Hot & Cold Therapy - How It Can Treat Pain?

Hot and Cold therapy is one of the most popular home remedies for any kind of joint pain or muscle sprain. In some cases applying heat therapy and cold therapy alternatively, can help relieve the pain while for others only one form of therapy is necessary. To understand what to use when, one should first understand the effect of heat and cooling on the body.

Applying a heat pack or immersing the affected limb in hot water improves blood circulation and makes the muscles more flexible. Thus, it can help relax a muscle and fasten the healing of damaged tissues. On the other hand, cold therapy or cryotherapy reduces blood flow to the affected site. This helps relieve inflammation and pain caused by it. It can also make the area feel numb by reducing nerve activity temporarily.

Types of Heat Therapy
Heat therapy can be categorized as dry heat and moist heat. Dry heat involves using saunas or dry heating packs to relieve pain. This is also known as conducted heat therapy. As the name suggests, moist heat involves the use of steamed towels, hot baths and moist heating packs. This is also known as convection heat therapy. Moist heat is usually considered more effective than dry heat.

Types of Cold Therapy
Cold therapy may be applied to a part of the body or to the entire body. This includes ice packs, coolant sprays, ice baths, ice massages and whole body, cold therapy chambers.

How to use heat and cold therapy?
The sooner heat and cold therapy is given to a person, the faster they will recover from their pains. Heat therapy can be given to a person for extended periods of time but when giving cold therapy, a person should not be exposed to a cold pack or ice bath for more than 10-15 minutes. Care should also be taken no to apply ice or a frozen pack directly on the skin as it can damage the skin and nerves.

When not to use Heat Therapy?
If the injury site is swollen or bruised, cold therapy is better than heat therapy. Heat therapy should also not be applied to an open wound. People suffering from conditions such as diabetes, dermatitis, deep vein thrombosis, vascular diseases and multiple sclerosis should also avoid heat therapy as these conditions can increase the risk of burns and other such complications. Pregnant women should also consult a doctor before using a hot tub or sauna.

When not to use Cold Therapy?
Cold therapy is avoidable for treating stiff joints and muscles. People suffering from poor circulation and sensory disorders should also avoid using cold therapy without a doctor’s supervision. If you wish to discuss any specific problem, you can consult a pain management specialist.

Ginger can help reduce inflammation of the blood vessels in the head, thus providing relief from a headache. Mix equal parts of ginger juice and lemon juice. Consume it once or twice a day. Another option is to apply a paste of one teaspoon of dry ginger powder, also called sonth, and two tablespoons of water on the forehead for a few minutes.

Avoid bending in front. Postural correction- sit tall, walk tall. Apply hot fomentation twice daily. Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretching. Do the cat/cow stretch.

My son today faced back pain. Consulted doc. His fever is 100.6, doc has asked for urine test to clear the doubt of kidney infection and also prescribed Syrup Dolopar MDS. What is your opinion sir? If doc's doubt is correct then is it curable with medicines or it would be any serious problem. What is preferable diet for my son. Please advice sir.

You have correctly been told to get urine examination done first. Urine complete & culture sensitivity. What is more important, please get your son treated with course of antibiotics first. He should drink lots of water. Give two times soda plain for alkalinisation of urine alongwith. No need to worry if it is infection. But in case he doesn't get well then must go for ultrasound abdomen to rule out any stones. Since I have not examined your son rarely it could be spine problem. But that is usually least possibility in younger age group. If you get your son treated in time you can avoid lot of problems for him.

Shoulder Pain - Know The Reasons Behind It!!

The shoulder is one of the most important joints in the body that uses a ball and socket to join the arm to the rest of the body. Any pain can make it difficult to carry out motion in a comfortable manner. The shoulder consists of the long arm bone called the humerus, the collar bone or the clavicle, as well as the shoulder blade called the scapula. A layer of cartilage provides essential padding to the bones in this area and its two main joints. There are a number of issues that can cause shoulder pain. Let us get to know the six most common ones. 

  1. Rotator Cuff Injury: This is the most common cause of shoulder pain and it occurs when the four muscles or the tendons are injured. These muscles and tendons usually control the movement of the arm. This injury can involve strains and tears which may also be caused by constant lifting of heavy objects. 
  2. Rotator Cuff Tendinitis: This condition affects the tendons and muscles that help in moving the arm freely. This usually happens due to inflammation in the tendons. It is commonly experienced by patients who are actively involved in sports and other heavy physical pursuits in their line of work. 
  3. Frozen Shoulder: This is a debilitating condition that restricts the free movement of the arm and is also known as adhesive capsulitis. When the shoulder tissues become too thick, it leads to the growth of scar tissue that hinders proper movement and causes pain. 
  4. Osteoarthritis: Osteoarthritis is a condition that comes as part of aging bones that also become hollow and brittle over the passage of time, which leads to joint and bone pains in various parts of the body. It is a degenerative disease that is caused due to prolonged wear and tear as well as other factors including family history and sports injury.
  5. Bursitis: This condition is known to affect the fluid filled sacs that lie in between the joints. These are known as bursae. When the bursae suffers from swelling, the effect usually shows up as pain in the joints. 
  6. Tendon Sheath Inflammation: The fibrous tissue that connects the bones and the muscles is called a tendon. The presence of the tendon makes it possible to pursue activities like running, jumping, lifting, gripping and more. It is protected by a sheath known as synovium, which also lubricates the same. Any injury or tear to this sheath can cause shoulder pain.

Acute and persistent pain that does not abate and gets even worse should be treated with immediate medical intervention. If you wish to discuss about any specific problem, you can consult an orthopedist.

Hello lybrate-user the kind of pain your parents experiencing are because of aging, it is generally called as osteoarthritis. If its on its early stage it could be curable with proper physiotherapy and if you want permanent relief you could go for stem cell treatment, if joints are deteriorated than joint replacement is the only option left. Sleep on a hard bed with soft bedding on it. SPRING BEDS, FOLDING BEDS OR THICK MATRESS ARE HARMFUL Use no pillow under the head. DO HOT FOMANTATION. Paracetamol 250mg OD & SOS x 5days. Caldikind plus 1tab OD x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again.. Do not ignore .It could be beginning of a serious problem.

Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

She is most probably having condition named plantar fascitis. Treatment involves footwear with soft soles or silicon soles (can get them at any surgical store) along with combination of diclofenac with seratiopeptidase for 10 days twice daily after food.

What to do?
You have it seems developed a neck strain due to maybe bad posture while sleeping etc. If you have localized neck pain with no other symptoms like radiating upper limb pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your neck while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medications which suits you, you can also add a muscle relaxant for relief of muscular spasm - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight specially overhead, sleeping with maintenance of proper posture and using a firm to hard mattress etc. We will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we might have to get some investigations like dynamic x rays of lower back and few blood tests for evaluation. Hello Abhimanyu, Waist pain every winter can be mostly because of some old injury. In case you have suffered any fracture, it is bound to hurt every winter. However , in order to rule out any other pathology, I would suggest you undergo an X Ray of the lumbar spine in AP and Lateral view which will show any deformity or abnormal calcification. Please share the reports and accordingly I will be able to suggest the further course of action.

You should take Arnica 30 thrice a day three drops in water one day only. After that Rhus Tox 30 thrice a day for two day. Do massage of calf muscles from downward to upward daily at night for five days. Take vitamin D once a week for a month.
