Endometriosis - How It Results in Infertility?

The uterine cavity has a tissue base that is known as the endometrium. When this endometrium spills over or spreads to the other parts of the reproductive organ, it begins to punch through the uterine lining. Such a condition creates complications and even infertility, and is known as endometriosis.

Read on to know more about endometriosis and how it can cause infertility.

Endometriosis

When the endometrium tissue spreads to areas like the ovaries, and the abdominal cavity, it can lead to numerous complications. Apart from pain, this may also cause infertility. 5 to 10% of the women worldwide suffer from this condition and have trouble conceiving. Additionally, about 30 to 40% of the infertility cases are due to endometriosis, not all cases. These figures are all confirmed by various medical studies.

How does it cause infertility?

Endometriosis can prevent ovulation, which is the process that occurs every month. During the ovulation process, the ovaries release eggs that may be fertilised in order for the woman to conceive. In this condition, the fimbria of the fallopian tubes also has trouble in capturing the egg, which prevents conception from taking place. This leads to infertility in the long run. Endometriosis also interferes with the proper production of the hormones that are required for normal ovulation and fertilisation of the egg.

Diagnosis: The diagnosis of the condition can be done with the help of lab tests and ultrasounds as well as imaging tests that can create the image of the womb and the endometrium lining. This will help in showing the severity and damage caused by the tissue. A surgical procedure called a laparoscopy may be carried out by the doctor to ascertain whether the patient is suffering from this condition. In this procedure, a small incision will be made and a camera will be inserted to view the insides on a screen, to learn more about the tissue’s spread and the kind of treatment that will be required. The doctor will also diagnose the condition on the basis of various symptoms like irregular and heavy bleeding.

Treatment:

Treatment for endometriosis associated with infertility needs to be individualized for each woman. There are no easy answers, and treatment decisions depend on factors such as the age of the woman, the severity of the disease and its location in the pelvis, the length of infertility, and the presence of pain or other symptoms. Some general issues regarding treatment are discussed below.

Treatment for Mild Endometriosis

Medical (drug) treatment can suppress endometriosis and relieve the associated pain in many women. Surgical removal of lesions by laparoscopy might also reduce the pain temporarily.

However, several well-controlled studies have shown that neither medical or surgical treatment for mild endometriosis improving the pregnancy rates for infertile women as compared to expectant management (no treatment).

For treatment of infertility associated with mild to moderate endometriosis, controlled ovarian hyperstimulation with intrauterine insemination - IUI is often attempted and has a reasonable chance to result in pregnancy if other infertility factors are not present.

Details about IUI success rates with endometriosis

IUI and endometriosis

Success rates with IUI for endometriosis have been variable in studies, showing:

  • A pregnancy rate of 6.5% for women with endometriosis vs. 15.3% per cycle for unexplained infertility
  • A pregnancy rate of 5.6% for women with advanced endometriosis vs. 22.7% per cycle for mild endometriosis vs. 25.7% for no endometriosis
  • Pregnancy chances with insemination for natural cycle IUIs (no drugs) with endometriosis are about 2% per cycle vs. 11% with injectables plus IUI for endometriosis

Treatment for severe endometriosis

Several studies have shown that medical treatment for severe endometriosis does not improve pregnancy rates for infertile women.

Unfortunately, infertility in women with severe endometriosis is usually resistant to treatment with ovarian stimulation plus intrauterine insemination. If the pelvic anatomy is very distorted, artificial insemination is unlikely to be successful. These women often require in vitro fertilization in order to conceive.

Although the studies of in vitro fertilization for women with severe endometriosis do not all show similar results, pregnancy success rates are usually good if the woman is relatively young (under 40) and if she produces enough eggs during the ovarian stimulation. If you wish to discuss about any specific problem, you can consult a Gynaecologist.

If your thyroid hormones now normal and other hormones also normal then you can get pregnant easy. Good time for sex is 8th days to 15 days after menses period. Ovulation time mostly is 7th days after menses. To confirm ovulation of egg. Pl. Do usg for ovulation of egg. And reply on lybrate. Com

Hello, this may be an ovulation bleed and not an implantation bleed. However you rule out pregnancy by a serum beta hcg test. You may review with reports.

First of all start taking folic acid once a day till 9 months of pregnancy avoid excess of oil and spices take seasonal fruits and green leafy vegetables try to take two times fruits and 4 to 5 times small frequent meals if she is not able to eat in one go each time she may have nausae and vomiting which may need symptomatic management with vitamins after checking with Dr. take lots if fluids avoid heavy work avoid jerky roads and travel get ultrasound done at 6 weeks 11 to 14 weeks as part of first trimester screening.

