Hello doctor. I am 21 years male boy. My question is that my doctor advised me to insert pentasa suppository in rectum and at least for remain it inside for 10 hours but on next morning when I go for motion .the medicine comes out in the form of liquid with potty. It is normal or not.

Hello lybrate-user, Any suppositories inserted in the rectum is bound to come out, unless it gets absorbed by the rectal mucosa. It would be helpful to know what are your actual symptoms and problems. Get a surgical consult. Hope this helps. Many thanks.

My mother have kidney infection from the past 2 years and the creatinine level is now 5.5. I want to know how to reduce this. On home by diet and other daily routine.

The condition you are describing is very grave and the creatinine level also suggests date. For further advice I will need to know her full medical history with her symptoms and will have to see her reports of blood and urine analysis n usg. My anal rim has become rigid. It also has small swellings. It used to come and go. But these days it's been for a long time and it has really become painful for me to go to toilet. I need a permanent solution for this problem.

It appears to be prolapse pile or fissure with sentinel pile or a combination of both. A general surgeon must examine you to arrive at the correct diagnosis & plan treatment, which may include the option of surgery. The general measures I would suggest are 1. Take lot of vegetables & fruits in your diet 2. Avoid chillies & spices 3. Take lot of fluids 4. Avoid constipation 5 periodically do anal exercises (alternately contracting & relaxing the buttocks, anal area & upper thighs) 6. Take sitz bath 3-4 times a day. This means sitting in a tub of warm water & doing anal exercises 7. Atleast once get it examined by a general surgeon.

I have in-large prostate since last 10 years and taking one Alfatam-D tablet regularly since then. Controlling urination sometimes becomes very difficult when pressure built up. I become restless. Why is this so? Please advise. Anand Kumar.

Dear, enlarged prostate causes the bladder muscles to work harder to push urine out. Over a long period of time, this leads to thickened bladder wall and overactivity of bladder muscle. Once overactive bladder develops, it can act spontaneously without our voluntary control. This is the common reason for not able to control urine in prostate enlargement. Other reasons are bladder stone or infection. Please get an ultrasound abdomen with post-void residual urine. Based on prostate size, bladder wall, residual urine, serum psa and urine routine examination findings, a urologist will be able to treat you better. If you do not have much residual urine, infection or stone, then you can be started on drugs to relax bladder (anti-cholinergics). This will help you with improving the control of urination.

Frequent urination usually occurs in diabetes due loss of glucose in urine as it pulls more water in to bladder due to osmosis, frequency also increases in urinary tract infection, but then colour changes to dark brown or reddish and invariably associated with burning pain. Some times anxiety can also mimic such symptoms.
