I am Dr. Preeti Doshi. I am the in charge of pain clinic at Jaslok Hospital and Research Centre, Mumbai.

In a pain clinic, we generally advocate a multimodal and multidisciplinary approach to get the best possible outcome. These patients can be offered multiple different modalities to get the best possible outcome.

So, we start mainly with pharmacotherapy in the form of various drugs as appropriate for the type of pain that the patient is present with. Besides this, we also got a spectrum of intervention right from basics to the advance intervention which can be offered to the patients as appropriate for their particular pain syndrome.

So, here we target the nerves or the pixies which are responsible for causing this particular pain and the interventions are done ideally under image guidance and nowadays we definitely prefer to offer interventions under image guidance in form of fluoroscopic or ultrasound guidance. This is to reach the target precisely with minimum side-effects. We also have physical, psychological and occupational therapies and several other complementary therapies.

Overall we can achieve the best outcome if we combine more than one modality depending on individual pain syndrome. So, the most common interventions that we perform as pain specialist are directed towards the spinal pain such as low back pain in which we most commonly perform transforaminal epidural injections under guidance or facet joint injection in form of medial branch diagnostic block or frequency ablation. And these are all performed under local anesthesia and on day care basis.

So, these patients can go home within a couple of hours after these procedures. Besides this, we also have several other options as required for the patients with failback surgeries, we perform a procedure called Epidural Adhesiolysis because they are found to often have fibrosis and scarring around the nerves after having undergone spinal surgery. If the patient fails to respond to the conservative line of management and they have severe pain, we also have the option of advanced pain therapies in the form of neurostimulation or intra-thical drug delivery system.

They have a good success rate with a good evidence in literature for the use in common clinical practice. The complications are very rare if it's been performed with all the due precautions. However, there is the remote possibility of infection occasionally or hematoma or nerve injury and the patient have to be explained about this but these are extremely rare performed by an experienced operator with all due precautions.

I am 46 years old female. I suffer from extreme body aches. I feel my body stiff most of the time. I remain almost normal in winters but my problem aggravates in summer. I have multiple fibroids and taking medicine for hypothyroidism since 8-9 yrs, are these the reasons of my problem. When ever I consulted a doctor, I am subscribed multivitamins, calcium and painkillers. These give temporary relief. Also I am overweight, struggling to keep it in the normal range. Get your self proper check up done by your family doctor who will find out the basic cause of body ache, he will advise to get blood test done for any other problem, get hot water bath and avoid cold.

How To Effectively Manage Various Types Of Pains?

The human body goes through aches and pains from time to time depending on injuries and other kinds of joint, muscle and nerve pain. The various types of pain include somatic pain, visceral pain, and neuropathic pain. While somatic pain refers to pain that comes from injuries caused to muscles, connective tissues and joints; visceral pain happens due to inflammation and excessive stretching of organs.

Finally, neuropathic pain is a more long lasting kind that can be caused by conditions like cancer, HIV or Shingles. The pain can be acute or chronic.

Somatic Pain: This bone pain is usually localised and can increase with movement, becoming sharper with increased motion. Somatic pain can usually be treated with the help of analgesics that can treat the pain area, especially for pain that takes place due to surgical incisions and other dull aches or sharp pains that are localised. This kind of pain can also be treated with physiotherapy, acupressure and other exercises based on the area of the pain.

Visceral Pain: While this kind of pain can be treated with the help of medication, it also usually improves and fades with time. The medication for this kind of pain usually alters the neurotransmitters of the brain that convey pain signals and trigger pain in the body. This medication also helps in bringing down the inflammation for pain improvement.

Neuropathic Pain: This kind of pain is usually accompanied by irregular limb movements and discomfort as well as motions like twitching. It is a more permanent kind of pain that lasts long and causes greater discomfort than the above two kinds of pain. Neuropathic pain can be treated with the help of anticonvulsants that usually help in the prevention of nerve pain and irregular movements that can trigger off this kind of pain. It also acts on the serotonin levels and other chemicals of the brain for better nerve pain management and as an anti-depressant as well.

The various kinds of pain discussed above can be treated in different ways, in medicated and non-medicated ways. The method will also depend on whether the pain is acute or chronic.

A month back while getting down from my sons bunker bed I landed my left leg on the chair's leg frame. Since then I have heel pain.
