If there is fear that one might get heart attack and there are risk factors then get her investigated do ecg, echo, blood sugar, lipid profile and thyroid hormons. In case of heart attacl pt should be immediately transferred to a hospital with cardiac facilities.

My age is 33. I am facing high bp 140/95 since more than 6 months. I visited the doctor last week and he has prescribed Pinom 40 mg. Is it ok to start with such high dose.

Starting treatment is ok but you need to rule out secondary hypertension. Some baseline investigations like KFT, urine analysis, usg abdomen, ECG, echocardiography are required.

We all know about it. In simple words, we can say damage to the brain from interruption of its blood supply. It’s also called CVA: cerebro vascularaccident. Now just have a simple look or idea about the patient’s structure means his look so that he can recognize about that. Firstly side of the face drops or feels numb with speech difficulties as face involves in it, then muscles weakness on one side of the body.

 There are three types of stroke:

1 Ischemic strokes 

2 Hemorrhagic  strokes 

3 Transient ischemic attacks (TIAs) 

BUT there is also something that we should know:-

There are two types of patients, first those patients with both flexor and extensor  spasticity; but predominant extensor hyper tonus of the leg, i.e.excessive co­-contraction.  

They can stand and take some weight for a moment.Other patients with more moderate degrees of spasticity and little co-contraction can walk and move the weight free leg.Both types of patients have balance problem, the first because of lack of mobility and the other due to lack of stability.

NOTE: If left brain involved then right side affected and if right brain involved then left side.

TREATMENT FOR STROKE

Before start treatment one should know about the following things. 

They are:·        

Check the muscle first either it’s flaccid or spastic.       

Sensory part involvement is there or not.     

Joints involved.       

Check the reflexes

PHYSIOTHERAPY APPROACHES: 

Bo bath: This is based in normal movement or Neuro -development approach.The main aim is to prevent abnormal movement and adverse plastic adaptations and to facilitate normal movement and subsequent plastic changes.

Brunstorn: He makes use of abnormal synergies and incorporate them into functional activities.

Motor relearning program: This training of motor control is based on an understanding of kinematics and kinetics of normal movement, motor control process and motor learning. 

Proprioceptive neuromuscular facilitation (PNF): To maximize sensory stimuli on the pool of AHC in order to stimulate purposeful muscle contraction.

Roods: This was mainly done to achieve purposeful muscular contraction by stimulating the skin through facilitating strokes on the skin. The combination of treatment used differs from patient to patient.

Respected doctors, What should one do in emergency 1) if an old man or women aged 45-53 get some severe chest pain and also breathing problem? 2) and also if suddenly get heart attack or something like that or loose consisnous? Which advice for both above situation? Thanks.

Get them to nearest doctor first to get ecg done immediately.. Till that time ask the patient to cough voluntarily so that the pressure generated due to coughing keeps adequate blood supply to brain...and he does not become unconscious

Dear lybrate user! greetings the leg swollen is common in bp and over weight person. Ask you mother to keep her legs above the hip level while sleeping and sitting with help of pillows to leg. Ask her to control salt in food ask her avoid salty food like dry fish, papad, pickles, chips, etc ask her to go for walking 30 min daily. Ask her to take the medication regularly.

These factors increase your risk for another heart attack: inactive lifestyle being overweight or obese high cholesterol high blood sugar, if you have diabetes high blood pressure smoking excess stress metal stents, devices placed into an artery to keep the vessel open, have been used in lieu of balloon angioplasty to treat patients with coronary artery disease (cad) for about 15 years, due to their better long-term patency. Nonetheless, with conventional (" bare metal") stents, there may be a disease recurrence rate of at least 20% to 30% due to scar tissue formation within the stent. Much research has been done to try to prevent this scar tissue formation. In the past decade, newer metal stents coated with medication to prevent scar tissue formation have become widely available. These stents have reduced the recurrence (" restenosis") rate of coronary artery disease to well under 10%. These percentages are vlid only if you take proper precautions to avoid risk factors for heart attack as mentioned above and you keep taking your prescribed medicines regularly.

I am 50 yrs old and physically fit since I am a regular jogger. I get myself medically checked every six months. I did not have any problem with any parameter till last month when my total cholesterol level came as 264 with all parameter within limit except LDL at 180 which is high, Please advice me as to what should I do to bring this level within limit.

high cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. you should be avoid bad habits ,fatty meal/oily foods/junk/fast foods/alcohol/smoking/noo-veg. take whole grain meal/oats/dalia/green veg./fruits/juice/ use garlic . you can take also cap. ecosprin gold-10 at night. and consult cardiologist.

Treatment for high cholesterol is lipid lowering drugs. But it is prudent to rather treat the cause than the effect. Which means if he avoids alcohol eats low fat diet and exercises regularly he might not need to take medicines.