Dear lybrate user, pcod (polycystic ovarian disease) is a disese of hormonal imbalance. Due to imbalanace in the ladies hormones you get small multiple nodules in the ovaries which is the main cause of infertility. It is genrally shown in fatty ladies. Due to excessive fat your digestive system and blood purify sysytem leads to hormonal imbalance. For that. Avoid fast food, junk food, oily food, soicy food, rice, potato, etc heavy food. Take easily digestible food like green leafy vegetables etc. Drink at least 3 litres of water in a day. Regular do yogasana /pranayama / deep breathing exercise etc to reduce stress and anger. This is the most necessary in this case. You must try to manage stress and anger. 100 steps walking immidiately after meal. Take dinner 2 hours before sleeping time. Take enough sleep. Regular walking for an hour 5 days in a week. You must try your best to reduce your weight. Take. Breakfast = 7 am to 8 am lunch = 12 noon to 1 pm dinner = 8 pm to 9 pm. Pcod is a such life style disease. Now a days obesity and stress increase the numbers of patients of this disease. So it is necessary for you follow above all instructions.

Most fruit ful days are from 8 day of cycle to 18 days of cycle. Ovulation occur in this period only. Intercourse should be done at least on alternative day during this period.

What are Vitamin D and Calcium?

Vitamin D (a hormone) and calcium (a mineral) are nutrients that sustain healthy bones. They are also needed for:

  • Muscle movement

  • Nerve communication

  • Absorption of calcium and phosphorous (vitamin D)

  • Immune system responses (vitamin D)

  • Signaling between cells (calcium)

  • Hormonal secretion (calcium)

  • Blood vessel flow (calcium)

Without enough vitamin D or calcium, your parathyroid glands compensate by producing too much of their hormone, a condition called hyperparathyroidism. That can lead to bone weakening (osteoporosis) and increased fracture risk. 
Other problems from calcium and vitamin D deficiencies include:

  • Skeletal deformities (rickets) in children ages 6-24 months

  • Muscle weakness in children and the elderly (vitamin D only)

Given the crucial role of both nutrients in bone health, The Endocrine Society and the Institute of Medicine recommend certain consumption levels based on age and health. They have not yet found, however, that taking vitamin D provides cardiovascular protection.

The recommendations come with two precautions:

  • Some people may need more than the RDA (after talking with their doctor) if they are:

    • Obese

    • Taking anticonvulsant medications, glucocorticoids, antifungals such as ketoconazole or medications for AIDS

  • Taking too much of either nutrient appears to be harmful, with:

    • Kidney stones associated with too much calcium from supplements

    • Very high levels of vitamin D (above 10,000 IUs per day) potentially causing kidney and tissue damage

How Do You Get Vitamin D and Calcium?

Your body makes Vitamin D when your skin is exposed to sun, but several factors limit its creation:

  • Living anywhere in the country above latitude 33 degrees (the top of Louisiana)

  • Wearing sunscreen to protect against melanoma

  • Aging, which changes absorption ability

  • The amount of sun you would need to achieve normal blood vitamin D levels is probably more than is safe for your skin, so most people may need supplements to achieve a normal vitamin D level.

Either form of vitamin D (D2 or D3) benefits the body, but very few foods naturally contain the nutrient or are fortified with it. That’s why doctors recommend supplements to make up the difference. Foods containing vitamin D include:

  • Cod liver oil: 400-1,000 IU per teaspoon

  • Wild caught salmon: 600-1,000 IU per 3.5 oz

  • Farmed salmon: 100-250 IU per 3.5 oz

  • Canned salmon: 300-600 IU per 3.5 oz

  • Canned sardines: 300 IU per 3.5 oz

  • Canned mackerel: 250 IU per 3.5 oz

  • Canned tuna: 236 IU per 3.5 oz

  • Fresh shitake mushrooms: 100 IU per 3.5 oz

  • Sundried shitake mushrooms: 1,600 IU per 3.5 oz

  • Egg yolk: 20 IU per yolk

Milk, orange juice, infant formula, yogurt, margarine, butter, cheese and breakfast cereals are often fortified with vitamin D.


Calcium is found in:

  • Dairy products

  • Chinese cabbage

  • Kale

  • Broccoli

  • Fortified fruit juices, drinks, tofu and cereals

If you avoid pregnancy donot do sex in unsafe period. Safe period is 1st day to 7th day and 20 th day to strat of next mc. You can sex without nirodh safely. During unsafe days use nirodh with cautions.
