1. There is no treatment that eradicates hepatitis b infection completely. 2. However treatment is available to arrest the liver damage and subsequent development of cirrhosis of liver/carcinoma of liver 3. In most of the cases the hepatitis b virus automatically gets cleared from the blood within 6 months without any treatment. 4. If the virus persists after 6 months it is known as chronic hepatitis b 5.30-90% of patient suffering from chronic hepatitis b subsequently develop cirrhosis of liver/ carcinoma of liver if not treated 6. You have to regularly do the blood test to monitor hbsag, hbeag hbv dna, real time pcr for viral load (rapid diagnostic test), liver function test for sgot/sgpt criteria for starting anti viral drugs a) patient should be observed for 6 months. If sgot (alt/sgpt (ast) level is more than two times the normal value), hbeag is positive, hbvdna is more than 20, 000/ml, and liver biopsy showing necro inflammatory changes, he may require treatment with anti viral drugs. Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

65% of Gluten-Free Celiacs Still Have a Raging Fire in Their Gut

The same 2009 study in The Journal of Alimentary Pharmacology and Therapeutics of 465 Celiac Disease patients 16 months gluten-free found that 65% still had ?persistent intraepithelial lymphocytosis,? a.K.A. Inflammation in the gut.[4]
Their intestines are on fire with inflammation even after 16 months gluten-free. Why is that important?
We know gut inflammation is associated with a laundry list of health issues, including cancer and early death. That?s bad news for the conventional Celiac prescription and even worse news for the people not getting better on a gluten-free diet. Want more evidence gluten-free doesn?t put the fire out?
A 2008 study in the Journal of Inflammation looked at 18 symptom-free Celiac Disease (SFCD) patients and found they still had elevated markers of gut inflammation even after 2 years on a gluten-free diet. The authors reported:
?Faeces of both active CD and SFCD (symptom-free 1-2 years on a GFD) patients, representing an imbalanced microbiota, significantly increased TNF-alpha production and CD86 expression in PBMCs, while decreased IL-10 cytokine production and CD4 expression compared with control samples.? [5]
In another 2009 study from the American Journal of Gastroenterology, researchers looked at small intestine biopsies from 45 children with Celiac Disease and 18 clinical controls. The authors found an increased presence of T cells (inflammatory marker) in well-treated CD patients:
?The long-lasting presence of high frequencies of T cells in the epithelial compartment in well-treated CD indicates that the epithelium is stressed possibly because of constant attack.?[6]
Both of these studies looked at patients that are supposed to be ?healed?? supposedly ?well-treated.? Even though they appeared to be symptom-free, the medical tests paint a much different picture. These asymptomatic adults and kids still had inflammatory fires raging in their gut? promoting further disease development (like Cancer).
So far this research has only reviewed patients following a gluten-free diet for 1-2 years? but what about long-term? Does the body just need more time to heal and get back to normal?

Ibs is due to alteration in neurological and gut co ordination. Ibs before being pronounced to a patient needs to be diagnosed correctly because there are many colonic diseases which mimics ibs. But once diagnosed, one need to be calm, stress free, change unhealthy to healthy lifestyle, avoid food which causes diarrhea/ constipation and be on supportive medications prescribed by clinician. One also need to have a follow up.

Dear lybrate user, you may be suffering from a clinical condition called phimosis. The most conventional method to get rid of it is surgery. However, if you are unwilling to undergo surgery then you can try the following process at home. 1) buy any local anaesthetic gel (e. G. Lidocaine, lignocaine, xylocaine) from allopathic retail shop. 2) apply a little amount of that anaesthetic gel at the lower part of the prepuce of penis where the foreskin remains attached. 3) after application of the gel, retract the foreskin backwards as far as you can. Due to the anaesthetic property of the gel you will be able to retract the foreskin backwards to some extent more than you can do in normal condition. 4) release the foreskin when you feel pain 5) again repeat the same process. Do it continuously for at least 30 minutes in a day. Within a month you will feel that your foreskin has become loose.

Sperm Quality: Is it Really on the Decline?

MBBS
Sexologist, Gurgaon
Sperm Quality: Is it Really on the Decline?

Though the world population seems to be increasing by leaps and bounds, there is a general concern about the dropping fertility rates in men. So, is the sperm quality of men today really at a risk?

To understand whether the sperm quality is on a decline or not, one must understand what sperm quality is and what affects it.

What defines Sperm Quality?

The ability of the sperm to accomplish fertility is known as sperm quality. Both quantity and quality of semen is important when it comes to measuring the male fertility rate. Any decrease in the sperm quality, irrespective of the quantity, can result in infertility. One can do a number of tests, such as semen analysis test, to find out their sperm quality, in turn getting an idea of how fertile they are.

Factors that cause a decline in sperm quality

There are a large number of factors that can easily affect the sperm quality in men. Some of the major determining factors are:

1. Age - Though there is no age limit for a man to father a child, but as men grow older, there is a significant drop in sperm quality as sperm motility decreases with age.

