prescribed by expert doctors. Hi , he' s taking too many medicines which will give him abdominal pain. I would suggest for a home physiotherapist who can make him to do some active assisted exercises and help him to walk. After looking at you dad and assessing him the physio will be able to suggest you whether cold or hot fomentation will help for his back. In case of disc bulge and any tear then cold pack works well and in case of muscle spasm hot works well Dr. Avanthiprabhakar

Hi Thanks for your query and welcome to Lybrate. I am Dr. Akshay from Fortis hospital, New Delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - To maintain proper posture of your back while working and sleeping - If pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - Physical therapy initially under supervision of a trained physiotherapist and then to continue at home - Adequate calcium & vitamin d intake if levels are low in body - Ice packs can applied if your pain is acute, then hot fomentation can be done at home - Analgesic spray for local application can be used and is easily available. - Avoid lifting heavyweight/ acute forward bending in mornings etc We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Wounds On The Feet, Not Healing? Find out why!

An injury runs its course of a few days to weeks depending on the size and heals and closes. There could be scarring or mobility issues in some though. However, this does not happen in all cases, and a wound that has not healed for more than 6 weeks is termed as chronic. They are most common in the hands and feet. In those who have mobility issues, hips, buttocks, and thighs are also common areas.

Read on to know more about why this happens and how they can be treated.

Reasons: Though there could health conditions aggravating the healing, most wounds heal late due to 3 reasons:

  1. Poor circulation: A healing wound requires nutrition for the cells to repair, and if there is improper circulation, then healing is definitely delayed. This can happen due to diabetes, malnutrition, reduced mobility, vascular disease, etc. In many unsuspecting people, nonhealing wounds have led to diabetes being diagnosed. Diabetes also affects nerve supply, so delayed healing may go unnoticed too. Post injury, it is also important to completely stop movement, as circulation needs to be maintained for proper, quick healing.
  2. Infection: By preventing tissue regeneration and interfering with blood clotting, they can prolong bleeding and cause chronic inflammation. This leads to delayed healing and in most cases, worsening of the current situation.
  3. Fluid accumulation/edema: Any injury causes leakage of cellular fluid into the surrounding tissues. This causes edema and the surrounding tissue is often swollen. This leads to poor circulation also, which is another cause for delayed healing. Edema also reduces tissue repair difficult, not allowing skin to form over the wound.

Symptoms: While the wound per se is an indication, there are also other symptoms which indicate a non-healing wound.

  1. Pain, which has not subsided
  2. Swelling, discharge, odor, and skin color changes due to the ongoing infection. A thin discharge is not worrisome, but a thicker or foul-smelling discharge is definitely a cause for concern
  3. Open wound: Most wounds heal within the 30-day period. If the wound is open beyond that, it is always advisable to seek medical support.

Management: This would include generic and local treatment.

Generic treatment would include a nutritious diet, good sleep and rest, adequate exercise and rehab program to ensure adequate mobility, and frequent monitoring. In addition, conditions like diabetes have to be monitored and brought under control.

Local treatment options would be:

  1. Wound debridement and skin or tissue grafting
  2. Medications to improve wound healing
  3. Hyperbaric oxygen to provide more oxygen to the wound in particular
  4. Vascular repair to improve circulation.

Delayed healing of wounds should not be ignored; timely medical help can avoid complications.

Hi, Take tab. Shallaki 400 mg 1-2 tabs 3 times a day depending on the severity of the pain. Take Balarista 2tsf 3 times a day, mixed with equal quantity of water and Maharasnadi Khadha 2 tsf 3 times a day, mixed with equal quantity of water. Apply Rumalaya liniment to the affected joints and massage in circular motion for 10-15 mins and then take hot water fomentation. Avoid potatoes, sweet potatoes, sweet pumpkin, peas, channa, recently harvested rice, jackfruit, etc. Which aggrevate the pain. You can even undergo physio- therapy for 10-15 days.

Advice. Avoid sitting cross legged. Ankle stretching. Ankle strengthening exercises. Avoid squatting. Quadriceps exercises. Core strenthening exercises. Stretch calves get a custom made insole. Hams stretching. Sports taping. Hot fomentation.
