Dear Lybrate User, You may be suffering from a clinical condition called PHIMOSIS. The most conventional method to get rid of it is surgery. However, if you are unwilling to undergo surgery then you can try the following process at home. 1) Buy Betamethasone cream from local allopathic shop. During buying you should check the composition of the cream to confirm whether it contains 0.05% Betamethasone in it. If you cannot find 0.05% Betamethasone cream then you can buy any local Anaesthetic Gel (e.g. Lidocaine, Lignocaine, Xylocaine) from allopathic retail shop. 2) Apply a little amount of that anaesthetic gel at the lower part of the prepuce of penis where the foreskin remains attached. 3) After application of the gel, retract the foreskin backwards as far as you can. Due to the anaesthetic property of the gel you will be able to retract the foreskin backwards to some extent more than you can do in normal condition. 4) Release the foreskin when you feel pain 5) Again repeat the same process. Do it continuously for at least 30 minutes in a day. Within a month you will feel that your foreskin has become loose

Nonsteroidal anti-inflammatory drugs, Corticosteroids, Analgesics. If pain is in the soft tissues, you may find relief from an analgesic salve, rub or balm available over the counter under trade names such as Aspercreme, Ben-Gay, Capzasin-P, Eucalyptamint, Icy Hot, Voltaren Gel and Zostrix.  These topical preparations work through a variety of active ingredients and may be helpful if you are unable to take oral medications, or if medications fail to relieve pain or reduce it to a manageable level.

Hi I have a pain in my neck from Saturday. I have consulted a doctor and as per the xray and diagnosis he informed there is a slight tear ( or scratch ) in the disc of upper portion of spinal chord. Just below the neck. I find very difficult to sleep in any position. I have the xray image. Kindly suggest me your best advice. I can send the X ray image as well

If your xray is there can you send me. But as per your pain xray is not suggestive of any type of tear in disc because disc is not visible in xray. And as per my suggestion you should do an MRI of cervical spine to get the confirmation of disc tear. Thanks

It may a mechanical pain as a result of your occupation which involves sitting and driving for long hours. I suggest you consult a Physiotherapist or an Orthopedic doctor, get yourself examined, get a X ray or MRI done according to their advice. If you already have X ray or MRI, then meet the doctor for treatment options. Visit a good physiotherapist, so that your problem can get resolved without any medication, if its not very severe.

Hello Mr Sreekar. Thank you for contacting. Sometimes vibration and fluctuations in muscle of leg occurs. It may be due to low water and salt intake. Take some pich of salt add in water. Drink it if you sweat a lot. Also stretch your buttock and thigh muscles. I hope this will help you.

Rheumatoid Arthritis - How to Manage its Pain?

Arthritis is an autoimmune disorder wherein, the immune system of the body attacks healthy tissues within the various joints of the body. This causes inflammation of the joints, leading to multiple problems in them. One common issue with all the problems is pain. Thus, managing pain with rheumatoid arthritis is one of the most important facets of the treatment. More than the deformities or other problems, it is the pain that limits your capabilities. Some techniques for the management of pain are mentioned below.

  1. Pain medication: This is the obvious and first choice to avail if you need to reduce pain. There are many types and levels of pain medications which can range anywhere from being mild to very strong. However, this isn’t an option that should be considered lightly as you may have a tendency to develop a dependency which can border on addiction. Also, many of them harm the hepatic and renal system of the body if used beyond a certain period. Thus it is advisable to use this prudently.
  2. Weight management: In many cases, the pain is exacerbated by obesity. It has been noticed that obese or overweight people tend to suffer more from arthritis pain as the added weight tends to add pressure on the joints. Thus, one of the best ways to reduce a significant amount of pain would be to reduce weight through dieting and limited exercises that don’t strain the joints. However, it is easier said than done as a person in pain cannot usually exercise! So, the best strategy is to visit a pain specialist / interventional pain physician and get your pain controlled. Thereafter, start the exercises and reduce the weight effectively.
  3. Medication for inflammation: Inflammation of the joints is the primary symptom of this disorder. Inflammation is also what causes the pain within the joints. Thus, treating the inflammation with relevant medication has also been known to reduce pain. This is usually the first option to go to for relieving arthritis pain. The catch point here is also the side effect profile of these drugs.
  4. Changes in Diet: A few type of foods, especially certain fish found in colder waters are a rich source of omega-3 fatty acids, which are very good at reducing inflammation within the joints. There are also certain food items which may be bad for your arthritis pain as they may aggravate inflammation and it is better to avoid such items altogether. Some examples of such items would be sugar, alcohol, white flour, processed meats, gluten rich foods among others.
  5. Massages: Whether it is a self-massage or from a therapist, it is a great way to encourage circulation which can help reduce inflammation and work as a pain reliever. A therapist would also know certain specific techniques to relieve arthritis pain. However, most of the times, improper massage shall do more harm than good.
  6. Interventional Pain Procedures: These may often be the most cost effective and usually the only option to reduce pain. These include joint injections, neurotomies, radio frequency therapy, laser therapy, ozone therapy, platelet rich plasma or anti-inflammatory injections.
  7. Other therapies: Some other therapies include heat and cold compress, acupuncture, wearing supportive wears and braces among others to encourage circulation and reduce pain. Transcutaneous electrical nerve stimulation also helps to relieve pain by stimulating the affected nerves in the joints with the help of a low voltage electric current. If you wish to discuss about any specific problem, you can consult a pain management specialist.

Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily.

Advice. Regular stretching of shoulder muscles. Warm up and cool down is a must. Hot fomentation. Shoulder shrugs. Take frequent breaks at work. Core strenthening exercises. Use cervical pillow. Use back support. Self massage the back of neck.

Hello lybrate-user, besides all these things, you can do stretching exercises of calf muscles, stretching of plantar fascia with wide opening of the feet, standing on toes. * walking regularly at slow pace without unduly straining helps as it improves blood circulation around knee and eases the joint. *you can use mustard oil for massage which is good in winter season for relieving pain. Eucalyptus oil is also good. Coconut oil will be soothing in summer. Homoeopathy has a good treatment for this without adverse effects. : Keep your leg raised while sitting or lying Quadriceps strengthening exercises- Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight Leg Raise: Lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise. Hamstring stretchingTowel Hamstring Stretch Lie on the back, supporting the thigh with the hand or with a towel wrapped around it. Slowly straighten the knee until a stretch is felt in the back of the thigh, trying to get the bottom of the foot to face the ceiling, one leg at a time. Hold the position initially for 10 seconds, and gradually work up to 20 to 30 seconds.