2. Heat - Sperms are highly heat sensitive, so at high or very low temperatures, the quality of sperms decline.

3. Diet - Having an unhealthy diet based on junk food does decrease sperm quality dramatically, primarily due to the unhealthy levels of chemicals added to the food.

4. Smoking - Smoking tobacco, as well as cannabis, reduces sperm quality.

5. Chemicals - Chemicals, whether taken intentionally or unintentionally, can cause a decline in sperm quality. Chemicals such as endocrine disruptors can cause severe hormonal changes that can change the sperm quality in any healthy male.

Apart from these major factors, an excess of physical and mental stress also influence sperm quality.

What is the Sperm Quality today like?

From the factors that affect sperm quality, one can get an idea of whether their sperm quality is good or bad. Your lifestyle can be responsible for your semen quality as well. The diet you have and the amount of exercise you get directly affects your sperm quality. The chemicals that you allow into your body also affect the semen quality. At the same time, research shows that even among healthy men, sperm quality is on a steady decline.

Is it about the everyday plastics?

Not always can you be in charge of your environment. Everyday plastic like phthalates, which are used to make daily use products such as shower curtains, car dashboards and cleaning materials, affects sperm quality. Even the chemicals in sun skin lotions can have a degenerative effect. So, the sperm quality in men is truly on a steady decline today due to chemicals in the environment as well as unhealthy lifestyle choices that most men make today.

BHMS
Homeopath, Raebareli
The first home treatment that is highly recommended is ice. Ice will help constrict the blood vessels, reduce swelling and give you immediate pain relief. •Apply an icepack or a partially melted piece of ice wrapped in a cloth directly on the affected area for about 10 minutes. Do this several times a day until the hemorrhoid goes away. •You can even use a small pack of frozen vegetables such as peas or corn. Wrap the pack in a tea towel and then apply it on the hemorrhoid. Do this for 20 minutes at a time, three times a day. You need proper Homeopathic treatment for your disease to cure. Consult me for homeopathic treatment.

Do not worry about size as far as you get good erection and timngs. Women vagina initial 2 inches is sensitive so size aythg bigger than 2 inch is sufficient. Imprtant is good erection and timings. Consult for detailed discssion and solve your erection and timing issues. Excessive of anything is bad same applies with masterbation. 2 things happen when one masterbate excessively 1. Premature ejaculation you can correlate when you started mastrrbation what was time and check current timing. You will find time to ejaculate might hv reduced significantly. Some people even ejaculate while rubbing in bed 2. Second problem is weak erection. Over a period you will feel lesser or weaker erections. One may feel shrinking penis due to this. If your erection and timing is still good then do not worry. Otherwise with guidance and ayurvedic medicines it can be cured. Consult for treatment.

Retinitis Pigmentosa: What You Should Know About It?

Diploma in Ophthalmology, MBBS
Ophthalmologist, Hyderabad
Retinitis Pigmentosa: What You Should Know About It?

Are you losing your sight day by day? Does it make difficult to see you at night? Is this the onset of Blindness? Get to know about Retinitis Pigmentosa with these tips.

What actually is Retinitis Pigmentosa?
Retinitis Pigmentosa (RP) refers to a group of diseases, which causes a slow, but progressive vision loss. It is a genetic disorder that affects the ability to respond to light. This inherited disease causes a slow loss of vision, beginning with decreased night vision and loss of peripheral (side) vision and eventually results in blindness.

Causes: Retinitis pigmentosa is often hereditary (runs in families). If you or your partner has retinitis pigmentosa, there may be up to a 50 percent chance that you will pass it on to your children. Ask an ophthalmologist about genetic counseling if you are planning to have children.

Symptoms

  1. Slow loss of vision
  2. Beginning of decreased night vision
  3. Loss of peripheral vision
  4. Difficulty in identifying colours

Diagnosis: If you have poor night vision or a loss of side vision or if there is a history of retinitis pigmentosa in your family, your ophthalmologist will conduct a comprehensive eye exam to determine if you have retinitis pigmentosa. Your ophthalmologist will dilate your eyes to look at the back of them for signs of disease.

Treatment: Currently there is no known cure for retinitis pigmentosa. However, research has shown that vitamin A palmitate may slow the progression of certain forms of RP. Your ophthalmologist can advise you about the risks and benefits of vitamin A palmitate and how much you can safely take. Taking too much vitamin A palmitate can be toxic, and evidence of vitamin A palmitate effect on RP progression is not substantial.
Another recommendation for slowing vision loss from RP is to wear sunglasses to protect your retina from harmful ultraviolet (UV) light.

Retinal prosthesis is also an important area of exploration because the prosthesis, a man-made device intended to replace a damaged body part, can be designed to take over the function of the lost photoreceptors by electrically stimulating the remaining healthy cells of the retina.
